Background: Anorectal malformation is a congenital abnormality that shows imperfect anus or absence of anus. The most common complication in patients with anorectal malformations after posterior sagittal anorectoplasty (PSARP) are disorders in defecation, especially constipation. Method: The purpose of this study was to determine the correlation of quality of defecation measured with the presence of constipation with the type of PSARP surgery in Arifin Achmad Hospital of Riau Province. This study used an analytical retrospective method from November 2018 to March 2019. Total sampling was the sampling method used in this study, which accounted for a total of 28 subjects. The population of this study was all patients with anorectal malformation and fistula in the Pediatric Surgery Division of Arifin Achmad General Hospital. Result: This study showed that among 28 cases found, there were 19 female patients (68%) and 9 male patients (32%). Based on the lesion level, there were 17 cases (60,7%) of intermediate lesions and 11 cases (39,3%) of high lesions. In this study, rectovestibular fistula had the highest incidence among other types of fistula (53,6%). Constipation were found in 53.6% patients with p-value of 0.025 (OR 6.40, 95% CI 1.183-34.613). Conclusion: There was a significant correlation between the PSARP method and constipation in patients who underwent anorectal malformation surgery, which constipation was a sign of the quality of defecation.
Acute appendicitis always need surgical treatment. If appendectomy is not performed immediately, perforation of appendix and peritoneal contamination will complicate the disease. The highest incidence of appendicitis is in late teen, with 5% incidence in children under 5 years old. The aim of this study was to determine the correlation between onset of abdominal pain with the severity of pediatric acute appendicitis and to know the different of average leukocyte count of pediatric acute appendicitis between appendicitis and appendicitis complication. This study was analytical comparative and analytical correlative with retrospective approach. About 68 medical records of pediatric acute appendicitis patient in Arifin Achmad General Hospital Riau Province period January 2018 – December 2019 were analyzed. The result showed appendicitis case in children was same affected girls and boys mostly in group age 6-12 years old. Onset of abdominal pain was mostly after 48 hours of the disease. About 69.1% patient was suffering of appendicitis complication. Leukocytosis was found in 76.5% patients. Statistics analysis using unpaired t test showed p=0.00, it meant a significant different of average leukocyte count between appendicitis and appendicitis complication. IC95%=4015.7-6683.1. Statistics analysis using Somers’d correlative test show p=0.00. From this study, it can be concluded that there was a significant different of average leukocyte count between appendicitis and appendicitis complication also there was a significant correlation between onset of abdominal pain with severity of pediatric acute appendicitis with strong power correlation.
ABSTRAKPenyakit Hirschprung adalah suatu kelainan bawaan berupa aganglionosis usus, mulai dari sfingter ani internal ke arah proksimal dengan panjang yang bervariasi, bersifat kosmopolitan, dengan insiden berkisar 1 diantara 2000 sampai 12.000 kelahiran. Penelitian ini bertujuan untuk mengevaluasi hasil luaran operasi pulltrough pada Hirsprung dengan skor Klotz periode januari 2010-Desember 2016, meliputi hubungan status gizi dengan hasil luaran dan komplikasi yang timbul. Penelitian ini dilakukan dengan rancangan deskriptif analitik dengan pengumpulan data secara retrospektif terhadap penderita Hirschprung yang dikerjakan terapi definitif dengan metode operasi pulltrough, data diambil dari rekam medis dan kuisioner di RS Arifin Achmad periode Januari 2010-Desember 2016. Dua puluh sembilan pasien terdapat 16 (55,2 %) pasien laki -laki dan 13 (44,8%) pasien perempuan. Terdapat 21 (72,4%) dengan gizi baik dan 8 (27,6%) dengan gizi kurang. Komplikasi yang terbanyak ditemukan adalah enterokolitis 5(17,2%) diikuti dengan stenosis rectum 1(3,4%). Terdapat hubungan bermakna komplikasi pasca operasi pulltrough dengan evaluasi skoring Klotz dan tidak terdapat hubungan bermakna antara status gizi dengan evaluasi skoring Klotz.
Background: Bochdalek hernia is a congenital defect on posterolateral diaphragm with an abnormal connection between the thoracic cavity and the abdominal cavity. This disease causes protrusion of abdominal organs into the thoracic cavity. Case: an 8-day-old baby girl admitted to hospital with shortness of breath 24 hours after delivered. The baby was born spontaneously assisted by midwife. Upon born, the baby was crying strongly and meconium came out 2 hours after birth. On physical examination, the abdomen was inspected flat. Darm contour and darm steifung was observed, and peristaltic sound was heard on left lung. Radiological examination demonstrated a diaphragmatic hernia with ileus obstruction. The patient underwent laparatomy and stomach, ileum, transverse colon, and spleen, was found on foramen Bochdalek. Post-surgery chest X Ray showed favourable result. Ten days after treatment, the patient was discharged in a good condition with no respiratory or digestive problems. After 1 months the patient’s condition remained good and there were no respiratory or digestive complaints. Conclusion: In a rare case like Bochadalek hernia, laparotomy performed as a promising attempt to return the anatomic position of organ.
