Head lice infestation (HLI) caused by Pediculus humanus capitis has been a public health problem worldwide. Specially designed combs are used to identify head lice, while anti-lice products are applied on the scalp for treatment. In the present study, we aimed to test whether combing only by precision detection comb (PDC) or metal pin comb (MPC) could be effective alternatives to the use of anti-lice products in children. A total of 560 children from two rural schools in Turkey were screened. In the PDC trial, children were combed every second day for 14 days, while in the MPC trial, combing was performed once in every four days for 15 days. Children were divided into two groups (dry combing and wet combing) for both trials and results were compared. The results showed no significant differences between dry and wet combing strategies for both combs for the removal of head lice (p > 0.05). The number of adult head lice declined significantly on each subsequent combing day in both approaches, except on day 15 in the MPC trial. In the end, no louse was found in 54.1 and 48.9% of children in the PDC and MPC trials, respectively. Since family members of infested children were not available, they were not checked for HLI. Four times combing within 2 weeks with MPC combs was found effective for both treatment of low HLI and prevention of heavy HLI. In conclusion, regular combing by special combs decreases HLI level in children and is safely applicable as long-term treatment.
ÖzetSağlık Bakanlığı, amipli dizanteri etkeni Entamoeba histolytica'nın (E. histolytica) kesin tanısının konabilmesi için, standart tanı protokollerinde Trichrome boyalı preparatlarda eritrosit fagosite etmiş trofozoit görülmesi ve/veya E. histolytica spesifik adhesin antijeninin ELISA testi ile pozitif tespit edilmesi şartlarını aramaktadır. Ege Üniversitesi Tıp Fakültesi Hastanesi Parazitoloji Polikliniği'ne Ocak 2010 -Haziran 2011 tarihleri arasında başvuran ve direkt dışkı bakısında amebiasis şüphesi oluşan 51 hastanın, dışkı örnekleri Trichrome boyası ile boyanarak incelenmiş ve 49 örnekte E. histolytica/dispar tespit edilmiştir. Toplam 51 örnekten 33 dışkı örneğine E. histolytica adhesin antijeni aramaya yönelik ticari ELISA kiti (Entamoeba CELISA-Path; CeLLabs Pty. Ltd., Brookvale, Australia) uygulanmış ve 23 (%71.8) dışkı örneğinde pozitif sonuç elde edilmiştir. Sonuçlarımıza göre geçmiş yıllarla karşılaştırıldığında son yıllarda olgu sayısında artış olduğu dikkati çekmektedir. E. histolytica'nın kesin tanısının konmasında Sağlık Bakanlığı'nın da önerdiği gibi farklı yöntemlerin bir arada uygulanmasının özellikle perifer laboratuvarların etkin sonuç vermesinde önemli olduğu sonucuna varılmıştır.
Objective: It was aimed to develop a new Multiplex Polymerase Chain Reaction (PCR) protocol with isolates obtained from local patients for the diagnosis of Blastocystis sp., Cryptosporidium sp. and Giardia intestinalis, which can cause severe gastrointestinal system complaints especially in immunocompromised patients and children. Method: DNA isolation was performed with a commercial kit from three stool samples of different patients whose microscopic examination showed dense amounts of Blastocystis sp., Cryptosporidium sp. and Giardia intestinalis. First, a special PCR protocol has been developed for each protozoon. Then, the multiplex PCR protocol, in which these three protozoa can be diagnosed together, was optimized. Results: In the multiplex PCR protocol performed after DNA isolation, bands of 95 bp., 227 bp. and 258 bp. were obtained for Cryptosporidium sp., Blastocystis sp. and G. intestinalis, respectively. Conclusion: Blastocystis sp., Cryptosporidium sp. and Giardia intestinalis were diagnosed by multiplex PCR with the original protocol developed. Due to the difficulties in using different methods in parasitological examination, by adding other protozoa important for public health to this optimized protocol, it will be possible to detect a large number of parasites with a single molecular method.
ÖzetEge Üniversitesi Tıp Fakültesi Hastanesi Parazitoloji Polikliniği'ne Temmuz 2009 -Haziran 2010 tarihleri arasında akut ve kronik gastrointestinal sistem yakınmaları ile başvuran 236 hastanın dışkı ve selofan-bant örnekleri değerlendirilmiştir. Bu 236 hastanın 111'inin (%47) akut yakınmaları olup bunların daha çok yaz ve sonbahar aylarında hastaneye başvurduğu, 125'inin (%53) ise kronik gastrointestinal sistem yakınmaları ile kliniklere başvurduğu ve bu hastaların da daha çok yaz ve sonbahar aylarında geldiği gözlenmiştir. Parazitoloji laboratuvarına başvuran 236 hastanın 112'sinde (%47.45) en az bir paraziter etken saptanmıştır. Retrospektif olarak incelendiğinde parazitoloji laboratuvarında incelemesi yapılan 236 hastadan 121'inde (%51.7) eş zamanlı olarak bakteriyolojik dışkı incelemesinin de yapıldığı görülmüştür. Bakteriyoloji ve parazitoloji laboratuvarlarına aynı zamanda başvuran 121 hastanın 7'sinde (%5.78) en az bir etken bakteri saptanmıştır. Mikrobiyolojik etken saptanan 7 hastadan 6'sında ise bir paraziter enfeksiyon etkeni de tespit edilmiştir. Çalışmamızda saptanan çoklu enfeksiyonlar nedeniyle intestinal sistem yakınmaları ile kliniklere başvuran hastalarda, parazitolojik ve bakteriyolojik inceleme amaçlı Sağlık Bakanlığının Standart Tanı protokollerinin birlikte uygulanmasının tanı, tedavi ve enfeksiyonların birlikte görülebilme koşullarının tanımlanabilmesi için önemli olduğu sonucuna varılmıştır.

The Comparison of Parasitological and Bacteriological Stool Examinations in Patients with Gastrointestinal Symptoms
SummaryStool samples of 236 patients with acute and chronic gastrointestinal symptoms admitted to Ege University Medical Faculty Parasitology Outpatient Clinic Laboratory between July 2009 to June 2010 were examined. One hundred eleven out of 236 (47%) patients admitted to the laboratory during summer and autumn period with acute gastrointestinal symptoms while other 125 patients (53%) admitted during winter and spring with chronic symptoms. At least one parasite was determined in 112 out of 236 (47.45%) patients. In retrospective analysis, it has determined that the bacteriological examination of stool samples were also performed in 121 out of 236 (51.7%) patients. Seven out of 121 (5.78%) patients admitted both bacteriological and parasitological laboratories together had at least one bacterial agent in their stool examination. Five out of 7 patients were found to be having bacteriological and parasitological mixed infection. Our findings showed that it is important to perform both bacteriological and parasitological examinations together in patients admitting to hospital with intestinal symptoms due to these mixed infections. Performing both diagnostic techniques together will improve accurate diagnosis, treatment and understanding possible etiological reasons of these mixed infections.
