In modern electronic medical records (EMR) much of the clinically important data -signs and symptoms, symptom severity, disease status, etc. -are not provided in structured data fields, but rather are encoded in clinician generated narrative text. Natural language processing (NLP) provides a means of "unlocking" this important data source for applications in clinical decision support, quality assurance, and public health. This chapter provides an overview of representative NLP systems in biomedicine based on a unified architectural view. A general architecture in an NLP system consists of two main components: background knowledge that includes biomedical knowledge resources and a framework that integrates NLP tools to process text. Systems differ in both components, which we will review briefly. Additionally, challenges facing current research efforts in biomedical NLP include the paucity of large, publicly available annotated corpora, although initiatives that facilitate data sharing, system evaluation, and collaborative work between researchers in clinical NLP are starting to emerge. There are currently many NLP systems that have been 2 successfully applied to biomedical text. It is not our goal to review all of them in this chapter, but rather to provide an overview of how the field evolved from producing monolithic software built on platforms that were available at the time they were developed to contemporary component-based systems built on top of general frameworks. More importantly, the performance of these systems is tightly associated with their "ingredients" (i.e., modules that are used to form its background knowledge), and how these modules are combined on top of the general framework. We highlight certain systems based on their landmark status as well as on the diversity of components and frameworks they are based on. [7]. The review in this chapter differs from previous work in that it emphasizes the historical development of landmark clinical NLP systems, and presents each system in light of a unified system architecture.We consider that each NLP system in biomedicine contains two main components: biomedical background knowledge and a framework that integrates NLP tools. BackgroundExtraction of clinical information such as medications or problems from clinical text is an important task of clinical natural language processing (NLP). Rule-based methods are often used in clinical NLP systems because they are easy to adapt and customize. Recently, supervised machine learning methods have proven to be effective in clinical NLP as well. However, combining different classifiers to further improve the performance of clinical entity recognition systems has not been investigated extensively. Combining classifiers into an ensemble classifier presents both challenges and opportunities to improve performance in such NLP tasks.MethodsWe investigated ensemble classifiers that used different voting strategies to combine outputs from three individual classifiers: a rule-based system, a support vector machine (SVM) based system, and a conditional random field (CRF) based system. Three voting methods were proposed and evaluated using the annotated data sets from the 2009 i2b2 NLP challenge: simple majority, local SVM-based voting, and local CRF-based voting.ResultsEvaluation on 268 manually annotated discharge summaries from the i2b2 challenge showed that the local CRF-based voting method achieved the best F-score of 90.84% (94.11% Precision, 87.81% Recall) for 10-fold cross-validation. We then compared our systems with the first-ranked system in the challenge by using the same training and test sets. Our system based on majority voting achieved a better F-score of 89.65% (93.91% Precision, 85.76% Recall) than the previously reported F-score of 89.19% (93.78% Precision, 85.03% Recall) by the first-ranked system in the challenge.ConclusionsOur experimental results using the 2009 i2b2 challenge datasets showed that ensemble classifiers that combine individual classifiers into a voting system could achieve better performance than a single classifier in recognizing medication information from clinical text. It suggests that simple strategies that can be easily implemented such as majority voting could have the potential to significantly improve clinical entity recognition.

Multiple sequence alignments are the usual starting point for analyses of protein structure and evolution. For proteins with repeated, shuffled and missing domains, however, traditional multiple sequence alignment algorithms fail to provide an accurate view of homology between related proteins, because they either assume that the input sequences are globally alignable or require locally alignable regions to appear in the same order in all sequences. In this paper, we present ProDA, a novel system for automated detection and alignment of homologous regions in collections of proteins with arbitrary domain architectures. Given an input set of unaligned sequences, ProDA identifies all homologous regions appearing in one or more sequences, and returns a collection of local multiple alignments for these regions. On a subset of the BAliBASE benchmarking suite containing curated alignments of proteins with complicated domain architectures, ProDA performs well in detecting conserved domain boundaries and clustering domain segments, achieving the highest accuracy to date for this task. We conclude that ProDA is a practical tool for automated alignment of protein sequences with repeats and rearrangements in their domain architecture.
