Purpose -In recent years the pace of technological innovation in construction operations has escalated with resulting intensification of the inputs necessary for contemporary multi-disciplinary construction projects, and thereby has exacerbated their potential complexity. In its widest form the topic of complexity in construction continues to attract considerable research and re-evaluation; however the resulting published work remains largely unapplied by construction professionals due to either to its intricacy or difficulty of application. This study concludes that recent advances in information management can facilitate data sharing and thereby resolve many of the early life and project production complexity issues. The aim of this paper is to suggest a new direction for research studies. Design/methodology/approach -This desktop study uses a key word protocol to identify the relationship between published research papers both past and present to first determine the development of complexity research and then looks forward to an interaction between research and industry. Findings -Questions if complexity in multi-disciplinary projects may well be diminished by information modelling, this practical solution evolved in parallel with computer based design. Originality/value -This descriptive study suggests a new direction for research studies.
Opinion is divided as to the necessary extent of parathyroid resection in primary hyperparathyroidism. Some surgeons urge that subtotal parathyroidectomy be performed routinely, while others reserve subtotal resections for patients with parathyroid hyperplasia. In a review of 102 patients treated by parathyroidectomy for primary hyperparathyroidism and subsequently followed up for an average of 6 years, there were 73 patients with parathyroid adenoma, 26 with hyperplasia and 3 with carcinoma. The adenoma was not found at the first operation in only 3 cases; in the other 70 after removal of the adenoma no further hypercalcaemia was observed during the follow-up period. Persistent or recurrent hypercalcaemia was a problem in 9 out of the 26 patients with primary hyperplasia who had inadequate resection at the first operation, and also in the 3 patients with carcinoma. Experience shows that peroperative biopsy with frozen section of all the parathyroids is useful in confirming their identity and can be a guide to the type of disease present and the appropriate extent of resection.
Summary: We describe two cases of septic arthritis occurring in association with lower urinary tract infection in elderly men. In both cases the organism isolated from both the joint and the urine was Staphylococcus aureus. Further investigation of the urinary tract in both individuals identified bladder outflow obstruction secondary to benign prostatic hyperplasia predisposing them to infection.The urinary tract should be suspected as a focus ofinfection in septic arthritis in elderly men and further investigation of the urinary system may disclose a surgically correctable lesion.
