The authors conclude that the Pi Plate is a very good surgical option, especially in complicated, comminuted fractures classified as C3. It is not associated with an overly high risk of extensor tendon ruptures, as has been suggested by other authors.
The etiologies of Keinböck's disease and intraosseous ganglion remain unknown. Both entities are rare and the coexistence of these two pathologies in the same patient and hand is even less frequent. We report the case of a 40-year-old man with a longstanding history of martial arts practice (karate) who developed an avascular necrosis of the lunate concomitant with a giant intraosseous ganglion of the scaphoid bone successfully managed by proximal row carpectomy. We review the literature of these two diseases.
The operative treatment of thumb carpometacarpal joint arthritis may include resection-suspension-interposition-arthroplasty. Although quite a technically demanding procedure, a suspension arthroplasty using a strip of the flexor carpi radialis tendon is quite a popular technique. Other techniques, which use the abductor pollicis longus (APL) tendon, is also widely accepted.The aim of this randomized, prospective study was to compare the results of these 2 procedures 8 months postoperatively.From May 2005 to December 2006 a total of 55 operations in 53 patients with symptomatic Grade III or IV 1st CMC joint arthritis were identified and recruited into the study. They were then randomized to one of the 2 groups (APL vs. FCR). Patients were assessed preoperatively, and then immediately and 8 months postoperatively. Both, subjective parameters (Visual Analog Scale and DASH-Score) and functional parameters (maximum radial abduction, opposition and pinch- and key grip strength) where recorded. Additionally, the trapezial space was assessed radiographically postoperatively.After 8 months both groups had comparable subjective results (APL-group: VAS 2.3 points, DASH 24 points; FCR-group: VAS 1.9 points, DASH 20 points) as well as functional results (APL-group: radial abduction 55.3°, key-grip strength 8.1 kg, pinch-grip strength 5.5 kg; FCR-group: radial abduction 55.8°, key-grip strength 7.2 kg, pinch-grip strength 4.7 kg). Radiologically both groups showed an approximate 50% reduction in the height of the trapezial space.In resection-suspension-interposition-arthroplasty of the 1st CMC joint, similar results can be obtained using the technically less demanding APL-procedure when compared with the FCR-technique 8 months postoperatively.
<b>Objectives:</b> To determine the incidence of smell and taste disorders in our health department and to analyse the factors that could be associated with these symptoms in patients with COVID-19. <br><b>Methods:</b> We conducted an observational descriptive study of all patients with COVID-19 in our health area diagnosed between 2020/03/10 and 2020/04/14. Factors related to smell and taste disorders were analysed. <br><b>Results:</b> A total of 126 patients, 63 women and 63 men, aged 16–80 years, were included. As many as 69 patients (62.7%) presented hyposmia, and 58 (46%) of them had anosmia. A total of 75 patients (59.5%) presented hypogeusia, and 57 (45.2%) of them had ageusia. The risk factors that were most commonly associated with these disorders were the female sex (adjusted odds ratio, aOR 2.43 for smell disorders and 2.44 for taste disorders), allergic rhinitis (aOR 3.34 for smell disorders) and a younger age. A protective factor was arterial hypertension (aOR 0.51 for smell disorders and 0.35 for taste disorders). A history of tonsillectomy was the risk factor for taste disorder (aOR 5.23). <br><b>Conclusion:</b> Our results indicate that these sensory disorders occurred more frequently in female patients and in young patients with mild to moderate COVID-19 infection who progressed with mild nasal congestion, posterior rhinorrhoea and without anterior rhinorrhoea. The recovery of taste occurred before the recovery of smell.
Coalition between the pisiform and the hamate is a rare congenital anomaly, often presenting as an asymptomatic incidental finding on radiographs. In some cases, it may become symptomatic, typically after trauma. We present a 13-year-old patient, with no history of trauma, presenting a bilateral painful coalition of the pisiform and hamate. Both of which were treated with excision of the pisiform resulting in complete pain relief. Pisiform-hamate coalition is a rare condition, which can become symptomatic even without any trauma or overuse activity, and is an important differential diagnosis in ulnar-sided wrist pain.
