Background: İn coronary artery disease, the positive contribution of the presence of coronary collateral circulation to the course of the disease is known. As for the factors that trigger the formation of collateral circulation and interactions at the cellular level, no satisfactory results have yet been reached. Studies on this issue will also pave the way for potential treatment opportunities in the future. The aim of this study is to try to determine the relationship between coronary collateral circulation and elabela and apelinergic system.Methods: This is a cross sectional study. Totally 150 subjects were included the study. Demographic data, laboratory test data, and comorbidities were collected from the hospital data. Welch, One Way Anova, Chi-Square, and Kruskal-Wallis H tests were used for comparison of the groups. Games-Howell, Tukey HSD, and Dunn tests were used for post-hoc analysis where necessary.Results: Evaluating the differences between the groups in terms of the parameters examined. Looking at which groups the differences arise from; it was observed that the statistically significant difference observed for elabela, HDL-C, NT-ProBNP, LVEF and HS-C-Reactive protein was between all groups separately.Conclusions: In this study, we aim to reveal a positive relationship between the development of coronary collateral vessels and apelinergic systems. Our results indicated that Elabela protein could be evaluated as a potential agent for treatment.
Background: This study aims to examine the relationship between the development of coronary collateral circulation and serum elabela levels. Methods: Between January 2020 and December 2021, a total of 50 control individuals (29 males, 21 females; mean age: 63.2±10.0 years; range, 52 to 73 years) with no significant coronary artery disease as confirmed by angiography (Group 1) and 100 patients (55 males, 45 females; mean age: 66.6±9.6 years; range, 56 to 75 years) with coronary artery disease were included. The patients were further divided into two equal groups according to the Rentrop classification as poor (Group 2) and good coronary collateral circulation (Group 3). All groups were compared in terms of several parameters, particularly serum elabela levels. Results: Serum elabela levels were found to be statistically higher in the group with good collateral than the other groups (p<0.05). Low serum elabela levels increased the risk of developing weak collaterals by 2.43 times. Conclusion: The elabela protein is directly related to good collateral development and can be considered a potential agent for treatment.
Kronik Obstrüktif Akciğer Hastalığı (KOAH), koroner arter bypass greft operasyonu (CABG) yapılan hastalarda sık bir komorbid hastalıktır ve insidansı %4 ile %20,5 aralığındadır. KOAH geleneksel olarak CABG için cerrahi kontraendikasyon olarak bilinir. Cerrahi teknik, anestezi ve postoperatif hasta yönetimindeki son gelişmelerden dolayı CABG artık KOAH'lı hastalarda çok yaygın bir şekilde yapılmaktadır. Ancak son çalışmalar, CABG sonrası postoperatif mortalite ve morbidite üzerine KOAH'ın çeşitli etkilerini göstermiştir. Çalışmamızda CABG ameliyatının solunum fonksiyonlarına etkisini preoperatif ve postoperatif dönemde yapılan solunum fonksiyon testi ile araştırmayı amaçladık. Gereç ve Yöntemler: Kliniğimizde izole CABG cerrahisi uygulanan 50 hasta çalışmaya dahil edildi. Preoperatif solunum fonksiyon testinde FEV1 değeri <%70 olan toplam 24 hasta KOAH grubunu (Grup 1), geri kalan 26 hasta ise kontrol grubunu (Grup 2) oluşturdu. Bulgular: KOAH grubunda mekanik ventilasyonda kalış süresi (Grup 1'de 16.5 ± 2.6 saat, Grup 2'de 14.9 ± 2.7 saat; p=0.0001) ve hastanede yatış süreleri (Grup 1'de 13.8 ± 5.4 gün, Grup 2'de 10.1 ± 2.5 gün; p=0.004) bakımından kontrol grubuna göre anlamlı olarak uzamış idi. Solunum fonksiyon test parametrelerine bakıldığında KOAH grubunda zaten düşük olan değerlerde anlamlı bir düşme gözlenmezken, Kontrol grubunda postoperatif değerlerde preoperatif değerlere göre anlamlı bir düşme gözlenmiştir. Sonuç: CABG ameliyatı geçirecek hastalara preoperatif dönemde KOAH tanısının konması ve şiddetinin belirlenmesi; preoperatif dönemde gerekli tedavinin başlanması, intraoperatif ve postoperatif dönemde gelişebilecek komplikasyonları azaltmak ve/veya önlemek, takip edilecek stratejiler ve alınacak önlemler açısından önemlidir. Bu nedenle CABG ameliyatı olacak her hastaya preoperatif dönemde solunum fonksiyon testi rutin olarak uygulanmalıdır.
AbstractÖz Purpose: Chronic Atrial Fibrillation (AF) is the most prevalent arrhytmia in patients undergoing cardiac surgery. In this study we aim to share our cryoablation experiences. Material and Methods:We performed 25 cryoablations during cardiac surgery in Adana Numune Education and Research Hospital between September 2013 and July 2014. Patients evaluated retrospectively. The operations were performed with the use of cardiopulmonary bypass (CPB) and cold blood cardioplegia. Results: Fourteen mitral valve replacement, 7 aortic valve replacement and mitral valve replacement, 1 aortic valve replacement and tricuspid repair, 2 coronary artery bypass grafting, 1 coronary artery bypass grafting and aortic valve repair were applied. A -50-(-90) degrees of cryoprobe temperature was applied about 120 seconds. No complications developed in any of the patiens. Postoperative first day, first week, third month and after one year electrocardiography (ECG) of the patiens has evaluated. The corelation between preoperative left atrial diameter (M mode echocardiography) and postoperative first year atrial fibrillation has been investigated with corelation analysis. After one year controlls showed that 17 of 25 patient restored sinus rhytm and 8 of 25 patiens has still atrial fibrillation rhytm. Conclusion: Cryoablation using nitrous oxide based device for the treatment of atrial fibrillation has been applied recently and we consider that it is safe and effective.Amaç: Kronik atriyal fibrilasyon populasyonda olduğu kadar açık kalp cerrahisinde de sık görülen bir aritmidir. Bu çalışmamızda atriyal fibrilasyonun cerrahi tedavisinde uygulanan azot bazlı kriyoablasyonun güvenilirliğini ve etkinliğini araştırdık. Gereç ve Yöntem: Adana Numune Eğitim ve AraştırmaHastanesi'nde Eylül 2013-Temmuz 2014 yılları arasında 25 hastaya kriyoablasyon uygulandı. Hastalar retrospektif olarak değerlendirildi. Tüm hastalara operasyonlar kardiyopulmoner bypass altında ve kan kardiyopleji kullanılarak yapıldı. Bulgular: Ondört hastaya mitral kapak replasmanı, 7 hastaya aort ve mitral kapak replasmanı, 1 hastaya aort kapak replasmanı ve triküspid kapak tamiri, 2 hastaya koroner arter bypass greftleme, 1 hastaya koroner arter bypass greftleme ve aort kapak replasmanı yapıldı. Kriyoablasyon probu -50-(-90) dereceye kadar soğutulup yaklaşık 120 saniye uygulanarak kriyoablasyon işlemi tamamlandı. Hastaların hiçbirinde komplikasyon gelişmedi. Operasyona alınan hastaların postoperatif 1.gün, 1. hafta, 3.ay ve 1 yıl sonraki elektrokardiyografileri incelendi.1 yıl sonra 17 hastada sinüs ritmi, 8 hastada atriyal fibrilasyonun devam ettiği görüldü. Preoperatif sol atriyal çap (m mod ekokardiyografi) ile postoperatif 1.yılda sinüs ritmine dönüş arasında korelasyon analizi yapıldı. Sol atriyal çap azaldıkça, atriyal fibrilasyonda da azalma görüldü. Tartışma: Azot bazlı kriyoablasyon tedavisi yeni bir uygulama olmakla beraber, güvenilir ve etkin olduğunu düşünmekteyiz.
