Muchos trabajos aportan datos que sostienen que los pacientes con reflujos de alto grado son los que más posibilidades tienen de presentar nefropatía por reflujo. Esto es más importante, obviamente en aquellos pacientes afectados por reflujo vesicoureteral bilateral. Lo que no queda claro es si dentro de este útimo grupo de pacientes, no existe un subgrupo con RVU bilateral que se relacione con un peor pronóstico a lo largo del tiempo, aún realizando el tratamiento correspondiente. Incluimos nuestra propuesta como un enfoque novedoso complementario de las tendencias y estrategias habituales. A tal fin, formulamos las siguientes preguntas: - ¿Tiene el RVU de 5º grado bilateral un riesgo diferente de asociarse con insuficiencia renal congenita? - ¿Los pacientes con RVU de 5º grado bilateral presentan más riesgo de hipoplasia y/o escaras pielonefríticas que los pacientes con RVU de 4º grado bilateral? - ¿Los pacientes con RVU de 5º grado bilateral, son comparables con los pacientes de 4º grado bilateral de acuerdo al pronóstico y respuesta al tratamiento?
