Introduction Vitamin C is an essential nutrient for both humans and rats and has been noted for its beneficial properties, among them, healing. Objective To verify the effect of oral and subcutaneous vitamin C supplementation on the healing time of surgical wounds of rats skulls. Statistical Methodology Thirty male Wistar rats were divided into 3 groups: 10 from the control group (GI), 10 from the group treated with oral vitamin C (GII), and 10 from the group treated with subcutaneous vitamin C (GIII). Vitamin C was administered to GI and GIII animals from the 3rd to the 7th postoperative day, totaling 10 days of administration at a dose of 100 mg/kg/day. On the 4th day of the study, the rats were submitted to a surgical procedure consisting of a 2-cm incision of the skin of the animals' heads and suturing with single stitches. After a determined period, the rats were killed and submitted to the collection of material for study by the picrosirius red technique for the evaluation of collagen types I and III, the degree of hematoxylin and eosin healing, and the rate of contraction of the wound on subsequent days. The results were described in averages, medians, minimum and maximum values, and standard deviations. For the comparison of the three groups, the analysis of variance with one factor (one-way ANOVA) or Kruskal-Wallis non-parametric test was used. The normality of the variables was evaluated by the Shapiro-Wilk test. Values of p < 0.05 indicated statistical significance. The data were analyzed using the IBM SPSS Statistics for Windows, v.20.0. software. (IBM Corp., Armonk, NY, USA). Results The amount of collagen type III was higher in the groups that received vitamin C, however, without significant difference (n = 0.292). In relation to the rate of contraction of the surgical wound, it was higher in the groups treated with vitamin C, with a significant difference between groups I and II (p = 0.001), and between groups I and III (p < 0.001). No significant difference was found between the groups that were treated with vitamin C (p = 0.227). Conclusion Healing was more effective in the groups treated with vitamin C than in the group that did not receive vitamin supplementation. There was no significant difference in healing between the groups receiving oral or subcutaneous vitamin C.
Introdução: Tumores intradurais intramedulares são lesões raras que correspondem a cerca de 2-4% das neoplasias do SNC e a 8-10% dos tumores da medula espinhal. O tipo tumoral mais comum é o ependimoma, seguido dos astrocitomas e hemangioblastomas, os sintomas são insidiosos e inespecíficos sendo a dor a queixa mais comum. O diagnóstico se faz por RNM e através do exame anatomopatológico, sendo que o tratamento preconizado é a ressecção cirúrgica. Objetivo: Caracterizar os pacientes em relação à média de idade, a frequência em homens e mulheres, o tipo tumoral, a localização topográfica mais frequente, o grau de ressecção da lesão, o estado neurológico do paciente no pré-operatório e o prognóstico no pós-operatório. Métodos: Revisão retrospectiva dos prontuários de pacientes adultos submetidos a cirurgia para tumores intramedulares do ano de 2005 a 2016. O comitê de ética aprovou este estudo. Os dados foram coletados quanto à demografia, características clínicas e radiológicas e detalhes cirúrgicos. A escala de McCormick modificada foi utilizada para avaliar o estado neurológico dos pacientes na admissão, alta e seguimento. Para avaliar a associação entre variáveis qualitativas foi considerado o teste de Qui-quadrado. Valores de p<0,05 foram considerados estatisticamente significativos. Resultados: Trinta e seis casos foram revisados. A média de idade foi de 51,04 ± 17,05 anos, enquanto o seguimento médio foi de 12 meses (intervalo: 0,25-112 meses). O tipo histológico da lesão mais encontrado foi ependimoma (n = 18; 50%), seguido de astrocitoma (n=11; 30,55%) e hemangioblastoma (n=5; 13,88%). A região cervical foi a mais comumente envolvida (n = 17; 47,22%), seguida da região lombar (n=11; 30,55%) e torácica (n=8; 22,22%). A ressecção total foi obtida em 27 casos (75%). O grau de McCormick pré-operatório foi significativamente associado ao grau de McCormick de seguimento (valor p = 0,002). Cinco pacientes receberam radioterapia pós-operatória. Vinte e oito pacientes (77,78%) tiveram sobrevida livre de progressão no último seguimento. O estado neurológico pré-operatório foi associado com melhor escore McCormick no pós-operatório com p<0,05. Conclusão: O estado neurológico no pré-operatório e o grau de ressecção cirúrgica da lesão foram os fatores preditivos significativos de melhores resultados no seguimento. O tipo histológico tumoral não apresentou resultados significativos em relação à sobrevida dos pacientes.
Introdução: Hematoma extradural crônico é uma afecção incomum que representa 3,9-30% de todos os casos de hematoma extradural. Não há consenso entre os autores quanto à definição exata da lesão e nem ao intervalo de tempo necessário para o seu surgimento. Objetivo: Descrever um caso de hematoma extradural crônico calcificado diagnosticado após 18 anos de traumatismo crânio-encefálico (TCE), além de revisar a literatura sobre o tema. Caso Clínico: O caso foi pautado em exames de imagem, estudo histológico e resultado cirúrgico, além de uma revisão sistematizada da literatura. Paciente masculino de 45 anos que iniciou com quadro de tontura e instabilidade postural a 11 dias da admissão. Relatou que há 18 anos sofreu acidente automobilístico com TCE, mas que não passou por procedimento neurocirúrgico. A tomografia computadorizada revelou uma lesão biconvexa hiperdensa em suas bordas e hipodensa em seu interior caracterizando o hematoma extradural crônico. O paciente foi operado e após 5 dias da cirurgia recebeu alta sem déficit neurológico. Conclusão: Apesar de ser uma afecção incomum, deve-se ter consciência de sua eventualidade e das medidas corretas para o seu tratamento.
