500 Menschen mit Epilepsie sterben jährlich allein in Großbritannien an SUDEP. Manchmal berichten Familienangehörige über meine Internetseiten oder über E-Mail vom plötzlichen Tod eines Epilepsiepatienten. Von SUDEP selbst haben die meisten noch nie etwas gehört. Eine Untersuchung in England hat gezeigt, dass gerade einmal 5% aller behandelnden Neurologen ihre Patienten und deren Familien über SUDEP informieren. Der behandelnde Arzt sollte seine Patienten und deren Familien frühzeitig über die Gefahren, deren Risiken und die Prävention von SUDEP aufklären. Er sollte dieses Informationsgespräch in seinen Akten vermerken. Menschen mit Epilepsie und deren Familien sollten nicht verunsichert werden. Ihnen sollten die bestehenden Prädiktoren nicht verheimlicht werden. Ihnen sollten auch die Möglichkeiten zur Risikominderung genannt und verdeutlicht werden. Die Vermittlung von präventiven Maßnahmen im Gespräch mit Patienten und Familien ist hierbei die wesentliche Aufgabe. Wissenschaftliche Untersuchungen haben gezeigt, dass bestimmte Patienten mit Epilepsie mehr Risiken haben als andere. Mögliche Risikofaktoren sind: • Junge Männer und Frauen im Erwachsenenalter • generalisierte tonisch-klonische Anfälle • nächtliche Anfälle • mehrfacher Wechsel von Antiepileptika, unregelmäßige Einnahme und • schwer behandelbare (pharmakorefraktäre) Epilepsie • Alter unter 16 Jahren (hauptsächlich, wenn weitere Erkrankungen vorliegen)Wichtigste Hilfsmittel für eine unterstützende Arbeit sind Materialien für die Endzielgruppe und deren Familien. Beispiele hierfür liefern die Länder Groß-britannien, Australien und USA. Ausgehend von Studienergebnissen wurden Kurzbroschüren, Internetseiten und Fachhefte erstellt. Eine der meist genutzten und hilfreichsten Broschüren ist das Faltblatt "Epilepsy -Be safe, reduce risks", welche von dem "Joint Epilepsy Council" aus Großbritannien und Irland geschrieben wurde. Des Weiteren wurden Sprechstunden und Fachberatungen eingerichtet, um Antworten auf die wichtigsten und häufigsten Fragen zu geben.Wünschenswert sind diese Arbeiten auch für den deutschsprachigen Raum. Menschen mit Epilepsie finden immer wieder Hinweise über SUDEP im Internet, die nicht in deutscher Sprache veröffentlicht sind.Menschen mit Epilepsie und deren Familien stellen sich immer öfter die folgenden Fragen: Was ist SUDEP? Wen betrifft es? Sind auch Kinder betroffen? Was sind die Risiken? Gibt es hierzu Präventions-maßnahmen? Wer hilft mir und wer gibt hierzu Beratung?Derzeit besteht noch keine gemeinsame Abstimmung und Vorgehensweise. Im Interesse der deutsch-
