Objectives To understand healthcare professionals’ experiences and perceptions of nurses’ potential or ideal roles in pharmaceutical care (PC). Design Qualitative study conducted through semi-structured in-depth interviews. Setting Between December 2018 and October 2019, interviews were conducted with healthcare professionals of 14 European countries in four healthcare settings: hospitals, community care, mental health and long-term residential care. Participants In each country, pharmacists, physicians and nurses in each of the four settings were interviewed. Participants were selected on the basis that they were key informants with broad knowledge and experience of PC. Data collection and analysis All interviews were conducted face to face. Each country conducted an initial thematic analysis. Consensus was reached through a face-to-face discussion of all 14 national leads. Results 340 interviews were completed. Several tasks were described within four potential nursing responsibilities, that came up as the analysis themes, being: 1) monitoring therapeutic/adverse effects of medicines, 2) monitoring medicines adherence, 3) decision making on medicines, including prescribing 4) providing patient education/information. Nurses’ autonomy varied across Europe, from none to limited to a few tasks and emergencies to a broad range of tasks and responsibilities. Intended level of autonomy depended on medicine types and level of education. Some changes are needed before nursing roles can be optimised and implemented in practice. Lack of time, shortage of nurses, absence of legal frameworks and limited education and knowledge are main threats to European nurses actualising their ideal role in PC. Conclusions European nurses have an active role in PC. Respondents reported positive impacts on care quality and patient outcomes when nurses assumed PC responsibilities. Healthcare professionals expect nurses to report observations and assessments. This key patient information should be shared and addressed by the interprofessional team. The study evidences the need of a unique and consensus-based PC framework across Europe.
Zusammenfassung. Hintergrund: Die aufgrund der Coronavirus-Pandemie veranlassten Maßnahmen haben weitreichende Folgen für den Alltag von Menschen mit Demenz und ihren pflegenden Angehörigen. Beide Gruppen gehören meist zur Risikogruppe und sollen sich entsprechend stark einschränken. Die Alltagssituation wird dadurch erschwert, dass Betreuungsleistungen und Entlastungsangebote pausieren. Zudem haben Menschen mit Demenz Schwierigkeiten, die Beschränkungen und Hygieneregeln zu verstehen und umzusetzen. Ziel: Die Studie will die aktuelle Lebenssituation aus Sicht der pflegenden Angehörigen und der Menschen mit Demenz beschreiben. Methode: Zu zwei Erhebungszeitpunkten im Frühjahr 2020 wurden Telefoninterviews mit 21 pflegenden Angehörigen und Menschen mit Demenz geführt und inhaltsanalytisch ausgewertet. Ergebnisse: Die Situation wird von den Befragten unterschiedlich erlebt. Besonders die soziale Isolation, der erhöhte Betreuungs- und Pflegeaufwand, die Ungewissheit der Situation und die Zunahme von psychischen Symptomen werden als belastend beschrieben. Bezüglich der Bewältigung der Situation werden die Unterstützung durch das soziale Umfeld, alternative Kommunikationsmöglichkeiten, Erfahrungen mit vergleichbaren Krisen, die Stabilität der Versorgung und der Umgang mit Informationen thematisiert. Schlussfolgerungen: Pflegende Angehörige und Menschen mit Demenz fühlen sich durch die Coronavirus-Pandemie belastet, verfügen häufig jedoch über Bewältigungsstrategien für die besondere Situation. Insbesondere informelle Hilfen sind dabei ein wichtiger Unterstützungsmechanismus.
Background Sleep disturbances are common in nursing home residents and challenging for their nurses. Knowledge about sleep and sleep promoting factors is essential to provide adequate sleep management, where nurses play a key role. Therefore, nurses’ knowledge and attitudes towards sleep and sleep promoting interventions is important as enabling or inhibiting factor for successful sleep management. Methods A multicenter cross-sectional study was conducted among nurses working wholly or partially at night in nursing homes in Germany. Data were collected between February and April 2021 via online or paper and pencil questionnaires, comprising 56 items. Nursing homes were recruited through existing cooperation with the study centers as well as via nursing home registers. Results Finally, 138 nursing homes participated and 271 nurses completed the survey. Nurses agreed that sleep disturbances are an important topic with important impact on resident’ health. Although, the assessment of sleep was seen as nurses’ responsibility, only 40 nurses (14.7%) stated that residents’ sleep was always documented. Only 21.7% reported the availability of policy documents providing guidance regarding the management of sleep disturbances. The vast majority (93.2%) reported never having received training about sleep and management of sleep disturbances after their basic nursing training. Conclusions Our results indicate that nurses working at night can play an important role in residents’ sleep promotion. The findings indicate nurses’ educational needs regarding sleep and sleep promotion. Nursing homes should implement institutional guidelines in order to promote residents’ sleep based on adequate evidence-based non-pharmacological interventions.
Zusammenfassung. Hintergrund: Case Management bei Demenz wird in der Nationalen Demenzstrategie Deutschlands gefordert. Dessen Wirksamkeit ist vielfach untersucht, die Ergebnisse sind widersprüchlich. Demgegenüber sind Umsetzungsprozesse von Case Management-Konzepten bislang kaum beschrieben. Ziele: Ziel der Machbarkeitsstudie war es, die Umsetzung einer aufsuchenden Hilfe für Menschen mit Demenz zu analysieren sowie Veränderungen in der Versorgung, Akzeptanz sowie förderliche und hemmende Faktoren der Umsetzung zu explorieren. Methoden: Die aufsuchende Hilfe für Menschen mit Demenz wurde im Zeitraum 08 / 2018 bis 07 / 2019 umgesetzt. Einbezogen wurden Menschen mit Demenz und pflegende Angehörige. Es erfolgte eine prospektive Erfassung quantitativer und qualitativer Daten anhand von halbstandardisierten Interviews im Rahmen der aufsuchenden Hilfe. Ergebnisse: Die aufsuchende Hilfe konnte bei 113 Menschen mit Demenz umgesetzt werden und größtenteils konnten pflegende Angehörige einbezogen werden. Im Mittel fanden acht Kontakte im Zeitraum von 74 Tagen statt. Die Bedarfs- und Unterstützungsbereiche waren vielfältig. Die Inanspruchnahme von Unterstützungsangeboten steigerte sich nach Interventionsabschluss um 19 %. Schlussfolgerungen: Die Umsetzung einer kontinuierlichen und prozesshaften Begleitung von Menschen mit Demenz und Angehörigen ist möglich, wobei Struktur und Prozedere transparent sein sollten. Förderlich ist eine umfangreiche Ausrichtung und vernetzte Arbeit. Die Wirksamkeit der Intervention bleibt in einer kontrollierten Studie zu untersuchen.
Zusammenfassung. Hintergrund: Menschen mit Demenz und ihre Angehörigen sind aufgrund der Komplexität einer Demenz vor besondere Herausforderungen gestellt. Die Familien benötigen Informationen sowie Beratung, um sich in der unübersichtlichen Versorgungslandschaft zurechtzufinden und bedarfsgerechte Angebote in Anspruch nehmen zu können. Fragestellung / Ziel: Der Fallbericht zielt darauf ab, die häusliche Situation eines Ehepaares aufzuzeigen, bei dem der Ehemann an einer Demenz bei Parkinson-Krankheit leidet und erhebliche Herausforderungen innerhalb der Familie und im Versorgungssystem bestehen. Methode: Im Rahmen des Dementia Care Nurse Projektes erfolgte eine Fallbegleitung. Anhand unterschiedlicher Assessmentinstrumente wurden die verschiedenen Informationen der Familie zusammengetragen und beschrieben. Ergebnisse: Die Problemfelder und der Hilfebedarf der betroffenen Familie sind vielfältig und reichen über Kostenerstattung, Beantragung von Pflegeleistungen bis hin zum Umgang mit herausfordernden Verhaltensweisen sowie Reduktion der Belastung der pflegenden Angehörigen. Schlussfolgerung: Die Familie, insbesondere die pflegende Ehefrau, konnte wirksam bei den Herausforderungen der Demenzerkrankung unterstützt werden, indem z. B. professionelle Dienste einbezogen und Leistungsansprüche geklärt wurden. Aus Projektperspektive sind unabhängige Beratungsstrukturen im Sinne eines Case Management Ansatzes notwendig, um die häusliche Situation zu stabilisieren.
