Mục tiêu: Mô tả một số biến thể giải phẫu mũi xoang thường gặp trên cắt lớp vi tính ở các bệnh nhân viêm mũi xoang mạn tính.Đối tượng và phương pháp nghiên cứu: nghiên cứu mô tả hồi cứu và tiến cứu 200 bệnh nhân (BN) viêm xoang mạn tính đã được chụp cắt lớp vi tính đa dãy (MSCT) tại Trung tâm Chẩn đoán hình ảnh và Điện quang can thiệp Bệnh viện Đại học Y Hà Nội. Kết quả: 63,5% BN có bất thường vách ngăn mũi, lệch vách ngăn gặp trong tất cả các BN, theo phân loại của Mladina nhóm I chiếm tỷ lệ cao nhất với 25,2%, tiếp đến là nhóm III, II ,VII và V gặp ở lần lượt 18,9%, 17,3%, 16,5% và 15%. Biến thể giải phẫu cuốn mũi gặp ở 54,5%, trong đó xoang hơi cuốn mũi hay gặp nhất. Biến thể giải phẫu mỏm móc hay gặp nhất là xoang hơi mỏm móc với 7%. Tế bào Haller gặp ở 46% BN, kích thước trung bình của tế bào Haller bên phải là 4,89mm và bên trái là 5,04mm. Tế bào đê mũi gặp ở 85% BN, kích thước trung bình của tế bào đê mũi bên phải là 6,38mm và bên trái là 6,59mm. Kích thước trung bình của bóng sàng bên phải là 8,9mm, bên trái là 9,32mm. Tắc phức hợp lỗ ngách gặp ở 76,5% BN, vị trí hay gặp nhất là ống xoang hàm, tiếp đến là phễu sàng. Kết luận: Nghiên cứu thực hiện ở 200 BN viêm xoang mạn tính, kết quả cho thấy biến đổi giải phẫu tế bào Onodi hay gặp nhất, tiếp đến là tế bào đê mũi và lệch vách ngăn mũi.
Mục tiêu: Đánh giá khí hóa xoang bướm trong bilan trước phẫu thuật nội soi (PTNS) xoang ở bệnh nhân viêm xoang mạn tính (VXMT) bằng cắt lớp vi tính đa dãy (MSCT). Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang các bệnh nhân viêm xoang mạn (VXM) được chụp MSCT xoang đồng thời được phẫu thuật nội soi (PTNS) xoang. Sau đó ghi nhận các hình thái xoang bướm theo phân loại Hammer. Mức độ khí hóa xoang bướm được đánh giá, phân độ trên mặt phẳng đứng dọc nhằm dự phòng biến chứng khi mổ. Kết quả: Từ tháng 09/2020 đến tháng 9/2022, có 200 bệnh nhân VXM được chụp MSCT xoang, được PTNS xoang tại bệnh viện Đại học Y Hà nội. Trong số này, có 78 bệnh nhân quá phát xoang bướm, chiếm tỷ lệ 39% tổng số bệnh nhân nghiên cứu. Có 40 bệnh nhân có quá phát xoang bướm vào nền sọ chiếm 51.28% số bệnh nhân quá phát xoang bướm. Có 29 bệnh nhân quá phát xoang bướm vào mỏm yên trước, chiếm 31.28%, 4 bệnh nhân có lồi động mạch cảnh trong vào xoang bướm, chiếm 5.13% và 5 bệnh nhân có quá phát xoang bướm gây vách ngăn bất thường, chiếm 6.41%. Kết luận: Quá phát xoang bướm là loại bất thường xoang hay gặp, thấy rõ trên MSCT mặt phẳng đứng dọc. Việc phát hiện và đánh giá quá phát xoang bướm có vai trò quan trọng cho bilan trước PTNS xoang.
TÓM TẮTMục đích: Mô tả đặc điểm hình ảnh và giá trị chẩn đoán của CHT đối với u nguyên bào thần kinh đệm hai bán cầu.Đối tượng và phương pháp nghiên cứu: Hồi cứu 2505 hồ sơ BN u não (trong đó có 48 BN có kết quả GPB là u nguyên bào thần kinh đệm) có chụp CHT, được phẫu thuật và có kết quả giải phẫu bệnh, thời gian từ tháng 01 năm 2009 đến tháng 3 năm 2011 tại Bệnh viện Việt Đức. Tất cả phim chụp CHT được đọc lại đối chiếu với kết quả phẫu thuật và GPB.Kết quả: Tuổi hay gặp 51±16, nam/nữ = 1,3/1; vị trí hay gặp là thùy trán (26,4%), ít gặp nhất ở thùy chẩm, vị trí giữa thùy trán-thái dương chiếm 20%; kích thước u > 5cm (60,4%), giảm tín hiệu trên T1W (66%), tăng tín hiệu trên T2W (70%), sau tiêm ngấm thuốc mạnh và không đều (43,4%), dạng viền (47,3), hoại tử trong u (95,6%), thoái hóa nang dịch trong u (52,8%), chảy máu trong u (28,4%); phù não quanh u chủ yếu độ II và III (90%); 47,2% có xâm lấn màng não, 39,6% xâm lấn thể trai. Đối chiếu phẫu thuật CHT chẩn đoán chính xác vị trí u 100%. Đối chiếu với GPB, CHT có độ nhạy 81,1%, độ đặc hiệu 97,6%, độ chính xác đạt 98%.Kết luận: CHT là phương pháp có giá trị cao trong việc chẩn đoán u nguyên bào thần kinh đệm hai bán cầu.
