;. DIETA ENTERAL PRECOCE EM PACIENTE DESNUTRIDO NO PÓS OPERATÓRIO DE ABDOME AGUDO OBSTRUTIVO: RELATO DE CASO. ABCDExpress. 2017;1(2):250.Instituição: HOSPITAL GERAL ERNESTO SIMÕES FILHO JSS, masculino, 59 anos, desnutrido grave (albumina 1,8mg/dL), portador de doença de Chagas, com relato de disfagia progressiva há 1 ano, em uso de SNE para alimentação há 1 mês. Refere diarréia há 1 semana, com parada de eliminação de fezes e dor abdominal difusa, evoluindo com sinais de sepse. Abdome na admissão apresentando distensão abdominal, RHA presentes. Passado sonda gástrica sob aspiração com retorno de conteúdo fecalóide. Evoluiu com peritonite, sendo submetido a cirurgia com inventario de cavidade evidenciando volvo de sigmoide com necrose de alça, sendo procedida retossigmoidectomia a Hartmann. Paciente evoluiu de forma satisfatória, sendo reintroduzida dieta via SNE 72h após o procedimento cirúrgico. Atingiu o VET diário no 8? dia pós operatório, quando teve alta da cirurgia. Permanece internado na enfermaria com quadros repetidos de infecção do trato respiratório. Discussão: A desnutrição é um fator que aumenta o risco de infecção em pacientes pós operarórios [1]. O aporte calórico adequado introduzido precocemente em pacientes desnutridos em estado catabólico reduz o risco de infecções, principalmente do trato respiratório inferior [2]. Albumina sérica abaixo de 2,2mg/dL está relacionada a um maior risco infeccioso, evidenciando um estado catabólico aumentado [3]. A via enteral deve ser sempre preferida devido ao seu menor risco de complicações, como menor risco de translocação bacteriana e estimulação da mucosa intestinal, porém algumas vezes a via parenteral deve ser utilizada. Uma das indicações de associação é quando o paciente não consegue atingir sua meta diária com a via enteral, devendo ser o mais precoce possível em pacientes previamente desnutridos, geralmente sendo introduzida antes de 5 dias de internamento. [2,3]. Conclusão: Conforme a literatura preconiza, a dieta parenteral deve ser introduzida precocemente em associação com a dieta enteral para pacientes desnutridos em estado catabólico. No caso apresentado, o paciente foi tratado como sendo eutrófico, o que contribuiu para as repetidas infecções do trato respiratório inferior. A introdução precoce da dieta parenteral para pacientes desnutridos em associação com a dieta enteral com objetivo de atingir o VET diário mais rapidamente reduz a incidência de infecções no pós operatório desses pacientes.
