ResumoA fratura do côndilo occipital (FCO) é uma rara forma de trauma raquimedular. Há 96 casos previamente relatados na literatura mundial, nenhum dos quais bilateral.Apresentamos um caso único de FCO bilateral em um paciente de 23 anos do sexo masculino, vítima de acidente automobilístico. A apresentação clínico-radiológica e o manejo ideal são discutidos. Um alto grau de suspeição clínica é fortemente recomendado para evitar retardos no diagnóstico.
