Jornal Brasileiro de Pneumologia 31(Supl 1)-Junho de 2005 Vasculites PulmonaresAs vasculites pulmonares representam um espectro de doenças que causam inflamação e destruição dos vasos pulmonares com conseqüente isquemia e hemorragia das regiões anteriormente irrigadas por esses vasos.Seu diagnóstico requer conhecimento das possíveis manifestações dessas doenças, assim como reconhecimento da sua expressão radiológica e anatomopatológica. O diagnóstico diferencial das vasculites é extremamente importante, pois seu tratamento adequado propicia a remissão da doença em cerca de 90% dos pacientes, enquanto que o não tratamento adequado pode propiciar uma morbimortalidade elevada.Assim, esta revisão abordará as principais apresentações das vasculites pulmonares, bem como suas manifestações radiológicas e tomográficas. Serão abordados os aspectos diagnósticos, com ênfase no anticorpo anticitoplasma de neutrófilos e na interpretação dos resultados anatomopatológicos.Serão discutidas as principais abordagens terapêuticas e as possíveis alternativas nos casos refratários a tratamento.Esperamos que esta revisão contribua para o melhor entendimento das vasculites pulmonares e para seu reconhecimento dentre as diferentes apresentações das doenças pulmonares.
