Staphylococcus epidermidis in immunocompromised patients can cause bacteremia related to the use of catheter due to biofilm production. There are different phenotypic methods to detect biofilm formation. One method is based on culture in brain heart infusion agar (BHIA) containing sucrose and red Congo dye (original Congo red agar). Our group created a new CRA formula and we have confirmed its capacity to detect biofilm production in 210 S. epidermidis strains, including 76 (36.2%) icaAB gene-positive strains. Other parameters were also evaluated. The new CRA formula that gave the best results was BHIA with sucrose (5%), Congo red (0.08%), NaCl (1.5%), glucose (2%), and vancomycin (0.5 mg/mL) (vancomycin-modified CRA-CRAmod). The CRAmod plus vancomycin may be a promising tool and can help to determine the real participation of S. epidermidis in the infectious process.
Objective:to identify possible risk factors for acquisition of Enterobacterial strains with
a marker for resistance to carbapenems. Methods:exploratory case-control study performed in hospital settings. The study sample
consisted of patients with biological specimens that tested positive for
carbapenem-resistant Enterobacteriaceae (cases), with the disk diffusion test and
Etest, and controls with biological samples testing negative for
carbapenem-resistant Enterobacteriaceae. In all, 65 patients were included: 13
(20%) cases and 52 (80%) controls. Results:the microorganisms isolated were Serratia marcescens (6), Klebsiella pneumoniae
(4), and Enterobacter cloacae (3). Univariate analysis revealed that length of
hospitalization prior to sample collection (p=0.002) and having a surgical
procedure (p=0.006) were statistically significant. In the multivariable logistic
regression model, both were still significant, with odds ratios of 0.93 (p =
0.009; 95% CI: 0.89 to 0.98) for length of hospitalization prior to sample
collection, and 9.28 (p = 0.05; 95% CI: 1.01 to 85.14) for having a surgical
procedure. Conclusion:shorter hospitalization times and increased surveillance of patients undergoing
surgery could play a decisive role in reducing the spread of carbapenem-resistant
microorganisms in hospital settings.
INTRODUÇÃO: O gênero Staphylococcus é de grande importância devido a sua alta prevalência em infecções hospitalares e por apresentar taxas elevadas de resistência a oxacilina e a outros antimicrobianos. Assim, a avaliação da acurácia dos métodos fenotípicos usados para determinação do perfil de suscetibilidade a antimicrobianos é essencial para garantir a escolha da terapia mais adequada. MÉTODOS: Foram usadas 114 amostras de Staphylococcus sp (53 S. aureus e 61 SCN) na avaliação da acurácia dos métodos de difusão de disco, microdiluição em agar, ágar triagem oxacilina e sistema automatizado em comparação com a PCR para verificação da resistência a oxacilina. RESULTADOS: O gene mecA foi detectado em 48 (42,1%) amostras e 27 (23,7%) amostras apresentaram discrepância de resultados em pelo menos um dos métodos (74,1% SCN, 25,9% S. aureus). Para S. aureus, com exceção do Microscan Walkaway, todos os métodos apresentaram 100% de especificidade e sensibilidade. Já para os SCN, o sistema automatizado e o disco de cefoxitina apresentaram menor acurácia. CONCLUSÕES: O uso de dois métodos deve ser a melhor opção para a melhora da acurácia, principalmente quando o laboratório de diagnóstico utiliza somente sistema automatizado ou teste de difusão do disco de oxacilina. A associação destes métodos com outros apresentaram praticamente 100% de sensibilidade e especificidade em nosso estudo.
A produção das chamadas Betalactamases de espectro estendido (ESBLs) entre membros da família Enterobacteriaceae tem se tornado comum, sendo cada vez mais identificadas em pacientes hospitalizados, o que representa um importante problema de saúde pública. Detectar a produção das enzimas ESBL em enterobactérias provenientes de diferentes amostras hospitalares originadas de pacientes ambulatoriais e pacientes internados. Foram coletadas 73 amostras, sendo 45 amostras de pacientes internados e 28 de pacientes ambulatoriais de diferentes sítios, e submetidas ao teste fenotípico de disco-aproximação. Do total de amostras, 28,7% foram produtoras da enzima ESBL. A prevalência da enzima foi maior nos gêneros Escherichia sp. (43%) e Klebsiella sp. (38%) sendo a maioria dos isolados provenientes de infecção urinária. Também, 10,7% dos isolados ambulatoriais foram positivos para detecção da enzima e 5 isolados produtores de ESBL apresentaram sensibilidade no teste de triagem. A prevalência geral de produção de ESBL foi considerável, sendo que a realização de testes fenotípicos é importante para essa detecção. Além disso, a presença dessas bactérias em pacientes ambulatoriais demonstra sua disseminação para fora do ambiente hospitalar.
O presente trabalho teve por objetivo pesquisar a presença de enterobactérias resistentes a diferentes antimicrobianos nas águas de poços tubulares perfurados na região de Santa Teresa e Santa Maria de Jetibá – ES. Para isso foram coletadas amostras de 10 poços, sendo cinco poços em cada localidade. Essas amostras foram submetidas a testes laboratoriais para isolamento e identificação das bactérias presentes e posteriormente o antibiograma para enterobactérias. Na cidade de Santa Teresa 60% dos pontos de coleta apresentaram contaminação microbiana, já em Santa Maria de Jetibá, 80% dos poços estavam contaminados. Houve uma prevalência de coliformes totais, tais como Klebsiella pneumoniae, Enterobacter cloaceae, Enterobacter gergoviae e Serratia rubideae. Dentre as enterobactérias isoladas 33,4% apresentaram resistência a cefalotina e a tetraciclina e 16,7% a gentamicina. A contaminação microbiana em poços tubulares, e ainda a presença de contaminantes com resistência a antimicrobianos, mostra um considerável nível de interferência humana nesses ecossistemas, evidenciando a necessidade de um cuidado com a forma de utilização dessas águas, principalmente se a mesma estiver sendo utilizada em ambientes que lidam com a saúde humana.
