O tratamento da dor lombar é um desafio para profissionais da área da saúde. Sua alta prevalência determina custos elevados ao sistema de saúde. É prioritária a busca por modalidades de tratamento eficazes em melhoria de dor e incapacidade funcional associada. A evidência sobre diversos tratamentos conservadores de dor lombar, aguda ou crônica, vem crescendo nos últimos anos, sem possibilitar, no entanto, afirmações definitivas sobre reais efeitos da maioria das intervenções, bem como de possível superioridade de uma sobre a outra. Há razoável evidência contemporânea de benefício com tratamento medicamentoso, intervenções fisioterapêuticas, exercício e abordagem biopsicossocial da dor lombar. Medicamentos são utilizados como sintomáticos, a maior eficácia de curto prazo sendo obtida com AINEs e relaxantes musculares em dor aguda e com opioides e anticonvulsivantes (sobre componente neuropático) em dor crônica. Dentre as medidas não medicamentosas, mostram-se razoavelmente eficazes: manutenção da atividade, exercícios, terapia manual, laserterapia de baixa potência (dor com radiculopatia), acupuntura (dor lombar crônica), educação, programa de reabilitação multidisciplinar biopsicossocial e terapia cognitivo-comportamental.
