O presente artigo discute os motivos que levaram à adoção de uma política de medicamentos no Brasil. Para tanto busca, através de uma revisão bibliográfica, fazer um resgate histórico dos múltiplos fatores que interferiram na construção dessa política. Além de traçar a trajetória deste processo, o artigo procura assinalar algumas de suas características no Brasil contemporâneo. Enfatiza-se, aqui, a política de saúde como uma política universalista e igualitária, reclamando a responsabilidade do Estado na efetivação do direito à saúde preconizado na Constituição de 1988. Nesse contexto, apontam-se as razões para que a política de medicamentos seja parte integrante da política de saúde, com o objetivo de se obter um acesso mais próximo do integral, entendendo que o campo da saúde é também um espaço onde a cidadania deva ser buscada de modo pleno.
A saúde mental tem sido marcada atualmente por diversas reformulações, incluindo a intensificação da medicalização da saúde e presença da pandemia do novo coronavírus. Este trabalho teve como objetivo descrever a percepção da saúde mental pelos profissionais dos programas de residência em saúde, em 2020, durante a pandemia da COVID-19. Tratou-se de estudo observacional e transversal, com abordagem quantitativa, com aplicação de questionário estruturado, auto aplicado e online, para 52 residentes dos Programas de Residência Uni e Multiprofissionais do Hospital Universitário da Universidade Federal de Juiz de Fora. Os dados obtidos foram trabalhados pela análise estatística descritiva e o teste qui-quadrado para verificar a associação entre a autopercepção da saúde mental, as variáveis sociodemográficas e o perfil profissional. Com o intuito de verificar o efeito da realização do acompanhamento da saúde mental, realizou-se análise de regressão logística. Os resultados demonstraram agravamento significativo na percepção da saúde física e mental pelos residentes, comparando o período antes e durante a pandemia, em 2020, e que o acompanhamento da saúde mental foi fator protetor para alterações fisiológicas percebidas pelos residentes no período. Assim, o apoio psicossocial aos profissionais de saúde é de fundamental importância, principalmente quando consideramos o cenário pandêmico.
O presente estudo teve como objetivo discutir a Assistência Farmacêutica (AF) no município de Juiz de Fora, inserida nos Planos Municipais de Saúde (PMS), no período de 1988 a 2017. A pesquisa parte de revisão bibliográfica e avança pelo campo da investigação documental. Para contextualização do cenário foi utilizado o resgate histórico da AF no Brasil e no estado de Minas Gerais em recorte a partir dos anos 1980 tendo por base documentos, leis e portarias. A fonte principal da pesquisa inclui a busca, a leitura e a categorização dos Planos Municipais de Saúde de Juiz de Fora/MG, nos seguintes períodos: 1988; 1997; 2002/2005; 2006/2009; 2010/2013 e 2014/2017. Para a avaliação da assistência farmacêutica apoiou-se nos indicadores da Organização Pan Americana de Saúde, de 2005: acesso, qualidade e uso racional de medicamentos. O estudo demostrou que os indicadores acesso aos medicamentos e uso racional dos medicamentos foram, de certo modo, contemplados nesses planos. Todavia, o indicador qualidade dos medicamentos ainda se encontra incipiente. Foi possível concluir que a AF avançou no período com a introdução da Assistência Farmacêutica no organograma da Secretaria Municipal de Saúde, criação do Departamento de Assistência Farmacêutica (DAF), implantação da Comissão de Farmácia e Terapêutica (CFT), presença do farmacêutico como gestor da AF, aumento do número desses profissionais no quadro funcional do município e criação de uma equipe para acompanhar os processos de aquisição de medicamentos. No entanto, ainda são identificadas fragilidades em relação ao sistema de informação sobre os medicamentos e gestão da AF; ao acesso racional aos medicamentos, falta e desperdício de medicamentos; à não implantação do NASF, bem como, à discussão e materialização das farmácias distritais, a exemplo de outros municípios mineiros, polos das macrorregiões de saúde.
Background COVID-19 is a pandemic disease caused by the novel coronavirus SARS-CoV-2, which spread worldwide, revealing uphill repercussions. The infection is mediated by coronavirus spike protein and host cell receptor ACE2 (Angiotensin Converting Enzyme 2). Several cell lines have been used as an in vitro model to test potential drugs and to study the viral kinetics. Since lungs are one of the first organs affected by the virus, pulmonary cell lines such as: A549 (lung cells) and BEAS-2B (Bronchial epithelium cells) are the most studied. However, these cells present a slow viral replication profile which complicate the testing of new antiviral drugs and viral replication studies. High levels of ACE2, which is observed in some groups of patients, is associated with the increase of viral replication and severe symptoms. Thus, the development of a BEAS-2B cell line overexpressing ACE2 would be a useful model to study SARS-CoV-2 infection and to study new drugs. Aims Develop a BEAS-2B cell line overexpressing ACE2 and evaluate the role of ACE2 on SARS-CoV-2 viral kinetics. Methods BEAS-2B were transfected with pCEP-ACE2-myc and selected with hygromycin (125ng/ul) for 20 days, creating BEAS-2B-ACE2 cell line. ACE2 expression was quantified by RT-qPCR. Western Blotting and Immunofluorescence were used to quantify the ACE2 protein levels. ACE2 activity was evaluated using (MCA-Ala-Pro-Lys(Dnp)-OH) substrate. Cells were infected with SARS-CoV-2 (MOI=0.2). Viral kinetics were analyzed by RT-qPCR. Proliferation analysis was performed by MTT assay. Results Long-term overexpression ACE2 in BEAS-2B-ACE2 was confirmed by RT-qPCR, Western Blotting and Immunofluorescence. Compared to BEAS-2B, ACE2 mRNA expression was 100-fold higher (p<0.05), ACE2 protein levels increased 12X (p<0.05) and the immunofluorescence showed that ACE2 protein was more abundantly in BEAS-2B-ACE2. A 50X (p<0.0001) increase in the ACE2 activity was observed 2 months after the transfection. There was no difference in proliferation between BEAS-2B-ACE2 and BEAS-2B. Overexpression of ACE2 increased the viral kinetics. BEAS-2B-ACE2 presented 1000X (p<0.05) more SARS-CoV-2 RNA in cells and supernatant compared with BEAS-2B, 48 and 72 hours after infection. Conclusion Here we show for the first time that overexpression of ACE2 in BEAS-2B drastically changes the infection profile of SARS-CoV-2 and increases viral load – making it an useful cell line for future studies of SARS-CoV-2.
