Objetivo: estimar la prevalencia de percepción de discriminación y su asociación con estrés laboral, ansiedad y miedo al COVID-19 en médicos generales colombianos. Métodos: Estudio transversal que pertenece al proyecto Dinámicas Psicosociales Universitarias, aval institucional de la Universidad de Cartagena, Colombia. Médicos generales colombianos fueron invitados por redes sociales a diligenciar, los primeros cinco días de abril-2020, un formulario Google Forms© con preguntas sobre temores y opiniones respecto al COVID-19 y las herramientas: Test de Estrés Laboral, Generalized Anxiety Disorder y Fear of COVID-19 Scale. Estudio anónimo, voluntario, aceptación de términos y consentimiento informado. Se conformaron dos grupos: con percepción de discriminación y sin percepción de discriminación, según respuesta a la pregunta “haber tenido percepción de discriminación por su condición de médico”, en los últimos días de marzo-2020. Se realizó regresión logística entre percepción de discriminación con ansiedad, estrés laboral y miedo al COVID-19. Resultados: Participaron 531 médicos colombianos, la mitad entre 20-30 años. 207 (39%) informaron percepción de discriminación. Ansiedad, estrés, temores y miedos relacionados con la pandemia y la labor profesional fueron altamente prevalentes y más frecuentes en el grupo con percepción de discriminación (p<0.05). La presencia y severidad de ansiedad, estrés y temores por el COVID-19 se asociaron con mayor presencia de percepción de discriminación; sentirse protegido por sus superiores laborales se asoció con menor percepción de discriminación. Conclusión: Dos de cada cinco participantes informaron percepción de discriminación, evento social que se asoció con ansiedad, estrés, temores y opiniones sobre la actividad médica en la pandemia.
Introduction: Familism is a multidimensional cultural construction that gives importance to family ties. Study was done to estimate prevalence of familism and its link to psychosocial variables in pregnant adolescents who received prenatal care. Material and Methods: A cross-sectional study that is part of the project "Gestación" was carried out. Four hundred and ninety-nine pregnant adolescents received obstetric care, completed sociodemographic characteristics forms and answered scales of religiosity, spirituality, family function, happiness, resilience and familism based on the Bardis scale in Cartagena, Colombia. Correlation was established among familism, sociodemographic and psychosocial variables. Multiple linear regressions were made to identify associated variables; p<0.05 was significant. Results: the average age was 17 years old. 41% of participants strongly agreed to give their parents their earnings, 84% obeyed siblings, 65% thought family is more important than personal interests, 75% defended family, 87% are loyal to family, 90% help their parents, 75% help their uncles, 74% help their parents in-law and 66% live with other relatives. There were significant factors related to greater familism: a higher level of resilience, sexual partner with secondary studies, having a functional family and three or more ultrasounds p<0.05. Primary studies, not attending church, having a dysfunctional family and poor resilience were negative predictors of familism p<0,001. There was significant positive correlation between spirituality and happiness with familism. Conclusions: a high level of familism was observed in a group of pregnant adolescents. We recommend that healthcare professionals explore psychosocial aspects of teenage pregnancy to promote coping strategies regarding motherhood responsibilities.
Introduction Hysterectomy is a common gynecologic surgery carried out to remove the pathologic uterus. Objective: To establish if sleep disorders and sexual function are associated with deterioration of the quality of life (QoL) in hysterectomized and sexually active women. Methods: A cross-sectional study was carried out with inhabitants from two cities of the Colombian Caribbean. The pollsters invited women aged between 40-59 years to participate; in their communities they applied surveys with demographic characteristics: Female Sexual Function Index, Atenas Insomnia Scale and Menopause Rating Scale. Sexually active women were selected; then the association was established with logistic regression. Results: 522 women were studied with an average age of 50 years: 30% oophorectomized, 59.8% Hispanic, 40.2% afro-descendants and 22.2% hormonal therapy users. 80% of them had somato/vegetative, psychological or urogenital deterioration; 29.1% with severe deterioration of QoL and 47.5% with insomnia. Out of 390 (74.7%) with sexual activity, 59.7% suffered from sexual dysfunction. Insomnia: OR:3.05 [95%CI:1.86-4.99], sexual dysfunction OR:3.52 [95%CI:2.01-6.17], dissatisfaction about sexuality OR:4.77 [95%CI:2.08-10.93], low or non-existent sexual desire OR:2.94 [95%CI:1.65-5.25], daytime drowsiness OR:3.15 [95%CI:1.59-6.24] and decrease in daytime well-being OR:3.18 [95%CI:1.79-5.64]. These were factors associated with severe worsening of QoL, while the presence of genital lubrication was protective, OR: 0.44 [95%CI:0.21-0.93], p=0.0332. Conclusion: It was observed that insomnia and sexual dysfunction behaved as factors associated with three times more severe deterioration of the QoL in climacteric and sexually active women previously hysterectomized.
Introducción. Los programas universitarios del área de la salud son exigentes y la rutina es extenuante, por lo tanto, generan estrés y un potencial impacto psicológico negativo en los estudiantes. El objetivo de este estudio fue determinar la prevalencia y los factores asociados a la ansiedad y a la depresión en estudiantes del área de la salud. Metodología. Estudio de tipo transversal aplicado a estudiantes de Medicina, Enfermería y Química Farmacéutica de una universidad colombiana que diligenciaron anónima y voluntariamente un formulario con datos sociodemográficos y, a su vez, la Escala de Ansiedad y Depresión de Goldberg. Se realizó regresión logística: ansiedad o depresión (variable dependiente) y las características sociodemográficas (independientes); p<0.05, estadísticamente significativo. Resultados. Losparticipantes corresponden a 697 jóvenes, cuyas edades oscilan entre 20.3±1.7. El 43.6% estudiaba Medicina, el 26.2% Química Farmacéutica y el 30.1% Enfermería. Se identificó ansiedad en el 49.8% (intervalo de confianza 95%:46.0-53.4) y depresión en el 80.3% (intervalo de confianza 95%:77.2- 83.1). Estudiar Medicina, Enfermería, ser de sexo femenino, padecer de sobrepeso e infrapeso, haber tenido consulta previa psicológica o psiquiátrica, tener medicación permanente y sufrir de alguna enfermedad crónica, se asocia a mayor presencia de ansiedad. Proceder de zonas rurales se asoció a mayor depresión. Discusión. Los hallazgos muestran alta prevalencia de síntomas de ansiedad y depresión en el grupo de jóvenes universitarios del área de la salud. Si bien la escala utilizada no hace diagnóstico, sí sugiere la existencia de manifestaciones patológicas. Factores psicosociales, sobre todo del entorno juvenil, estuvieron asociados. Situaciones similares han sido señaladas por diferentes autores. Conclusiones. Se identificó ansiedad en la mitad de los estudiantes y depresión en ocho de cada diez. Factores inherentes a la vida universitaria, costumbres, hábitos y actividades recreativas se asociaron significativamente.
Objetivo: estimar la frecuencia de la ansiedad preoperatoria (AP) y los factores asociados enlas gestantes programadas con cesárea electiva.Métodos: estudio transversal en mujeres con embarazo de término sin actividad uterina nicomplicaciones médicas, quirúrgicas u obstétricas, realizado previo a la intervención de cesárea electiva, en Cartagena, Colombia. Participación anónima y voluntaria con consentimiento informado. Se aplicó un formulario de características sociodemográficas y la versión en español de la escala Amsterdam Preoperative Anxiety and Information Scale (APAIS). Se realizó regresión logística no ajustada para observar la asociación de las variables cualitativas con AP, se estimó el coeficiente de correlación entre las puntuaciones de las dos subescalas del APAIS con las variables cuantitativas.Resultados: se estudiaron 326 embarazadas, con edad entre 28,5 ± 6,4 años; 7,9 % adolescentes; 24,2 % primigestantes; 81,9 % con cesárea anterior y 70,8 % sin otras cirugías previamente. El 45 % informó no estar de ningún modo preocupada por la cirugía o anestesia y el 7,8 % quería saber mucho más sobre el procedimiento. La frecuencia de AP y de la necesidad de tener mucha más información de la cirugía o anestesia fue del 9,2 %. El número de familiares en la sala de espera se correlacionó negativamente con AP, rho: -0,127 [IC 95 %: -0,232-0,019]. La necesidad de mucha más información sobre la cirugía o anestesia se asoció con el aumento en la frecuencia de AP, OR: 10,9 [IC 95 %: 4,54-26,36]. También lo fueron los estados civiles: unión libre y separada, con respecto a casada: OR: 3,7[IC 95 %: 1,5-9,2] y OR: 4,4 [IC 95 %: 1,0-19,3], respectivamente.Conclusión: la frecuencia de AP previa a la cesárea fue de 9,2 %. La necesidad de tener más información sobre la cirugía o anestesia se asoció diez veces mayor la presencia de AP y el número de familiares en la sala de espera se correlacionó negativamente con AP, pero el tamaño del coeficiente de correlación se interpretó como despreciable.
