<p><strong>Objetivo: </strong>analizar la prescripción farmacológica inapropiada que existe en los pacientes de 60 años o más con diagnóstico de síndrome metabólico en la unidad de medicina familiar (umf) no. 31 del Instituto Mexicano del Seguro Social (imss).<strong> Métodos: </strong>el diseño del estudio fue descriptivo y transversal, se seleccionaron a través de un muestreo aleatorizado sistemático a 147 adultos mayores de 60 años o más, con diagnóstico de diabetes mellitus tipo 2. Se realizó la revisión de su expediente electrónico con la finalidad de integrar el diagnóstico de síndrome metabólico y revisar las notas médicas de tres meses previos a la última consulta, se registró la clase y número de medicamentos que ingerían, y se determinaron prescripciones inapropiadas, así como el tipo y el porcentaje de dichas prescripciones. Fueron aplicados los instrumentos<strong> </strong>stopp/start<strong> </strong>y<strong> </strong>beers.<strong> Resultados: </strong>se encontró dentro de la prescripción inadecuada en los pacientes analizados, el uso de diuréticos sin la presencia de insuficiencia cardiaca congestiva<strong> </strong>(icc) en 25% de los pacientes. Se determinó que casi una tercera parte de los pacientes con estreñimiento ingería de manera simultánea calcioantagonistas, lo cual está contraindicado ya que estos medicamentos disminuyen el tránsito intestinal. Además se encontró el uso combinado de antiinflamatorios no esteroideos (aines) en pacientes hipertensos, lo cual puede influir en un mal control de la cifras tensionales. <strong>Conclusión:</strong> en este estudio se demostró que la prescripción inapropiada o potencialmente riesgosa se presenta en casi un tercio de los adultos mayores; esto posiblemente está condicionado por la polipatología derivada del envejecimiento de la población y por el desconocimiento o falta de actualización en farmacología, por lo cual, es necesario establecer un mecanismo de retroalimentación para que los médicos realicen una mejor prescripción farmacológica en adultos mayores.</p>
<p><strong>Objetivo:<em> </em></strong>determinar la prevalencia de dislipidemia y riesgo cardiovascular (rcv) en pacientes con diabetes mellitus tipo 2 (dm2). <strong>Métodos:<em> </em></strong>estudio transversal analítico, se evaluaron datos de 875 pacientes derechohabientes de la Unidad de Medicina Familiar (umf) no. 31 del imss de la Ciudad de México durante el mes de julio de 2017. Se incluyeron pacientes con diagnóstico de dm2. Se analizaron parámetros bioquímicos y antropométricos para obtener la prevalencia de alteraciones lipídicas y el rcv. Se recabaron datos demográficos y antecedentes patológicos. Se realizó estadística descriptiva, cálculo de <span>χ<sup>2</sup></span> y razón de momios (rm). <strong>Resultados:<em> </em></strong>se evaluaron datos de 607 mujeres y 268 hombres, se obtuvo una mediana de edad de 57.24 años (rango intercuartil [ri] 51 a 63). La prevalencia de dislipidemia fue de 41% (n=359). El<strong> </strong>rcv<strong> </strong>severo tuvo una prevalencia de 34.2% (n=300). La hipertrigliceridemia y la enfermedad renal crónica (erc) estadio 3 estuvieron asociadas con<strong> </strong>rcv severo (p<0.05). <strong>Conclusiones:<em> </em></strong>la prevalencia de dislipidemia es alta en los pacientes con dm2 y el riesgo cardiovascular severo afecta a un tercio de esta población. Se recomienda implementar medidas para disminuir dicha prevalencia y mejorar la calidad y esperanza de vida de los pacientes.</p>
ResumenObjetivo: detectar casos nuevos de tuberculosis pulmonar mediante baciloscopia dirigida a pacientes con presencia de tos de más de dos semanas de evolución en una unidad de medicina familiar. Métodos: estudio transversal analítico. Incluyó 85 pacientes mayores de 25 años de ambos sexos, tosedores crónicos, derechohabientes de la umf no. 31 de la Ciudad de México. Los pacientes fueron invitados a participar mediante una campaña llamada "operación tuberculosis" que incluyó carteles, lonas, trípticos y platicas informativas. Se realizó tamizaje mediante baciloscopia en esputo en serie de 3. Se realizó análisis descriptivo mediante el programa stata 12. Resultados: 47 mujeres (55.29%) y 38 hombres (44.71%) fueron incluidos. La media de edad fue 62.34 años (±14.79). 33 pacientes se encontraron con índice de masa corporal normal (38.82%), 36, con sobrepeso (42.35%) y el resto, con obesidad. 35 pacientes fueron identificados por su médico como tosedores crónicos, solo cinco recibieron solicitud de baciloscopia y 23 recibieron tratamiento para infección de vías aéreas. Cinco pacientes fueron diagnosticados con tuberculosis pulmonar (tbp), tres mujeres y dos hombres, todos con diabetes mellitus tipo 2. Conclusión: el tamizaje intencionado para tbp aumenta la detección de casos nuevos, por lo que es necesario mejorar las estrategias para identificar e incluir de forma oportuna a los tosedores crónicos en el programa.
