Background. Preventive measures that are implemented through the introduction of regional prevention programs have a massive focus and do not lead to a decrease in dental morbidity, because they are based on routine preventive measures without taking into account the individual characteristics of metabolic processes in the patient's oral cavity. All this confirms the need to abandon the mass approach when carrying out preventive measures, and requires the search for modern methods for predicting dental caries, based on predictive diagnostics of the risk of caries, which should be a key element of clinical examination of the pediatric population by a dentist. Purpose of the study: on the basis of a comprehensive clinical and laboratory approach with the use of mathematical analysis of the obtained data on homeostasis of the oral cavity, to establish predictors of the development of the carious process in children in order to determine the possibility of predicting dental caries during the period of mixed bite. Methodology. To achieve this goal, a comprehensive clinical and laboratory examination of metabolic processes in the oral cavity was carried out in 60 caries-resistant children during the period of mixed bite: from 7 to 12 years. Statistical analysis was performed using the STATISTICA 8.0 software. Results. According to the results of the study, strong relationships were revealed between individual clinical and laboratory indicators-predictors, an optimal set of predictors was established for constructing classification models for patients with different types of MCS, which is a prerequisite for the possibility of predicting the risk of caries development in children during the mixed bite period. Conclusions. An optimal set of predictors has been established for constructing classification models for patients with different types of ISS for creating computer programs aimed at preclinical diagnostics of the subclinical course of the carious process with access to prediction, which will allow planning individual primary preventive measures in children during the period of active formation of hard dental tissues.
Background. At the present stage of development of scientific knowledge, to know the etiology and pathogenesis of dental diseases is impossible without a systematic approach to this problem. Currently, the problem of forecasting and the formation of “risk groups” at the stages of disease development and determining the boundary values of background indicators of diagnostic criteria in order to prevent the development of pathology and take preventive measures is relevant. In order to process the information received, elements of applied mathematics are increasingly being used, namely cluster and factor analyzes for processing the data from physiological parameters to the onset of pathology. These methods of applied mathematics are a component of modern computer software packages and are used for mathematical modeling of the clinical situation. The goal ― is to create and test a model for predicting dental caries in children in the pediatric dentistry clinic with the help of a systematic approach using elements of applied mathematics. Methods. 1158 caries-resistant children of pre-school and school age were examined for the study of dental status and the formation of a regional data bank. Results. The applied system approach with the use of elements of applied mathematics allowed introducing into practical public health computer processing of data of clinical and laboratory diagnostics of the dental status of caries resistant children of preschool and school age in order to predict the risk of developing a carious process in each individual. Findings. Predicting the risk of a carious process will increase the effectiveness of primary preventive measures, save public funds allocated in the amount of compulsory health insurance to provide dental care for children in the region, maintains caries resistance in each individual and, subsequently, will reduce the incidence of caries among the children's population at populations.

На сегодняшний день высокая заболеваемость кариесом зубов в детском возрасте остается актуальной проблемой современной профилактической стоматологии. За последние годы появилось много работ, посвященных первичной профилактике стоматологических заболеваний на основе прогнозирования риска развития кариеса зубов у детей. Все это делает проблему прогнозирования кариеса зубов, направленную на совершенствование методов индивидуальной профилактики в детском возрасте, актуальной и требующей дальнейшего изучения. Цель исследования -установить предикторные параметры минерального обмена полости рта в период сменного прикуса на основе результатов кластерного анализа. Материалы и методы. Для достижения поставленной цели проведено комплексное клинико-лабораторное обследование обменных процессов в полости рта у 60 кариесрезистентных детей в период сменного прикуса: от 7 до 12 лет. Статистический анализ проводили с использованием программ Statistica 8.0. Результаты. По результатам исследования выявлены сильные взаимосвязи между отдельными клинико-лабораторными показателями-предикторами и установлен оптимальный набор предикторов для построения моделей классификации пациентов с разным типом МКС, что является предпосылкой для возможности прогнозирования риска развития кариеса у детей в период сменного прикуса. Выводы. Полученные данные предикторных показателей минерального обмена в полости рта можно использовать для планирования индивидуальных первичных профилактических мероприятий у детей в период активного формирования твердых тканей зубов и для создания программ для ЭВМ, направленных на доклиническую диагностику субклинического течения кариозного процесса с выходом на прогнозирование.
