Introducción: el blastoma pulmonar (BP) es un cáncer pulmonar poco frecuente, corresponde al 0.5 % de las neoplasias pulmonares primarias. Desde la primera descripción en 1945, solo se han descrito alrededor de 350 casos en todo el mundo. La edad promedio de los pacientes al momento del diagnóstico es entre 39 a 53 años. Los factores de riesgo asociados al BP no están claramente definidos. El tabaquismo se ha informado hasta en el 80 % de los casos. Los síntomas reportados con mayor frecuencia son: tos, hemoptisis, disnea, infecciones respiratorias recurrentes, fiebre y pérdida de peso. La cirugía es el tratamiento de elección en el BP localizado; la radioterapia se utiliza en tumores irresecables. El uso de la quimioterapia se ha informado en pocos casos, no existe un esquema de preferencia en primera línea. El pronóstico del BP es malo, el 60 % de los pacientes mueren los primeros dos años del diagnóstico. La presencia de metástasis al momento del diagnóstico está presente en el 43 % de los casos. Las recaídas ocurren generalmente en los primeros 12 meses posteriores a la resección quirúrgica. Presentación del caso: mujer de 35 años sin antecedente de tabaquismo, cuadro clínico de dos meses de dolor torácico, tos, hemoptisis, disnea y pérdida de peso. La tomografía de tórax documenta masa de 41x45x43 mm en lóbulo inferior derecho con cavitación y necrosis. Laboratorios con anemia (Hb 10.8 g/dl) sin alteraciones en función renal ni hepática. Tuberculina, baciloscopias, prueba de VIH negativas. Fibrobroncoscopia con lesión vegetante ocluye la luz bronquial en un 100 %, sangrado fácil en bronquio para el segmento apical del lóbulo inferior derecho. Reporte de patología con necrosis y negativo para malignidad. Biopsia por Tru-cut con reporte de patología con necrosis no concluyente. Se solicita PET/CT (Figura 1) en pulmón derecho con SUV max de 14.7 masa en segmento apical del lóbulo inferior, mide 6.5x6.5x5.5cm. Se realiza lobectomía inferior y lobectomía media derecha con linfadenectomía mediastinal por toracotomía posterolateral derecha. Estudios de congelación negativos para malignidad. Reporte de patología postoperatorio informa blastoma pulmonar de 6.7 cm, necrosis presente en más del 50 % del tumor, ganglios negativos, inmunohistoquímica positiva para beta-catenina, CK, CD56 y vimentina; pT3pN0.
