Introduction:The purpose of this study was to evaluate the use of complementary and alternative therapies (CAT) by palliative care patients treated at home in Poland. Patients and methods: A total of 241 adult patients with advanced or metastatic cancer who were qualified for palliative care provided at home filled out the CAT screening tool. Data were analysed to assess CAT use association with several variables. Results: 82.16% of individuals who completed the survey declared using CAT at least once in the last 12 months. Self-help practices were the most used CAT category (74.47%), it was followed by herbal medicine and dietary supplements (62.66%) and visits to CAT providers (41.91%). CAT use was more prevalent among women, patients with basic education, and patients currently married and widowed. The most common reason pointed for using CAT was to improve well-being (35.4%). 50.5% of CAT users declared that they find used therapy helpful or very helpful. The study revealed an exceptionally high prevalence of spiritual practices (self-prayer, spiritual healing) in comparison to previous European studies conducted among the cancer patient population. Conclusions:The study indicated that usage of CAT among advanced cancer patients treated at home is significant, with a higher prevalence of spiritual practices than reported in previous studies among cancer patients in Europe.
Basal cell carcinoma (BCC) is the most frequent malignant neoplasm of the skin. If a neoplasm is recognized and an accurate treatment is implemented, the prognosis is very positive. However, negligence of the neoplasm could lead to infiltration of the adjacent bone, cartilaginous or vascular structures as well as the eyeball, which could result in its damage and anatomical deformations. The presented case illustrates the possible aggressive and destructive impact of facial skin basal cell carcinoma with rare involvement and destruction of the underlying bone. Long-term growth of the neoplasm resulted in an ulcerative lesion of the forehead area, disability, severe depression and social isolation, as well as numerous somatic complications. This article presents the issues and challenges of palliative care of a patient with advanced facial skin cancer.
Ocena i leczenie bólu przebijającego u chorych na nowotwory Evaluation and treatment of breakthrough pain in cancer patients Streszczenie Wstęp: U chorych na nowotwory mogą występować epizody bólu o znacznym natężeniu. Przebijający ból nowotworowy (BTCP, breakthrough cancer pain) to najczęstsze określenie przemijającego zwiększenia natężenia bólu, jednakże pomimo przeprowadzenia wielu badań klinicznych, nadal istnieją kontrowersje dotyczące definicji, epidemiologii i leczenia farmakologicznego BTCP. Cel pracy: W pracy dokonano przeglądu badań klinicznych dotyczących rozpoznania, oceny klinicznej i postępowania terapeutycznego u chorych z rozpoznaniem BTCP. Metody: Przeprowadzono przegląd literatury naukowej z baz Pubmed, MedLine i Embase ze szczególnym uwzględnieniem definicji, podziału, oceny i leczenia BTCP, zgodnie z wytycznymi Preferred Reporting Items for Systematic Reviews and Meta-Analyses. Wyniki: Najlepszym wyborem w terapii BTCP wydaje się być opioid o szybkim początku działania (ROO, rapid-onset opioid). Produkty fentanylu przewyższają morfinę podawaną drogą doustną zarówno pod względem skuteczności analgetycznej, jak i szybkości działania. Dostępnych jest niewiele badań klinicznych porównujących skuteczność różnych produktów fentanylu w leczeniu BTCP. Wybór analgetyku powinien być dokonywany indywidualnie, w zależności od aktualnego stanu klinicznego pacjenta. Dawka leku stosowanego w leczeniu BTCP powinna być miareczkowana, jednakże przy początkowej dawce należy również uwzględnić tolerancję na opioidy podawane długotrwale w leczeniu bólu podstawowego. Algorytmy rozpoznania BTCP powinny być wykorzystywane wraz z dokładną i kompleksową oceną stanu klinicznego pacjenta.
