Opioidiriippuvuuden korvaushoidon potilaiden siirtyminen apteekkijakelun ja perusterveydenhuollon piiriin ei ole toteutunut viimeisimmän korvaushoitoasetuksen (33/2008) tavoitteiden mukaisesti. Tässä artikkelissa tarkastellaan korvaushoidon työntekijöiden näkemyksiä siitä, mitkä tekijät vaikuttavat korvaushoitolääkkeen valintaan ja potilaan mahdollisuuteen siirtyä apteekkijakeluun. Artikkeli perustuu korvaushoitoyksiköiden työntekijöiden yksilö- ja ryhmähaastattelujen (N=40) ja työntekijöille suunnatun kyselyn (N=88) avovastausten teemakohtaisiin aineistolähtöisiin sisällönanalyyseihin. Työntekijöiden haastattelujen perusteella korvaushoidon käytännöt ja lääkevalintojen perustelut vaihtelivat eri paikkakuntien välillä. Buprenorfiinia käytettiin yleensä korvaavana lääkkeenä kuntouttavassa ja metadonia haittoja vähentävässä korvaushoidossa, mutta näkemykset buprenorfiinin ja metadonin hyödyistä ja haitoista suhteessa toisiinsa ja eri potilasryhmiin vaihtelivat. Työntekijöiden mukaan metadonin käytön yleistymiseen korvaushoidossa on voinut vaikuttaa sen alhaisempi hinta. Apteekkijakeluun siirtymisestä oli myönteisiä kokemuksia: se voi vahvistaa potilaan vastuunottoa omasta kuntoutumisestaan, vähentää hoitoon liittyvää stigmaa ja normalisoida hoitoa tavalliseksi sairaudenhoidoksi. Korvaushoitolääkkeen omavastuuosuus, jatkohoitopaikkojen vähyys, hoitoyksikön vaihtumiseen liittyvät tekijät sekä oheiskäytön jatkuminen ovat kuitenkin estäneet potilaiden siirtymisiä apteekkijakeluun. Haastattelujen perusteella työntekijät kaipaavat valtakunnallisia hoitolinjauksia, mutta korostavat myös yksilöllisen hoidon merkitystä. Hoidon toteutuksessa tarvittaisiin edelleen yhtenäisempää näkemystä hoidon tavoitteista ja periaatteista. Hoidon kehittäminen osana laajempaa hoito- ja kuntoutusjärjestelmää vaatii sosiaali- ja terveydenhuollon toimijoiden suunnitelmallisempaa yhteistyötä. Uusi pistettävä pitkävaikutteinen lääke voi vähentää merkittävästi apteekkijakelun tarvetta.
