Resumo OBJETIVO Identificar as reações adversas ligadas à infusão de células-tronco hematopoéticas no dia zero do transplante de células-tronco hematopoéticas. MÉTODO Revisão integrativa da literatura, sem recorte temporal, a partir de fontes de informação: PubMed, CINAHL, SCOPUS, BVS, SciELO, Web of Science e CAPES; a amostra final foi constituída por dezoito artigos científicos, publicados entre 1998 e 2017, com base nos critérios de inclusão e exclusão. RESULTADOS Reações adversas leves e moderadas foram mais frequentes nos estudos que utilizaram a classificação por severidade, sendo náusea e êmese as de maior incidência; os sistemas orgânicos mais afetados foram o cardiovascular, respiratório e gastrointestinal. CONCLUSÕES As principais reações adversas identificadas nos estudos foram náusea e êmese. As classificadas como leves e moderadas foram as mais frequentes nos estudos que utilizaram a classificação por severidade; e os sistemas cardiovascular, respiratório e gastrointestinal foram os mais afetados naqueles que utilizaram a classificação por sistemas orgânicos.
RESUMO: Este estudo objetivou buscar evidências científicas que abordassem os principais erros de medicação e suas causas observados pela enfermagem nas instituições hospitalares de internação integral, bem como, descrever as estratégias utilizadas para promover a segurança medicamentosa nestas instituições. Foram selecionados 79 artigos de três bases de dados, entre os anos de 2004 a 2015. Os tipos de erros mais citados foram dose imprópria 35 (13,3%), omissão de dose 30 (11,5%), e horário errado 29 (11,1%). As causas mais citadas foram as relacionadas ao fator humano 43 (35,2%), sistema 37 (30,3%) e comunicação 22 (18%). As estratégias que se destacaram foram implantação de protocolos de segurança de preparo e administração de medicamentos, prescrição eletrônica, inclusão do profissional farmacêutico na equipe e treinamento/orientação da equipe. Acredita-se que esta revisão possa contribuir para melhoria do processo de administração de medicamentos e para criação de estratégias de prevenção que fomentem a segurança do paciente.
Objective: to report the use experience of convergent healthcare research for developing a nursing care protocol. Method: convergent care research developed in university hospital, from July to December 2016, with 27 participants. Results: the stages of the research and its results are described in the steps: conception, instrumentation, screening and analysis. The end result was the nursing care protocol in day zero of hematopoietic stem cell transplantation. Conclusion: convergent care research was an appropriate method for developing the care protocol, and an important contribution to the approximation between theory and practice. The nursing care protocol was the result of this study and confirmed both the purpose of the research as a professional Master's in acquiring knowledge aimed at improving professional practice.
RESUMOO estudo objetivou identificar os cuidados do enfermeiro no Dia Zero do Transplante de células-tronco hematopoéticas e identificar as reações adversas apresentadas pelos pacientes neste dia. Realizou-se pesquisa descritiva com abordagem qualitativa na Unidade de Internação de um Serviço de Transplante de Medula Óssea de um hospital universitário da região sul do Brasil. No total, 11 enfermeiros fizeram parte da pesquisa. A coleta de dados ocorreu entre julho e setembro de 2016, em dez dias de transplantes, e totalizou 72 horas de observação passiva. Os dados foram registrados em instrumento previamente elaborado e analisados em categorias preestabelecidas. Foram observados transplantes alogênicos aparentados e não aparentados, os quais utilizaram como fonte de células-tronco hematopoéticas medula óssea, sangue periférico e sangue de cordão umbilical e placentário. As modalidades de infusão observadas foram criopreservada-descongelada e fresca, e para esta, classificou-se a compatibilidade do sistema ABO em compatível, incompatível ABO maior e incompatível ABO menor. Observou-se reações adversas em oito dos dez transplantes, sendo a mais prevalente hipertensão arterial sistêmica. Os cuidados do enfermeiro foram agrupados em antes, durante e após a infusão de células-tronco hematopoéticas, entre eles: administração de medicações prescritas pelo médico pré-infusão de células-tronco hematopoéticas (antes); aferição de sinais vitais (durante), monitoramento de aspecto e volume de diurese e balanço hídrico (após). Concluiu-se que os cuidados prestados ao paciente no Dia Zero do Transplante de células-tronco hematopoéticas pelo enfermeiro visam prevenir, detectar e intervir precocemente em reações adversas relacionadas ao procedimento de infusão das células-tronco hematopoéticas. ABSTR ACT The aim of the study was to identify the care provided by nurses on the Day Zero of Transplantation of hematopoietic stem cells and identify the adverse reactions presented by patients on this day. A descriptive study of qualitative approach was conducted at the inpatient unit of a bone marrow transplant service of a university hospital in the southern region of Brazil. Objective: to construct a protocol of nursing care to the patient on day zero of hematopoietic stem cell transplantation. Method: a convergent care research was developed from August to December 2016 in a Bone Marrow Transplant Service. The participants were twenty-two nurses from this service. The technique of data collection used was discussion groups. For the analysis the following steps were taken: transcription of the data, highlighting the suggestions of the participants; distribution of contributions by theme, for synthesis of the elements in a coherent whole, scientific evidence and contributions of the participants; and construction of the protocol, with refinement and approval of the final version by nurses. Results: the protocol, guides nursing care to be provided by the nurse on day zero of hematopoietic stem cell transplantation, according to the infusion mode: fresh and cryopreserved-thawed. These precautions aim to prevent, identify and intervene early in complications related to cell infusion. Conclusion: the protocol, product of the research, was elaborated in the union of scientific evidences, with the reality of the service and the experience of the participating nurses. The utilization of the methodological steps of convergent care research was a facilitator, because, as it presupposes, it provided the union of care practice with scientific research. The participation of nurses in the construction and approval of the protocol enabled the subsequent implementation and use of this tool in nursing service.
