BackgroundThroughout the world all health professionals face stress because of time-pressures, workload, multiple roles and emotional issues. Stress does not only exist among the health professionals but also in medical students. Bangladesh has currently 77 medical colleges 54 of which are private. This study was designed to collect baseline data of stress-level among Bangladeshi students, which we believe will form the basis for further in depth studies.MethodsA cross-sectional study was conducted on medical students from 2 public and 6 private medical-schools in Bangladesh. All medical schools have common curriculum formulated by the Government of Bangladesh. The study population was 1,363 medical students of Year-III and IV of academic session 2013/2014. Universal sampling technique was used. The period of study was February to June 2014. Data was collected using a validated instrument, compiled and analysed using SPSS version-20.ResultsA total of 990 (73%) out 1,363 medical students participated in the study, of which 36% were male and 64% were female. The overall prevalence of stress of the study population was 54%. 53% of male and 55% of female were reported suffering from stress. 54% of Year-III students and 55% of Year-IV were noted suffering from stress. There was statistically significant (p = 0.005) differences in the level of stress between public (2.84 ± 0.59) and private (2.73 ± 0.57) medical schools student.ConclusionsMore than half of Bangladeshi medical students are suffering from measureable academic stress. SummaryBackgroundTrivalent oral polio vaccine (tOPV) was replaced worldwide from April, 2016, by bivalent types 1 and 3 oral polio vaccine (bOPV) and one dose of inactivated polio vaccine (IPV) where available. The risk of transmission of type 2 poliovirus or Sabin 2 virus on re-introduction or resurgence of type 2 poliovirus after this switch is not understood completely. We aimed to assess the risk of Sabin 2 transmission after a polio vaccination campaign with a monovalent type 2 oral polio vaccine (mOPV2).MethodsWe did an open-label cluster-randomised trial in villages in the Matlab region of Bangladesh. We randomly allocated villages (clusters) to either: tOPV at age 6 weeks, 10 weeks, and 14 weeks; or bOPV at age 6 weeks, 10 weeks, and 14 weeks and either one dose of IPV at age 14 weeks or two doses of IPV at age 14 weeks and 18 weeks. After completion of enrolment, we implemented an mOPV2 vaccination campaign that targeted 40% of children younger than 5 years, regardless of enrolment status. The primary outcome was Sabin 2 incidence in the 10 weeks after the campaign in per-protocol infants who did not receive mOPV2, as assessed by faecal shedding of Sabin 2 by reverse transcriptase quantitative PCR (RT-qPCR). The effect of previous immunity on incidence was also investigated with a dynamical model of poliovirus transmission to observe prevalence and incidence of Sabin 2 virus. This trial is registered at ClinicalTrials.gov, number NCT02477046.FindingsBetween April 30, 2015, and Jan 14, 2016, individuals from 67 villages were enrolled to the study. 22 villages (300 infants) were randomly assigned tOPV, 23 villages (310 infants) were allocated bOPV and one dose of IPV, and 22 villages (329 infants) were assigned bOPV and two doses of IPV. Faecal shedding of Sabin 2 in infants who did not receive the mOPV2 challenge did not differ between children immunised with bOPV and one or two doses of IPV and those who received tOPV (15 of 252 [6%] vs six of 122 [4%]; odds ratio [OR] 1·29, 95% CI 0·45–3·72; p=0·310). However, faecal shedding of Sabin 2 in household contacts was increased significantly with bOPV and one or two doses of IPV compared with tOPV (17 of 751 [2%] vs three of 353 [1%]; OR 3·60, 95% CI 0·82–15·9; p=0·045). Dynamical modelling of within-household incidence showed that immunity in household contacts limited transmission.InterpretationIn this study, simulating 1 year of tOPV cessation, Sabin 2 transmission was higher in household contacts of mOPV2 recipients in villages receiving bOPV and either one or two doses of IPV, but transmission was not increased in the community as a whole as shown by the non-significant difference in incidence among infants. Objects: A hospital based descriptive study was carried out among under-5 children suffering from severe protein-energy-malnutrition (PEM) Methodology: This was a descriptive analytical study conducted during June 2002 to May 2003 in three purposively selected hospitals of Dhaka city, Bangladesh. Results: Mean±SD age of the children was 15.1±10.9 months, majority being under three years (91.5%), 47.2% were female, and 51% did not receive or complete immunization according to their age. No significant age and sex difference was found among the admitted children in different hospitals. Most of their parents were illiterate (mothers 71%, fathers 56%) with low family income and social status. Marasmus cases were 61%, followed by kwashiorkor 25% and marasmic kwashiorkor 14%. The children were managed broadly according to standard protocol based on the guideline of World Health Organization but adopted/modified by the individual hospitals. Overall outcome of the patients were: recovery 62.4%, default 21.4%, death 13.7% and transfer 2.6%. Mean duration of hospital stay was 14.2±9.9 (ranging from 0 to 59) days. Recovery rate was better among male (67.3%) than female (56.8%) children, p=0.033. Default rate was higher among female (27.3%) than male (16.1%) children, p=0.007. Case fatality rate was highest (40%, p=0.001) in children suffering from septicaemia. Conclusion: Evidences of faulty case management practices viz. over use of intravenous fluids and blood transfusion, inappropriate diets, less use of micronutrients, incorrect antibiotics were observed occasionally in all the three hospitals. Updated and comprehensive treatment guidelines need to be implemented, and every hospital should review its own clinical management procedures and performance with a view to improving practice. Summary The emergence of circulating vaccine-derived polioviruses through evolution of the oral polio vaccine (OPV) poses a significant obstacle to polio eradication. Understanding the early genetic changes that occur as OPV evolves and transmits is important for preventing future outbreaks. Here, we use deep sequencing to define the evolutionary trajectories of type 2 OPV in a vaccine trial. By sequencing 497 longitudinal stool samples from 271 OPV2 recipients and household contacts, we were able to examine the extent of convergent evolution in vaccinated individuals and the amount of viral diversity that is transmitted. In addition to rapid reversion of key attenuating mutations, we identify strong selection at 19 sites across the genome. We find that a tight transmission bottleneck limits the onward transmission of these early adaptive mutations. Our results highlight the distinct evolutionary dynamics of live attenuated virus vaccines and have important implications for the success of next-generation OPV.

Since the global withdrawal of Sabin 2 oral poliovirus vaccine (OPV) from routine immunization, the Global Polio Eradication Initiative (GPEI) has reported multiple circulating vaccine-derived poliovirus type 2 (cVDPV2) outbreaks. Here, we generated an agent-based, mechanistic model designed to assess OPV-related vaccine virus transmission risk in populations with heterogeneous immunity, demography, and social mixing patterns. To showcase the utility of our model, we present a simulation of mOPV2-related Sabin 2 transmission in rural Matlab, Bangladesh based on stool samples collected from infants and their household contacts during an mOPV2 clinical trial. Sabin 2 transmission following the mOPV2 clinical trial was replicated by specifying multiple, heterogeneous contact rates based on household and community membership. Once calibrated, the model generated Matlab-specific insights regarding poliovirus transmission following an accidental point importation or mass vaccination event. We also show that assuming homogeneous contact rates (mass action), as is common of poliovirus forecast models, does not accurately represent the clinical trial and risks overestimating forecasted poliovirus outbreak probability. Our study identifies household and community structure as an important source of transmission heterogeneity when assessing OPV-related transmission risk and provides a calibratable framework for expanding these analyses to other populations. Objectives : There is gradual reduction of exclusive breast feeding practice as age advances from birth to 6 months. The aim of the study was to see the status of exclusive breast feeding up to 6 month of age, among the babies of working mothers and housewife mothers and also to see different contributing factors among the working mothers for the initiation of breast milk substitute within this period.

Introduction:The Neurological injury is an important complication after CABG. There is considerable evidence that early postoperative cognitive dysfunction is related to a combination of three factors often associated with CPB: (micro) embolism, hypoperfusion, and the systemic inflammatory response. Intraoperative formation of gaseous emboli and aggregated platelets, atherosclerotic debris, hypoperfusion, hypotension, hyperthermia, hyperglycaemia, surgical trauma, blood loss, Anaesthetic agents, arrhythmia and transfusion all enhance the risk of cognitive dysfunction.There are two types of Neurological injury, Type-I includes stroke, transient ischemic attack and coma and the incidence is approximately 3 to 6 percent. Type-II injury is more subtle and includes impairment of cognitive functions. These defects associated with attention, concentration, short term memory, fine motor function and speed of the mental response. The incidence of neurocognitive impairment after cardiac surgery varies from 20% to 80%. 1 Based on prospective studies, however it is apparent that the incidence of subtle postoperative neurologic and neuropsychological abnormalities is much higher, closer to 50 percent in the first week after cardiac surgery. 2 These apparent high rate of subtle neurologic impairment detected prospectively are in sharp contrast to the considerably lower incidence of stroke after cardiac surgery, reported as 1-5 percent, in several large retrospective series from different centers. 3 There are several reasons for these apparent differences in the reported incidences the timing, thoroughness and the reproducibility of the neurologic examinations, as well as the incorporation of the preoperative assessment for comparison, all determine the sensitivity and Evaluation of Neurocognitive Dysfunction afterCoronary Artery Bypass Surgery p = 0.002 and 25.93 ± 3.11 vs. 26.63 ± 2.50, p = 0.023 respectively

Background: Platelet indices (plateletcrit, mean platelet volume, platelet distribution width and Platelet count) are potentially useful markers for the early diagnosis and outcome of many diseases. Platelet indices could serve as surrogate marker for remission in patients with ALL. Objective: To observe change of platelet indices in ALL during induction of remission. Material & Methods: This observational study was carried out with 52 newly diagnosed ALL patients ranging from 1.5 to 12 years admitted at DMCH, from January to December 2015. Platelet indices such as plateletcrit (PCT), mean platelet volume (MPV), platelet distribution width (PDW) and platelet count (PLT) were monitored from admission to induction of remission phase of chemotherapy and were analyzed. Result: Mean PLT was found 93442.3±29966.4 cmm before treatment, 137442.3±27217.9 cmm in 1st week and 231653.8±42543.5 cmm in 4th week. Mean PCT was found 0.09±0.11% before treatment, 0.16±0.11% in 1st week and 0.25±0.18 % in 4th week. Both PLT & PCT was increased significantly during induction of remission after one and four weeks. Mean PDW was found 13.1±3.9 fl before treatment, 12.9±3.5 fl in 1st week and 12.0±3.1 fl in 4th week. MPV was found 10.6±2.1 fl before treatment, 11.0±1.4 fl in 1st week and 10.7±1.4 fl in 4th week. The change of MPV & PDW were not statistically significant when compared with that of before treatment. Conclusion: It can be concluded that among four important platelet indices, PLT and PCT were significantly associated with remission in ALL during induction of remission.
