Мета: дослідження можливості використання комп'ютерного аналізу тонів серця у скринінговій діагностиці стану артеріальної протоки у недоношених дітей на підставі аналізу параметрів комп'ютерної фонокардіограми при різних ступенях гемодимічної значущості відкритої артеріальної протоки (ВАП). Матеріали і методи. Обстежено 45 недоношених новонароджених дітей, що знаходилися на лікуванні у відділенні інтенсивної терапії. Проведені фонокардіографія (ФКГ) за допомогою електронного стетоскопа, ехокардіографія. 40 (89%) новонароджених мали ультразвукові ознаки гемодина мічно незначущої ВАП (ВАП з незначним шунтуванням), 5 (11%) немовлят мали ознаки гемодинамічно значущої ВАП (ГЗВАП). Проведено порівнян ня показників комп'ютерного аналізу ФКГ, що характеризують тони серця та проміжок між першим і другим тоном, між групами недоношених дітей із різним ступенем шунтування через ВАП. Результати. При порівнянні показників ФКГ у групах дітей з незначним та помірним шунтуванням через ВАП в усіх точках вислуховування були зафік совані достовірні відмінності в параметрах, які характеризують проміжок між першим і другим тоном (ознаки систолічного шуму). При порівнянні даних комп'ютерного аналізу ФКГ груп з помірним і значним шунтуванням окрім відмінностей у показниках, які характеризують проміжки між тона ми, в першій та другій точках вислуховування достовірно відрізнялись співвідношення параметрів першого та другого тонів (що може характеризу вати різницю в об'ємах шунтування крові). Висновки. Встановлені статистично значущі відмінності показників комп'ютерного аналізу ФКГ у недоношених новонароджених з різними ступенями гемодинамічної значущості ВАП. Отримані результати можуть бути використані в якості скринінгового тесту у недоношених новонароджених з ВАП. Дослідження виконані відповідно до принципів Гельсінської Декларації. Протокол дослідження ухвалений Локальним етичним комітетом установи. На проведення досліджень було отримано інформовану згоду батьків, дітей. Автори заявляють про відсутність конфлікту інтересів. Background. Тhe incidence of diagnostic errors in neonatology and their impact on children’s health remain relatively unknown. The limited available evidence, however, suggests that diagnostic errors in the neonatal unit result in significant and long-term consequences. The purpose of the study was to generalize the literature data on diagnostic errors in neonatal units in view of their prevalence, impact on health status, and contributing factors. Materials and methods. The search was done in the PubMed Central® database using the keywords “misdiagnosis”, “diagnostic errors”, “neonatal intensive care unit”, “neonatal unit”, “neonatology”, “newborns”. Results. Methods to identify diagnostic errors were analyzed: autopsy results, consideration of complaints and conflicts, voluntary survey of doctors and patients, symptom-disease pair (SPADE approach), trigger methodology, Safer Dx Instrument (measurement and improvement of diagnostic safety). The incidence of detecting diagnosis errors ranged from 19.2 % when analyzing pathology studies (including 3.7 % of those that harmed a patient) to 6.2 % when using Safer Dx Instrument (including 8.0 % of those that that harmed a patient). A real assessment of this situation can be provided by a combination of the above methods. Conclusions. Further research is needed to better quantify misdiagnosis in neonatal intensive care units and to identify potential strategies to reduce its incidence or mitigate the harm associated with misdiagnosis.
The use of home teleconsultation in outpatient pediatric practice has become widespread in many countries over the world, regardless of their economic situation. Each country has its own experience in the practical implementation of home teleconsultation in medical care for patients, but the issue of increasing its effectiveness remains topical. Purpose - to consider the issue of improving the efficiency and safety of home teleconsultation for children under 5 years old with fever. Materials and methods. The terms «home teleconsultation», «telemedicine sorting», «children», «fever» were searched using Pub Med Central databases. The data of the regulatory and legal protocols in Ukraine were taken into account while determining the recommendations. Results. Research in recent years proves that new forms of communication between a doctor and a patient, his parents or guardians create both additional opportunities and risks regarding various aspects of the quality of medical care. According to the data of literary sources, conclusions were made that the quality of home teleconsultation for children under 5 years of age is affected by: social and ethical features of communication with children and their parents; lack of standards/scenarios for telephone consultation for a specific pathology; lack of appropriate professional training of doctors. The article provides measures to improve home teleconsultation: the use of telesorting scenarios (NICE protocol/standard for children under 5 years with fever) using a «traffic light system», alarm symptoms («red flags») to predict the risk of serious illness and taking into account the availability of the use of the «safety net». There is a well-founded need for professional training of medical workers in the field of telemedical consultation. Conclusions. The implementation of home teleconsultation services in outpatient pediatric practice in situations where a child’s condition is assessed by a medical professional who cannot examine him because the child is geographically distant will contribute to a faster setting of priorities for the provision of primary medical care. In today’s environment, the NICE (2021) protocol/standard for the assessment and initial treatment of fever in children under 5 years of age is a clinical scenario that meets the requirements for home teleconsultation. The introduction of the leading international experience of organizing home teleconsultation for children under 5 years of age is the basis for improving programs of continuous professional experience of medical workers who provide assistance to children.
