Получена оценка распространенности анатомических вариаций и асимметрии передней стенки грудной клетки, влияющих на контур молочных желез, а также на размер, форму и расположение лоскутов или им-плантатов при аугментационных вмешательствах. Проведено исследование с целью оценки и прогнозиро-вания степени асимметрии молочной железы после проведения аугментационных вмешательств. Дизайн ис-следования: сравнительное проспективное (в параллельных группах). Длительность исследования -24 мес (январь 2012 г. -март 2014 г.).Материал и методы. Под наблюдением находились 40 пациенток, которым произведена рентгеновская компьютерная томография. Для оценки влияния возраста на асимметрию грудной клетки пациентки были разде-лены на две группы -моложе и старше 45 лет. Период наблюдения после проведенной операции составил 6 мес.Результаты. Клинически значимая асимметричная деформация грудной клетки должна выявляться на ос-нове определенной совокупности ее размеров, а полученные данные учитываться при реконструктивно-пла-стических вмешательствах на молочных железах и мягких тканях грудной стенки.Заключение. Показано, что при выполнении аугментационной маммопластики в 15 % случаев у пациен-ток с более выпуклой передней стенкой грудной клетки определяется смещение проекции молочных желез в лате ральные стороны, а при уплощенной грудной клетке может наблюдаться выраженное медиальное смеще-ние сосков.Ключевые слова: грудная клетка, молочные железы, асимметрия грудной клетки, аугментационная маммо пластика.
Augmentation mammoplasty is one of the most demanded and most frequently performed plastic surgeries in the world. The first breast augmentation using first-generation implants performed in 1962. Since, the surgery technique and the quality of implants have been improved. Although the number of complications significantly decreased, they still occur both in the early and the late postoperative periods. Often complications include seroma, hematoma, asymmetry, double bubble (double fold), implant displacement, rippling, capsular contracture, etc. The most formidable late complication described in 1997, is breast implant-associated anaplastic large cell lymphoma (BIA-ALCL), which is manifested more than after a year, by the formation of the late malignant seroma after implantation. A review of modern literature provides a clear idea of the number of received complications. However, the pathogenesis of the development of some of them (capsular contracture, implant-associated lymphoma) is still unclear, and therefore, further long-term studies are needed to obtain more accurate data. This paper presents an assessment of the results of mastopexy surgery carried out by using the technique of autologous breast enlargement axillary lateral flap, which the authors developed for patients who have suffered a decline in breast volume, as a result of massive weight loss after bariatric surgery. The mastopexy was carried out by the improved method with preliminary Doppler ultrasound of the perforating branches of intercostal arteries in women after a significant reduction in body weight. This method provides a good aesthetic result with the correct position of the nipple and a satisfactory volume of the breast and with a simultaneous removal of excessive skin flaps and excess fat in the anterolateral area of the chest.
