ZusammenfassungUnter den 714 Schadelhirnverletzten, die yon 1969 bis 1972 stationar behandelt wurden, fanden sich 111 rnit Mi~telhirnsyndrom (MHS). Davon wurden 99 vergleichenden klinischen Untersuehungen unterzogen. Alle Patien~en wurden rail den gteichen Intensivmaf~nahmen und jeder zweite zusgtzlieh noeh mit der hyperbaren Oxygenation (HO) behandelt. Der Vergleieh zwisehen den nut intensiv behandelten (Gruppe A) und den noeh zusatzlieh mit der HO behandelten Fallen (Gruppe B) ergab: 1. Die L'berlebenszeit war bei den Fallen der Gruppe B wesentlieh 1/~nger. 2. Die L'berlebensrate war wesentlieh gr61?er in der Gruppe B sowohl bei den F~llen, die eine Kontusio, als aueh bei denen, die eine intrakranielle Blutung als Ursaehe des MI-IS batten. 3. Die Uberlebensquote war in den Altersklassen vom 1. bis 30. Lebensjahr wesentlieh h6her bei den zusatzlieh mit I-tO behandelten Patienten. Vom 40. Lebensjahr ab lieB sieh nut noett ein geringer Unterschied zugunsten der Falle der Gruppe B erkennen. 4. Am Ende der stationaren Behandlung waren in der Gruppe A 74% verstorben oder apalliseh, in der Oruppe B 53O/o . Eine vollst/indige Wiederherstellung wurde bei nut 6O/o der Gruppe A und bei 33% der Gruppe B festgestelli~. I)iese Ergebnisse zeigen, dal? dutch die zusgtzliehe Anwendung der I~IO das traumatisehe MHS in wesentlieh gr6Berem Mage reversibel ist und dal3 es unter Anwendung der HO bei einer erheblieh grSl~eren Anzahl der Falle mit MHS zu einer vollstandigen Wiederherstellung kommt. Der Einsatz der I-IO bei Fallen mit traumatisehem MHS hat sieh also bewahrt. Vor allem bei j/ingeren Patienten (1.--30. Lebensjahr) mit traumatisehem Mittelhirnsyndrom verbessert eine zusatzliehe Behandlung mit hyperbarer Oxygenation die Prognose erheblieh. Ilare Anwendung sollte deshatb besonders bei diesen F/illen gefordert werden.
ZusammenfassungVoraussetzungen, methodische Grundlagen, Anwendung und Ergebnisse eines neuen Verfahrens zur pr/~operativen Artdiagnostik hirnorganiseher Prozesse werden besehrieben. Auf der Basis des Bayesschen Theorems zur Wahrscheinlichkeitseinschgtzung yon I-Iypothesen wurde ein Computerprogramm entwickelt, das es an Hand yon Inzidenzmatrizen ermSglicht, aus den bei einem Fall erhobenen klinisehen und szintigraphischen Befunden prozentuale Wahrscheinlichkeiten fiir das Vorliegen yon zun/~chst neun verschiedenen Erkrankungen reehnerisch zu ermitteln. Das in einer ersten Ausbaustufe fertiggestellte Programm konnte an 85 histologiseh verifizierten F/~llen erprobt werden und fiihrte in 88% dieser F/~lle zu richtigen bzw. fakultativ richtigen Diagnosen. Die vorgesehene Weiterentwicklung sowie zuktinftige M6gliehkeiten einer Optimierung der neuen Methode werden dargelegt. Summary
1. The use of rapid sequential gamma camera scintigraphy in conjunction with computer aided storage and evaluation of data has been shown to be a promising method for the analysis of CSF dynamics in man. 2. Using this method rapid flow of CSF in the basal cisterns can be studied. The further course of tracer activity in different CSF compartments can be followed, and slow currents may be demonstrated. 3. Using this technique the patterns of varying regional CSF dynamics and reabsorption states can be examined. Intracranial activity after 24 hours is 40 to 45% anar behaviour of CSF circulation patterns in the basal cisterns, cisterna magna, and upper cervical area leads us to suggest that this complex of spaces forms a functional unity which may be called "The distribution center of CSF". Further movements of tracer substance seem to start from this complex. 5. In the well recognised CSF pathways over the cerebral convexities some characteristic patterns of fluid flow are demonstrated. The flow times at different situations over the convexities are given. 6. Analysis of spinal CSF flow shows that there are considerable variations in spinal CSF dynamics. 7. Finally, the different uses of this method in various situations are indicated.
We report two patients with a spinal and intracranial meningioma without evidence of von Recklinghausen's disease. The radiological and scintigraphical findings are demonstrated. Certain implications with regard to the examination and follow-up of patients with a meningioma are indicated.
