To assess the long-term effects of juvenile idiopathic arthritis in adulthood, unified diagnostic methods for articular and extra-articular lesions should be used which depend on the juvenile idiopathic arthritis variants, the disease activity and treatment.
Rheumatoid Arthritis (RA) is a chronic inflammatory rheumatic disease and a frequent cause of secondary osteoporosis induced by the chronic inflammatory conditions and a long-term glucocorticoid therapy (GC). Patients with RA have a greater risk of osteoporosis and low-energy fractures than a general population [1]. Bone disorders are main extra-articular complications of rheumatoid arthritis [2]. According to our previous data, osteoporosis in women with RA aged 50-59 years was observed in 25.6% patients at lumbar spine; in 30.8% cases at the femoral neck and 33.4% -at total radius [3].Glucocorticoid therapy is usually associated with increased fracture risk that cannot be fully explained by decreased Bone Mineral Density (BMD); this may be a consequence of alterations in the micro-architectural properties of bone. Other well known risk factors of systemic osteoporosis at RA are age, duration of postmenopausal period in women, low body mass index, reduced physical activity and duration of disease [4][5][6]. The TBS L 1 -L 4 is a new method which can be measured by DXA, and correlates with parameters of bone micro architecture [7,8]. According some literature data the combining TBS index with BMD is important in complex assessment of bone status. However, there are insufficient data about combination of BMD and TBS in bone assessment in women with RA.
This case report describes the occurrence of symmetrical dactylitis of toes combined with chilblain-like acral lesions in a 32-year-old female patient 19 days after a mild coronavirus infection. The article addresses existing problems of managing patients after COVID-19 in daily clinical practice. Scientific evidence is pointing to a growing number of cases of articular and skin involvement associated with COVID-19. However, it remains unclear what approaches to use in the treatment of such patients.
Науковцями всього світу визнана проблема остео порозу при ревматичних захворюваннях. Всесвітньою організацією охорони здоров'я ревматоїдний артрит (РА) виділено як самостійний чинник виникнення порушень структурно-функціонального стану кісткової тканини. Хворі на РА мають підвищений ризик розвитку системного остеопорозу та остео поротичних переломів унаслідок багатьох факторів, серед яких використання глюкокортикоїдів (ГК) посідає особливе місце. Вплив ГК на кісткову тканину обумовлений каскадом патологічних реакцій: збільшення спричиненої остеокластами резорбції, зниження формування кісткової тканину остеобластами, апоптоз остеоцитів, зменшення абсорбції кальцію в кишечнику, збільшення секреції паратгормона, підвищення реабсорбції кальцію нирками, зниження рівня статевих гормонів шляхом пригнічення секреції гонадотропіну, порушення метаболізму вітаміну D [1]. Визнаними факторами ризику системного остеопорозу при РА є вік, тривалість постменопаузаль-Резюме. Стаття присвячена вивченню показника якості трабекулярної кісткової тканини (TBS) у хворих на ревматоїдний артрит (РА). Обстежено 134 хворих на РА жінок віком від 31 до 78 років (середній вік-52,4 ± 12,7 роки; зріст-162,6 ± 6,4 см; маса-68,5 ± 13,8 кг; середня тривалість захворювання-9,1 ± 7,6 року). При вивченні впливу віку на структурно-функціональний стан кісткової тканини обстежених розподілили на п'ять груп за десятиріччями. З метою оцінки впливу глюкокортикоїдів (ГК) на показник якості трабекулярної кістки та мінеральної щільності кісткової тканини (МЩКТ) пацієнтів розподілили на три групи: 1-шу групу склали 37 пацієнтів, які не приймали ГК; 2-гу групу-47 пацієнток, яким призначалися ГК тільки при загостренні на короткий, менше 6 міс., термін; 3-тю групу-50 хворих, які системно отримували ГК у дозі ≥ 5 мг за преднізолоном понад 3 роки. Методом двохенергетичної рентгенівської денситометрії (Prodigy, GE Lunar, Мадісон, США) визначали МЩКТ на рівні поперекового відділу хребта, проксимального відділу стегнової та променевої кісток. Показник якості трабекулярної кісткової тканини поперекового відділу хребта визначали за допомогою методики TBS iNsight (Med-Imaps, Pessac, Франція). Встановлено, що починаючи з 50-річного віку вірогідно знижуються показники TBS L 1-L 4 (1,156 ± 0,140 проти 1,318 ± 0,155 у жінок вікової групи 30-39 років; t = 3,5; p = 0,001) та МЩКТ різних відділів скелета в жінок, хворих на РА. Прийом ГК асоційований із порушенням якості кісткової тканини. Виявлено, що при системному призначенні ГК (у 3-й групі порівняно з 1-ю групою) вірогідно нижчий показник TBS L 1-L 4 (1,147 ± 0,168 проти 1,250 ± 0,135; t =-3,07; p = 0,003), у той час як вірогідних змін показників МЩКТ поперекового відділу хребта та стегнової кістки встановлено не було. Протягом року спостереження показник TBS L 1-L 4 у 1-й групі зменшився на 1,4 %, у 3-й групі-на 5,8 %. Отже, визначення TBS L 1-L 4 є методикою, що дає можливість оцінити вплив ГК на кісткову тканину і динаміку втрати її в результаті лікування. Background. Nowadays in the field of syndromes and diseases associated with age, scientists focus especial attention on the problem of sarcopenia, which combines an increased risk of falls, deterioration of life quality, impaired functional activity, reduced life expectancy and increased mortality of patients. In 2016, sarcopenia has been included in the Internatio nal Classification of Diseases. There are the primary and secon dary forms of sarcopenia. This article presents the reference data and results of our own studies on sarcopenia in the rheumatoid arthritis patients. The aim of this study was to evaluate the bone mineral density, lean mass and frequency of pre-scarcopenia in patients with rheumatoid arthritis. Materials and me thods. 461 women aged 40-87 years (age 57.17 ± 0.71 years) were exa mined, among them 71 patients with rheumatoid arthritis and 390 controls. We conducted the clinical and laboratory examination (erythrocyte sedimentation rate and C-reactive protein level in serum). Pain intensity was evaluated by the visual analogue scale, the quality of life-by the HAQ questionnaire. Lean mass, bone mineral density were measured by the X-ray absorptiometry (Prodigy, GEHC Lunar, Madison, WI, USA). Pre-sarcopenia (first stage of sarcopenia) was determined when an appendicular lean mass index was less than 5.72 kg/m 2 (V.V. Povoroznyuk, N.I. Dzerovich, 2016). Statistical analysis was conducted using the "Statistica 6.0" software. Results. Patients with rheumatoid arthritis had a significantly lower femoral neck mineral density (p = 0.002), lean mass of the total body (p = 0.01) and appendicular lean mass (p < 0.01). The frequency of pre-sarcopenia in women with rheumatoid arthritis was 49 %, in the control group-18 %. Conclusions. Patients with rheumatoid arthritis had not only bone tissue, but also skeletal muscle tissue disorders, resulting in a significant deterioration of functional capacity and quality of life. Given the significant medical and social significance of the problem, further studies into the mechanisms of pathogenesis, development of diagnostic methods, prevention and treatment of sarcopenia in patients with rheumatoid arthritis are required.

We didn’t find any significant connection among the activity parameters (C-reactive protein, ESR, pain VAS, DAS-28), duration of the disease, life quality and lean mass indices in patients with rheumatoid arthritis. However, a significant correlation was found between the number of swollen joints and lean mass of upper limbs (r=0.67; р=0.02).The frequency of pre-sarcopenia in women with rheumatoid arthritis was 49 %, in the control group – 18 %.Conclusion:Patients with rheumatoid arthritis had not only bone tissue, but also skeletal muscle tissue disorders, resulting in a significant deterioration of functional capacity and quality of life.
