Einleitung Viele Schilddrüsenmetastasen sind klinisch inapparent und wurden bislang häufig nur als Zufallsbefund, z. B. im Rahmen von Autopsiestudien, beobachtet. ▸ Dennoch sind Schilddrüsenmetastasen insgesamt nicht so selten, wie es oft beschrieben wird. Dies ist nicht verwunderlich, wenn man bedenkt, dass die Schilddrüse das am zweitstärksten arterialisierte Organ des Menschen ist. Interessanterweise berichten verschiedene Institutionen über eine Zunahme der Fälle im Verlauf der letzen Jahre, was am ehesten mit einer Verbesserung diagnostischer Maßnahmen sowie der Patientenselektion in Zusammenhang steht. Die häufigsten Tumoren, die in die Schilddrüse metastasieren, sind in der Box "Überblick" (S. 340) aufgelistet (s. a. Tab. 1). Die Intervalle, bis Metastasen auftreten, schwanken je nach Primarius erheblich und können bis zu 11 Jahre betragen. Tumoren, die in endokrine Organe metastasieren, stellen eine besondere Herausforderung dar, da sie sowohl von malignen Primärtumoren dieser Organe wie auch von benignen Prozessen klar abgegrenzt werden müssen, um ein sinnvolles onkologisch-chirurgisches Therapiekonzept erstellen zu können. Dieser Beitrag beschäftigt sich mit der Epidemiologie, dem diagnostischen und dem therapeutischen Vorgehen, speziell der chirurgischen Therapie von Metastasen anderer Tumoren in die Schilddrüse und die Nebenniere.
