BACKGROUND:The number of pharmacies, which produce drug formulations locally, has recently considerably reduced in Russia. Pharmacies mainly operate as retailers of industrially manufactured drugs.Pharmaceutical consultation of customers at pharmacies aimed at responsible self-medication is the most popular and accessible feature of pharmaceutical care. In Russia there is a signifi cant list of medicines approved for sale in pharmacies on a non-prescription basis that is specifi ed in the product label. In this regard, the role of pharmacists in public health in Russia increases. Pharmacist, working directly with population, is an important fi gure for the rational use of medicines. This type of work requires high level of professional training and appropriate ethics.
Cure of allergy symptoms reported by a patient with the help of non-prescription antiallergic drugs, is one of the important aspects of a responsible self-medication.The aim of the study is to assess the peculiarities of consumers’ behavior as pharmacy visitors, when choosing non-prescription antiallergic drugs, and to identify potential problems of pharmaceutical counseling for allergy symptoms regarding the responsible self-medication.Materials and methods. Sociological survey in the form of a questionnaire; graphoanalytical and comparative analyses.Results. The profile of an over-the-counter antiallergic drugs buyer in the pharmacies of Kazan was assessed in the following way: it is a woman aged 18–44 of a middle level of income, having a family of 3–4 people, ready to spend from 101 to 500 rubles on the purchase of antiallergic drugs, buying anti-allergic drugs not for the first time. The main reasons to seek for treatment were skin rash, redness and itching, which had also been observed in the past. The allergic nature of the disease had already been confirmed by the doctor. For the average consumer of over-the-counter antiallergic drugs, the most important criteria for choosing a medicine were: efficiency, safety, the doctor’s recommendations and price. The medicines were purchased for the visitors themselves or their children. The customers were satisfied with the choice of non-prescription antiallergic medicines available in the pharmacy. The visitors generally trusted the pharmacists’ advice and recognized them as health professionals, but considered that the main goal was not only to provide a pharmaceutical care, but also to profit from the sale of the drugs. The following flaws of pharmaceutical counseling when dispensing non-prescription antiallergic drugs, were identified: an improper diagnosis by a pharmaceutical specialist, the lack of recommendations to consult a doctor, incomplete provision of information on the use, storage, and the possibility of interaction with other drugs and food.Conclusion. Misdiagnosis and incomplete provision of information by pharmaceutical specialists on antiallergic the drugs that are approved for the over-the-counter dispensing, require the implementation of a pharmaceutical consultation algorithm for the visitors contacting a pharmacy with allergy symptoms.
Реферат. Цель исследования-выявить информационные потребности фармацевтических работников для эффективного консультирования пожилых пациентов. Материал и методы. Проведено анкетирование работников «первого стола» аптечных организаций-провизоров и фармацевтов, обучающихся в системе дополнительного профессионального образования г. Казани. Специалистам была предложена авторская анкета «Особенности консультирования пожилых пациентов». Результаты и их обсуждение. 81,6% аптечных работников указали, что половину и более посетителей аптеки составляют пожилые пациенты. Их консультируют по вопросам применения рецептурных и безрецептурных лекарственных средств и других групп аптечных товаров, но сами фармацевтические работники не знают важных признаков заболеваний, требующих немедленного обращения к врачу, причем самооценка их уровня знаний неадекватна. Вызывает тревогу, что ни один фармацевтический работник не назвал полностью все особенности фармакокинетики пожилых пациентов, в то же время только 69,44% провизоров и 56,60% фармацевтов считают необходимым повышение уровня знаний по консультированию гериатрических пациентов. Установлены наиболее частые причины обращения в аптеку пожилых пациентов. Выявлена негативная тенденция: фармацевтические работники отпускают без рецепта рецептурные лекарственные средства и рекомендуют рецептурные лекарственные средства, об этом указали в анкете 9,43% фармацевтов и 19,44% провизоров. По-видимому, это связано с недостаточным контролем уровня знаний по вопросу фармацевтического консультирования гериатрических пациентов при обучении в системе дополнительного профессионального образования, непрерывного медицинского и фармацевтического образования и при сдаче сертификационного экзамена. Необходима разработка учебных материалов по консультированию пожилых пациентов с учетом особенностей фармакокинетики и фармакодинамики в пожилом возрасте. Выводы. Результаты проведенного анкетирования показали, что фармацевтические работники недостаточно подготовлены к консультированию гериатрических пациентов-посетителей аптек. Установлена необходимость разработки алгоритмов фармацевтического консультирования гериатрических пациентов и обучающих программ для системы дополнительного профессионального образования об особенностях фармакотерапии пожилых.
