Цель исследования -проанализировать результаты хирургического лечения острого билиарного панкреатита в соответствии с разработанным лечебно-диагностическим алгоритмом. Материал и методы: проанализированы результаты лечения 117 больных острым билиарным панкреатитом в возрасте от 20 до 78 лет. У 70% больных острым билиарным панкреатитом обнаружены отечная форма и мелкоочаговый панкреонекроз, у половины обследованных пациентов имели место деструктивные формы острого калькулезного холецистита. Результаты: разработан лечебнодиагностический алгоритм, включающий оригинальные запатентованные методики, оптимизирующий лечебную тактику при остром билиарном панкреатите и основанный на приоритетной активной хирургической тактике с применением миниинвазивных хирургических технологий лечения.Ключевые слова: острый билиарный панкреатит, хирургическое лечение. The urgency of acute biliary pancreatitis is due to the increasing number of patients suffering from cholelithiasis, as well as the lack of common surgeons' views on the diagnostic and therapeutic algorithms. The aim is to analyze the results of surgical treatment of acute biliary pancreatitis in accordance with the developed medical diagnostic algorithm. Material and methods: the results of treatment of 117 patients with acute biliary pancreatitis at the age of 20 to 78 years are analyzed. In 70% of patients with acute biliary pancreatitis, edematous form and small-focal pancreonecrosis were found, and in half of the patients examined had destructive forms of acute calculous cholecystitis. Results: a medical diagnostic algorithm has been developed that includes original patented techniques optimizing therapeutic tactics in acute biliary pancreatitis and being based on the priority active surgical tactics with the use of minimally invasive surgical treatment technologies.
Кафедра хирургических болезней №2 ГБОУ ВПО «Курский государственный медицинский университет» Минздрава России, Курск, Россия Цель работы-улучшение результатов лечения холедохолитиаза и стеноза большого дуоденального сосочка двенадцатиперстной кишки (БСДК) путем применения антеградной моно-и биполярной папиллосфинктеротомии (АПТ). Материал и методы. Изучены результаты диагностики и лечения 256 больных с осложненной желчнокаменной болезнью. У 132 больных разрешение холедохолитиаза проводилось с использованием эндоскопической папиллосфинктеротомии до или после лапароскопической холецистэктомии (ЛХЭ). У 124 пациентов во время ЛХЭ выполнялась антеградная (моно-или биполярная) папиллосфинктеротомия. Результаты. Антеградная моно-и биполярная папиллотомия выполнима в 99,25% случаев, позволяет малотравматично разрешить выявленную доброкачественную патологию внепеченочных желчных протоков и БСДК одноэтапно во время лапароскопической холецистэктомии и осуществима даже при стриктурах, интрадивертикулярном расположении БСДК, остром панкреатите и минимизирует возможность развития таких осложнений, как кровотечения, перфорации, панкреатита (р<0,05). Вывод. Антеградная моно-и биполярная папиллотомия выполнима практически в 100% случаев, позволяет малотравматично и безопасно разрешить выявленную доброкачественную патологию внепеченочных желчных протоков и большого сосочка двенадцатиперстной кишки сразу во время ЛХЭ, избежав при этом многоэтапности и неоправданного перехода на открытую операцию (конверсию, которая по данным литературы, колеблется от 5 до 10%). Она осуществима даже в тех случаях, когда невыполнима или противопоказана эндоскопическая папиллотомия: стриктура, интрадивертикулярное расположение большого сосочка двенадцатиперстной кишки, острый панкреатит.
