La sepsis es una patología frecuente en las unidades de cuidados intensivos neonatales, por lo tanto, se requiere contar con marcadores predictores tempranos, no invasivos y de bajo costo para hacer el diagnóstico. Se pretendió validar la variabilidad de la frecuencia cardíaca (VFC) en recién nacidos (RN) con factores de riesgo de sepsis temprana, para su aplicación como método diagnóstico. Esta es una investigación descriptiva observacional individual transversal, en 100 neonatos a término y pretérminos tardíos, con riesgo de sepsis temprana y con otras patologías no infecciosas; se les monitoreó la VFC con el equipo Polar V800, en las primeras 24 horas de vida. Se utilizaron medidas estadísticas descriptivas, Mann-Whitney, Fisher, sensibilidad, especificidad, valor predictivo positivo (VPP), valor predictivo negativo (VPN), razón de verosimilitud (RV) y curvas ROC (Característica Operativa del Receptor). El único parámetro estadísticamente significativo con una p 0,043, que diferenció a los RN con y sin riesgo de sepsis, fue el promedio de la frecuencia cardíaca (FC). El único parámetro de la VFC que tuvo mayor sensibilidad (64 %) y especificidad (37 %) fue el número de intervalos R-R (m R-R), ésta es inversamente proporcional al promedio de la FC. La VFC, por sí sola, no es un dato consistente para el diagnóstico de sepsis temprana en RN con ruptura prematura de membranas (RPM) mayor a 18 horas, debido a su baja sensibilidad y especificidad; es decir debe ser valorada conjuntamente con los antecedentes, cuadro clínico y exámenes complementarios de laboratorio.
