Objectives: Many authors’ have advocated a minimum of two implants to support a mandibular overdenture, but financial restraints specifically among the geriatric population in developing countries made this treatment plan economically difficult. Hence, this study was planned to assess the symphyseal (midline) single implant-assisted complete overdenture for patient satisfaction and masticatory performance. Materials and Methods: In this clinical study, 12 edentulous first-time denture wearers underwent placement of a single implant in the mandibular symphyseal region. After 1 week, new complete dentures were fabricated and delivered to the patients. Post 3 months, the denture was fixed with a nylon cap-ball attachment to the anchor implant. Patients were questioned about comparison in the level of satisfaction and complaint before loading the implant (control group) and after 1 week, 1 month, and 3 months. The implant-assisted overdenture was fabricated with the help of a questionnaire. Masticatory performance was calculated with the help of a bite force measuring device at the same time intervals. SPSS 17.0 statistical software was used to analyze the data. Results: It was found that single implant anchorage of the mandibular complete denture resulted in a significant increase ( P < 0.05) in patient's subjective satisfaction and a decrease ( P < 0.05) in complaints at the end of 3 months. There was a significant ( P < 0.01) increase in bite force in implant overdenture after 3 months (5.459 kgf) as compared to that of the complete denture (3.406 kgf). Conclusion: Single implant-assisted overdenture can be an appropriate treatment modality to treat edentulousness in the geriatric population. It insinuates the remarkable improvement of prosthesis function and oral comfort with minor surgical procedures.
Delayed sustained release formulation is to retard release of the drug form the stomach and release in the intestinal pH. Captopril is recommended as first choice of drug. Attacks of the myocardial infarction are generally in the early morning or night. Captopril is showing less bioavailability in presence of food. With enteric coating problem should be solved out. It also minimizes the gastric irritation of the captopril. Captopril is widely used for the arterial hypertension. It also used for myocardial infarction and in diabetic nephropathy. The drug dose is taken three times daily, which may give poor patients compliance. In the present investigation, an attempt was made to decrease dosing frequency by prepare a mucoadhesive tablets using various polymers such as HPMC K4M, bael gum and chitosan. Eudragit RL100 used as enteric coating polymer. Formulations friability, drug content, surface pH, wash off test, mucoadhesive strength and dissolution study. The results of friability tests carried out for all the formulations are within the official limit and acceptable. According to wash off test the formulation containing bael gum showing better results. All the formulation was showing better swelling property. F4 (formulation containing bael gum) was showing best mucoadhesive strength among all the formulations. In in-vitro drug release study formulation (F4) containing bael gum showing better control release among all the formulations i.e. 86.488 % in 12 hrs.
Background: This study analyzed the clinical outcomes in the obstetric patients with COVID-19 and their neonates in first wave of COVID-19 pandemic in North India, 2020.Methods: This prospective study was conducted between 10 May 2020 to 31 December 2020 on 83 obstetric patients with COVID-19 and their 52 neonates.Results: 36.14% obstetric patients presented with COVID-19 like symptoms with most common symptom as fever in 60% and cough in 53.33%. 4.81% patients were admitted in HDU and 1/83 (1.20%) patient who required ICU had mortality due post-operative complications. 20.48% had pre-existing medical diseases. Amongst (78) antenatal patients, 21.79% had pregnancy related hypertensive disorders, 12.82% had deranged liver function tests without hypertension and 8.97% had gestational diabetes mellitus. More probability of pre-term labour pains 2.4 (95% CI, 1.37-4.18) and IUFD 2.18 (1.13-4.20) were observed in symptomatic patients as compared to asymptomatic patients. Neonates born to COVID-19 symptomatic mothers had 1.81 (95% CI, 0.73-4.49) times the risk of being symptomatic, 1.37 (95% CI, 0.54-3.41) times the risk of getting admitted to NICU and 1.57 (95% CI, 0.48-5.09) times the risk of getting infected by SARS-CoV-2 and increased morbidity in neonates. 8% neonates had perinatal and 1.5% had horizontal transmission of SARS-CoV-2.Conclusions: First wave of COVID-19 pandemic did not cause significant adverse outcome in pregnant patients and mother-newborn dyads in our tertiary care centre when active and intensive management of mothers and newborns were done but still there is possibility of severe morbidity and mortality due to COVID-19.
