Цель. Оценить прогностическое значение клиникогематологических показателей, используемых в повседневной работе врача-гематолога для разделения больных диффузной В-крупноклеточной лимфомой (ДВКЛ) на группы риска, и обосновать необходимость поиска новых факторов прогноза. Методы. В исследование включен 101 пациент с впервые диагностированной ДВКЛ в возрасте 18-80 лет (медиана 58 лет). Женщин было 53, мужчин-48. В качестве терапии первой линии пациенты получали программу R-CHOP. В зависимости от ответа на терапию все больные разделены на 4 группы: с полным ответом (ПО; n = 58), частичным ответом (ЧО; n = 15), резистентностью к первой линии терапии (n = 19), ранними рецидивами (РР; n = 9). Медиана наблюдения за больными составила 22 мес. (диапазон 2-120 мес.). Результаты. При оценке влияния возраста на эффективность терапии по схеме R-СНОР в первой линии значимой разницы в частоте ответов у пациентов моложе и старше 65 лет не установлено. Выявлены статистически значимые различия при анализе двух параметров международного прогностического индекса (IPI; стадия заболевания, экстранодальные поражения) и В-симптомов в группах с ПО и резистентным течением заболевания. При этом статистически значимой разницы по этим же показателям в группах с ПО и РР не обнаружено. Медиана 2-летней безрецидивной выживаемости у больных с ПО не была достигнута. У пациентов с ЧО она составила 12 мес. Медиана 2-летней общей выживаемости у больных с ПО, ЧО и РР не была достигнута, а у пациентов с резистентным течением ДВКЛ она составила 10 мес. Заключение. Результаты проведенного исследования подтверждают прогностическую значимость факторов, используемых для разделения больных ДВКЛ на группы риска. Вместе с тем вариабельность клинического течения заболевания, особенно при низком показателе IPI, свидетельствует о необходимости поиска новых прогностических параметров, связанных с течением ДВКЛ.
Цель. Разработать комплексную модель прогнозирования течения диффузной В-крупноклеточной лимфомы (ДВКЛ) с использованием иммуногистохимического подтипа опухоли и параметров международного прогностического индекса (IPI). Материалы и методы. Из 104 больных ДВКЛ в базе данных критериям включения соответствовал 81 (77,9 %). Медиана возраста составила 58 лет (диапазон 23-83 года). Все больные получали лечение по схеме R-СНОР. Для создания прогностической модели общей выживаемости (ОВ) больных ДВКЛ использовали метод машинного обучения-деревья классификации и регрессии (CART). Анализ ОВ проводился по методу Каплана-Мейера. Для сравнения кривых выживаемости применяли лог-ранговый критерий и отношение рисков (ОР). Статистической значимостью любого теста считался полученный двусторонний уровень p < 0,05. Результаты. Согласно построенной модели, выделены три группы пациентов: 1-я-группа низкого риска (сочетание низкого, промежуточного низкого и промежуточного высокого риска по IPI и GCB-подтипа); 2-ягруппа промежуточного риска (сочетание низкого, промежуточного низкого и промежуточного высокого риска по IPI и non-GCB-подтипа); 3-я-группа высокого риска (независимо от подтипа). В группе низкого риска (n = 26) 2-летняя ОВ за исследуемый период составила 100 %. В группе промежуточного риска (n = 34) медиана ОВ не достигнута, 2-летняя ОВ составила 74 %, ожидаемая 5-летняя ОВ-68 %. В группе высокого риска (n = 21) медиана ОВ была 25 мес., 2-летняя ОВ-46 %, ожидаемая 5-летняя ОВ-37 % (p < 0,0001, лог-ранговый критерий). ОР, рассчитанное для группы высокого риска по сравнению с группами низкого и промежуточного, составило 5,1 (95%-й доверительный интервал 2,1-12,1; p = 0,0003).
