Presentamos el caso de una paciente de 66 años con antecedentes de enfermedad renal crónica desde hace 8 años, en terapia con diálisis peritoneal, que acude al servicio de emergencia por presentar deterioro del estado de conciencia asociado a convulsiones, y emergencia hipertensiva, posterior al tratamiento cede la sintomatología y se observa resolución de lesiones halladas en la resonancia magnética después de un mes de evolución, catalogado como síndrome de encefalopatía posterior reversible. Considerando este diagnóstico en pacientes con lesión renal crónica asociado a hipertensión arterial.
