This paper presents the results of an ethnographic study of an obstetric center within a hospital of Greater Buenos Aires and an analysis of the spatio-temporal dimension of hospital care. The fieldwork, carried out between 2007 and 2011, followed the everyday dynamics of the hospital and included observation in the obstetrics unit (waiting areas, the obstetric center where births and emergencies receive care, and the ambulatory care, ultrasound, and hospitalization rooms, among others) as well as spaces such as the appointments and statistics offices, the office of social services, the central hall, the pharmacy, and hallway waiting areas. Interviews were carried out with department heads, obstetricians, nurses, social workers, staff of the laboratory and the appointment and statistics offices, volunteers and patients. In this way, the article analyzes the medical-bureaucratic routines in the admission and hospitalization of women in the obstetrics center; the disputes, transactions and negotiations occurring among professionals, patients and families; the delimitation of spaces; and the temporal sequences and hierarchies involved in the passage from the "outside" to the "inside" of the hospital.
Este artículo recupera la experiencia de personas con VIH residentes del Área Metropolitana de Buenos Aires, en torno del «tratamiento como prevención», núcleo central de las políticas globales contemporáneas de gobierno del VIH-Sida. Se trata de una investigación etnográfica cuyo trabajo de campo (veinticuatro meses con una frecuencia de dos a tres visitas semanales de ocho a doce horas de duración cada una) se desarrolló entre 2016 y 2018. El corpus empírico consiste en trece entrevistas en profundidad y un extenso conjunto de registros de observación de interacciones cotidianas. Analizamos los modos en que indetectabilidad e intransmisibilidad del virus se entrelazan en la vida con la enfermedad. Mostramos que mientras las políticas presentan la relación indetectable-intransmisible como la clave para poner fin a la epidemia y a la vez como una poderosa herramienta para responder al estigma que rodea al VIH, estas relaciones se complejizan en la vida cotidiana de los afectados, dando lugar a procesos tensos y contradictorios entre percepción-interpretación-significación y prácticas. Entre estos procesos se destaca lo que llamamos una redirección del sentido del riesgo de enfermedad: desde la probabilidad de transmitir el virus a otros y otras hacia los otros y otras como fuente de trasmisión de nuevas condiciones de enfermedad.
A partir de 2018, a fórmula "Indetectável=Intransmissível" (I=I, indicando uma pessoa com carga viral indetectável tinha zero chances de transmitir sexualmente o virus HIV) foi levantada pelo UNAIDS e outras organizações internacionais como o "caminho para acabar com a epidemia". Acompanhados pelo desenvolvimento de tratamentos anti-retrovirais altamente eficazes, que implicam a redução de comprimidos e das doses diárias, os discursos "Pós-AIDS" de crescente otimismo têm levantado a "indetectabilidade-intransmissibilidade" como uma resposta abrangente, simples e eficaz tanto para o controle individual do vírus bem como para o seu governo ao nível comunitário. Este artigo pretende observar a “indetectabilidade viral” a partir das diferentes formas como ela se configura e se produz nos diferentes espaços, relações e tempos vitais em que se põe em jogo. A partir de um estudo etnográfico realizado em um Programa Municipal de HIV da Província de Buenos Aires, resgatamos as narrativas de duas pessoas em tratamento de longa duração para mergulhar na produção cotidiana de “ser”, “ficar” e “manter” indetectável. Consideramos, nesse quadro, que o devir-indetectável implica uma coexistência entre controle e descontrole que se desenvolve em um processo com-outros, no qual os sujeitos realizam buscas para encontrar respostas para seus sofrimentos. Esses processos implicam a formação tanto de vínculos frágeis, momentâneos e incertos quanto de relações com outros que constituem um resseguro para a manutenção dos tratamentos.
