Aims and objective. To verify association between PU development with nursing workload and illness severity and to verify whether nursing workload and illness severity are related with Braden Scale scores. Background. Critically ill patients are more susceptible to treatment complications because of the severity of their clinical condition. Design. Prospective descriptive study. Methods. Patients consecutively admitted to three intensive care units (ICUs) of a public university hospital located in Sao Paulo, Brazil and without pressure ulcer (PU) at admission and a minimum stay of 24 hours were included in the sample. Prospective data collection included demographic, clinical and hospitalisation data, Nursing Activities Score (NAS), Simplified Acute Physiology Score (SAPSII) and Braden Scale. Multivariate linear regression analysis was applied to verify whether nursing workload and illness severity are related with Braden Scale scores. Multivariate logistic regression analysis was used to verify whether nursing workload and illness severity were risk factors associated with PU development. Results. The study sample included 160 patients. The mean Braden score was 12·0 and PU incidence was 34·4%. Multivariate linear regression analysis identified as factors related to variation of Braden scores: illness severity (SAPSII), nursing workload (NAS) and age. Multivariate logistic regression showed a model with risk factors associated with PU development: sex, length of ICU stay, illness severity and nursing workload. Conclusion. Nursing workload, severity of illness, sex and length of ICU stay were identified as risk factors associated with PU development. However, nursing workload acted as a protective factor. Illness severity, nursing workload and age were related to Braden scores. Relevance to clinical practice. Accurate identification of risk factors and the use of clinical judgment in skin assessment are prerequisites for determining appropriate strategies to prevent pressure ulcers, to improve quality of care for patient safety and to reduce length of ICU and hospital stay and costs.
OBJETIVOS: Avaliar a qualidade de vida (QV) dos técnicos e auxiliares de enfermagem que trabalham em Unidades de Terapia Intensiva e identificar os fatores sociodemográficos e relacionados ao trabalho que podem influenciar a QV. MÉTODOS: Estudo transversal realizado em um hospital escola da cidade de São Paulo. A coleta de dados de 126 trabalhadores de enfermagem em nove Unidades de Terapia Intensiva foi realizada em outubro e novembro de 2005. Utilizou-se como instrumento de avaliação de QV o WHOQOL-BREF. RESULTADOS: As médias em cada domínio da QV foram: Relações Sociais 66,3; Psicológico 60,8; Físico 53,1 e Meio-Ambiente 49,4. A variável idade apresentou correlação positiva somente com o domínio físico e o número de empregos se correlacionou inversamente com os domínios Físico, Psicológico e Relações Sociais. CONCLUSÃO: A qualidade de vida dos trabalhadores de enfermagem em todas as dimensões é relativamente baixa, considerando-se os valores entre 0-100. Há fraca correlação dos domínios com idade e número de empregos.
Os resultados mostraram elevada incidência de delirium na UTI e sua ocorrência associada às idades mais avançadas e o uso de sedativos e analgésicos, ressaltando-se a importância da atuação do enfermeiro na prevenção e identificação precoce do quadro nos pacientes com essas características.
RESUMOTrata-se de estudo descritivo, cujos objetivos foram comparar a carga de trabalho de enfermagem em unidade de pós-operató-rio de cirurgia cardíaca indicada pelo NAS, TISS-28 e NEMS, e também verificar a proporção profissional de enfermagem por paciente existente na unidade e a proporção necessária, segundo os índices utilizados. Os dados foram coletados em um hospital-escola de outubro a novembro de 2004. A amostra, constituída de 55 pacientes, totalizou 283 medidas de carga de trabalho. A carga de trabalho mensurada pelo NAS (73,7%) foi estatisticamente superior ao do TISS-28 (62,2%) e ao do NEMS (59,7%). A proporção média de profissionais de enfermagem por paciente, estimada pelo NAS (1,0:1), TISS-28 (0,8:1) e NEMS (0,8:1) foi inferior ao observado na unidade (1,2:1). Concluiu-se que o NAS quantificou maior carga de trabalho de enfermagem e apresentou uma relação profissional de enfermagem por paciente mais próxima ao observado na unidade estudada. ABSTRACT This is a descriptive study, aiming at comparing the nursing workload in a postoperatory heart surgery unit (UPOCC) using NAS, TISS-28 and NEMS, and verifying both the observed and recommended nursing staff-to-patient ratio according to the workload indexes used. Data collection happened in a University Hospital, from October to November, 2004. The sample included 55 patients, resulting in 283 nursing workload measurements. The nursing workload measured by NAS (73.7%) were statistically higher than TISS-28 (66.2%) and NEMS (59.7%). The average of nursing staffto-patient ratio estimated by NAS (1:1), TISS-28 (0.8:1) and NEMS (0.8:1) were lower than the observed ratio in the unit (1.2:1). We concluded that NAS measured a higher nursing workload and showed a nursing staff-to-patient ratio similar to the results found in the unit studied.
