The supracondylar humerus fracture is the most common elbow fracture in children, corresponding to 58% of these cases. The objective of this study is to report a rare postoperative complication, the pseudoaneurysm of the brachial artery. A 9-year-old girl with was admitted with a Gartland III supracondylar fracture of the right humerus, presenting normal neurological and vascular exams. The patient underwent a surgical treatment with percutaneous fixation and immobilization and was re-evaluated after 2 and 4 weeks. In the last evaluation the immobilization and fixation were removed. She returned after 3 months, presenting a progressive palpable, painless mass in the 1/3 distal right arm. The arterial ultrasound showed a mass in the cubital fossa, which presented internal flow and some wall thrombi, compatible with pseudoaneurysm of the brachial artery. After diagnosis, the treatment was a vascular surgery for dissection and reconstruction of the artery. The incidence of vascular involvement in Gartland III fractures ranges from 2% to 38%. False aneurysms originate from arterial hematomas caused by trauma to the arterial lumen, and their developing time can vary. There are few reports of this complication, so there is no consensus about the treatment.
Objetivo: Elucidar os principais mecanismos de trauma ortopédico com fraturas em pacientes de 0 a 12 anos atendidas no pronto socorro pediátrico em um hospital do oeste do Paraná. Método: Estudo quantitativo epidemiológico, observacional; descritivo; retrospectivo a partir da coleta e análise de dados dos prontuários (352) de atendimentos no pronto socorro pediátrico entre janeiro de 2019 a dezembro de 2020. Foram excluídos da análise os pacientes que não obtiveram o diagnóstico de fratura (193). Resultados: Da amostragem de 159 prontuários analisados, o sexo biológico prevalente foi o masculino (52,5%). Acerca da idade, a faixa etária de maior relevância constitui-se no intervalo de 2 – 5 anos (49,3%), sendo o pico etário de 5 anos (15,8%). Quanto a classificação das fraturas, 71,4% mostraram-se completas e 82,3% geradas por trauma direto, sendo os membros superiores os mais acometidos (84,8%), predominantemente no hemicorpo esquerdo (55,7%). O osso mais atingido foi o rádio (39,2%) e o mecanismo de trauma mais frequente a queda de mesmo nível (48,5%), sendo a queda de bicicleta de maior relevância (10,1%). Secundariamente, a queda de plano elevado mostrou-se significativa, caracterizando 32,2% dos casos, das quais a queda de cama foi a prevalente, retratando 23,1%. Ademais, no que se refere a escolha do método terapêutico, o tratamento conversador foi prevalente em 75% das ocorrências. Conclusão: Estudos como o presente, apresentam informações importantes para medidas preventivas às crianças, através de políticas de promoção à saúde e prevenção de acidentes que provoquem danos ou sequelas nesta população.
Introdução: A separação traumática intraparto da epífise distal do úmero é uma fratura extremamente rara. Essa lesão resulta de forças de cisalhamento rotacional empregadas ao úmero distal que resulta em fratura tipo I de Salter-Harris por rotação vigorosa do antebraço com o cotovelo fletido ou hiperextensão do cotovelo. Objetivo: Relatar um caso de fratura transfisária distal do úmero em recém-nascido do sexo masculino. Descrição do caso: Recém-nascido, masculino, gemelar, prematuro tardio, nascido de parto cirúrgico, em apresentação pélvica, diagnosticado no quinto dia de vida com deslocamento epifisário póstero-medial do úmero distal direito (Salter-Harris tipo I) e submetido a redução incruenta com fixação percutânea com fio de Kirschner. Conclusão: A fratura Salter-Harris tipo I em recém-nascido possui elevado potencial de remodelamento ósseo e rápida recuperação quando diagnosticada precocemente e manejada de forma adequada ainda que seja uma lesão rara, de difícil diagnóstico, de interpretação radiológica complexa e de sinais inespecíficos.
