ISSN 0803-9488.The purpose of this study was to investigate whether depressed patients not responding to tricyclic antidepressants (TCA) would respond on addition of 3060 mg mianserin. Thirtyseven in-and out-patients still fulfilling DSM-III criteria for a major depressive episode after at least 4 weeks of treatment with 150 mg-TCA were randomly allocated to two groups receiving in addition either mianserin or placebo. The study was conducted double-blind and lasted for 3 weeks. There was a significantly better improvement in the mianserin-TCA group than in the placebo-TCA group after 3 weeks of treatment as measured on the Montgomery-Asberg's Depression Scale and the Global Assessment Scale. There were no differences in side effects between the two groups. Forfatterne har i mange år interessert seg for forbindelsen mellom medisin og humaniora, bl.a. med prosjektet Infectio ved Universitetet i Oslo i årene 2004 -08. Boken er en direkte videreføring av dette interessante prosjektet som ble støttet av Norges forskningsråd.Gjennom lesning av vestlig skjønnlitteratur ønsker forfatterne å problematisere hvordan vi lever i spennet mellom den allmenne kunnskap om sykdom og den spesifikke erfaringen med den. Boken er et godt eksempel på en kulturkritikk som evner å kombinere ulike metoder, perspektiver og kunnskapsområder: idéhistoriske, medisinske, litteraere og historiske.Det burde vaere av interesse for medisinere å kjenne noe til sitt fags kulturhistorie og hvordan sykdom hører med til skjønnlitteraturens store temaer. «Jeg... formoder at leger må forestille seg en fiktiv verden, i tillegg til en reell, hvis de skal utøve sitt yrke», hevder George Rousseau (1). All kunnskap om faget kan ikke rommes av faget alene. Selv om medisinens anliggende er sykdommen og avviket, innebaer ikke dette at den interesserer seg for hvordan det er å ha sykdommen eller å vaere avvikeren. Her har fagmedisinen muligheten til å inkorporere skjønnlitteraturen, som gjennom sin kompleksitet og sitt «prinsipielle avvik fra dagligspråket» er spesielt egnet til å fremstille individets sykdomserfaring og sykdommens plass i kulturen og samfunnet. På denne måten erklaerer forfatterne sitt ambisiøse, men ikke desto mindre relevante og interessante prosjekt: «[å] ... kanskje bidra til en utvidelse av det vi i generelle termer kan kalle en allmennmedisinsk forståelse av sykdom.» I hvert av de 13 kapitlene betrakter forfatterne sykdommen med bredt perspektiv. Det dreier seg om «fundamentalsykdommene» pest, lepra, syfilis, tuberkulose og hysteri, for de fleste av dagens vestlige medisinere medisinske «klenodier». Men også influensa, aids, epilepsi, kreft, hjertesykdom, anoreksi, depresjon og selvmord er med.Forfatterne har dessuten innhentet medisinsk fagkunnskap som presenteres integrert i hovedteksten. Derved unngår man en oppstykkende, leksikal stil som ellers kunne ha virket kjedelig. Medisinen fremstår som en egen diskurs, men like fullt som en del av vår kultur. Dette gjelder for eksempel kapitlet om hysteri, hvor forfatteren trekker lange historiske linjer fra tidlig moderne medisin på 1800-tallet til våre dagers fibromyalgi, kronisk tretthetssyndrom og AD/HD. Kapitlene om kreft og depresjon, blant vår tids mest utbredte tilstander, viser ved lesning av Zorn, Gustafsson og Duras hvordan lidelsen vender seg til en slags mening, ikke som en karriere, men ved å stå som en fyllestgjørende kontrast til det forflatende, jagende livet man lever som «frisk». Her blir sykdommen forstått og opplevd gjennom det litteraere språket -den gis en annen retning, bort fra den vanlige, og utfordrer medisinen på dens hippokratiske ed: Hvordan forholder vi oss til at «å få kreft er det meste intelligente jeg noen sinne har gjort» (Zorn), eller avvisning av behandling fordi kreftsmerten «... er likevel min smerte og ing...
