Los carcinomas de cabeza y cuello (CCC) son tumores con elevada morbilidad y mortalidad, que causan un deterioro significativo de la calidad de vida en los pacientes con adversas repercusiones familiares y sociales. La mayoría de los pacientes consultan en estadios localmente avanzados y según la localización y compromiso, el pronóstico de vida sin tratamiento puede llegar a ser desalentador. Los tratamientos no están libres de consecuencias adversas. Algunos pueden ser tan mutilantes y discapacitantes como la misma enfermedad. El abordaje siempre debe ser integral y multidisciplinario con inmensa claridad de la connotación humana del paciente y sus familiares. Se deben ofrecer diferentes opciones de manejo, que buscarán la conservación o recuperación en la mejor medida posible de la calidad de vida. Debe evitarse la iatrogenia y los tratamientosexcesivos por el impacto adverso. Dilucidarse el riesgo al suicidio, la expectativa de vida y la oportunidad real de mejoría con la intervención propuesta. Rev.cienc.biomed. 2015;6(1):179-182
El adenoma pleomorfo de septum nasal es una patología poco usual, que se manifiesta generalmente con obstrucción nasal, sensación de masa y epistaxis. El estándar de manejo es quirúrgico, al igual que los adenomas pleomorfos en otras localizaciones.La recurrencia es similar a la presentada en glándulas salivales mayores, sin embargo la tasa de malignización tiende a ser mayor. Se presenta el caso de una mujer de 28 años quien presentaba obstrucción nasal izquierda durante un año, a la rinoscopia anterior el tumor se asemejaba a un espolón septal, se realizó resección quirúrgica transnasal con márgenes negativos, y diagnostico histológico confirmatorio de adenoma pleomorfo.Es importante tener en cuenta que en cavidad nasal existen lesiones que pueden enmascararse como variantes anatómicas. En el caso presentado, un hallazgo tan común como el espolón septal ocultaba un adenoma pleomorfo.
Los paragangliomas son tumores infrecuentes, que tienen origen en las células de la cresta neural. Su presentación en el nervio vago representa tan solo el 5% de estas neoplasias en cabeza y cuello. Las manifestaciones clínicas más frecuentes son la presencia de masa indolora en cuello, disfonía y plenitud faríngea, seguidas por la parálisis del X y XII par craneal. La valoración imagenologíca inicial se realiza con tomografía computarizada contrastada del cuello, la cual se puede complementar con estudios de resonancia magnética y angiografía. El manejo quirúrgico de estos tumores es ampliamente aceptado, y existen controversias en cuanto a la embolización prequirúrgica. Se revisa la literatura y se presenta el caso de una paciente con masa cervical en nivel II derecho, de dos años de evolución, impresión diagnóstica clínica e imagenologíca de paraganglioma del vago, manejada en la E.S.E. Hospital Universitario del Caribe, con resección quirúrgica previa embolización.
