Background-Atrial fibrillation (AF) has a deleterious impact on health-related quality-of-life (HRQoL), but measuring this outcome is difficult. A comprehensive, validated, disease-specific questionnaire to measure the spectrum of QoL domains affected by AF and its treatment is not available. We developed and validated a 20-item questionnaire, Atrial Fibrillation Effect on QualiTy-of-life (AFEQT), in a 6-center, prospective, observational study. Methods and Results-Factor analyses established 4 conceptual domains (Symptoms, Daily Activities, TreatmentConcern, and Treatment Satisfaction) from which individual domain and global scores were calculated. Participants from 6 centers completed the AFEQT at baseline, at month 1, and at month 3. Psychometric analyses included internal consistency and known-group validity. Test-retest reliability was assessed by comparing 1-month changes in scores among those with no change in therapy. Effect size was used to assess responsiveness after intervention. Among 219 patients age 62Ϯ11.9 years, 94% completed the AFEQT at baseline and 3 months; 66% had paroxysmal, 24% persistent, 5% longstanding persistent, and 5% permanent AF. Internal consistency was Ͼ0.88 for all scales. Lower AFEQT scores were observed with increased AF severity, categorized as asymptomatic, mild, moderate and severe, respectively: 71.2Ϯ20.6, 71.3Ϯ19.2, 57.9Ϯ19.0, and 42.0Ϯ21.2. Intraclass correlations for Overall, Symptoms, Daily Activities, Treatment Concern, and Satisfaction scores were 0.8, 0.5, 0.8, 0.7, and 0.7, respectively. The relationship between digital rectal examination and the findings of faecal loading on abdominal radiograph was studied in elderly patients admitted to hospital. No significant association was found but there was a relationship between faecal loading of the recto-sigmoid, colonic and caecal areas of the radiograph. Soft faeces in the rectum were commoner in older patients and were associated with faecal loading in the caecal area. The digital rectal examination is useful for identifying anal and rectal lesions and faecal impaction but abdominal radiograph is required to assess faecal loading of the large bowel.
Injuries to the peroneal tendons are relatively common worldwide but tendon rupture without significant trauma is uncommon. Ankle mechanics can be seriously affected by disruption of one or both of the peroneal tendons although complete rupture can also remain asymptomatic. Accessory ossicles are sesamoid bones and are common findings in routine radiology of the foot and ankle. Although in the vast majority these "os" are normal variants of anatomy, they can lead to painful syndromes and suffer fractures and even undergo degenerative changes in response to overuse and trauma. Although similar syndromes have been discussed in the surgical literature, there is a lack of literature describing the use of modern imaging in the accurate diagnosis and its subsequent assistance towards appropriate management of os peroneum friction syndrome complicated by sesamoid fatigue syndrome. This article presents the plain film, sonographic and magnetic resonance imaging findings in a case of os peroneum friction syndrome complicated by a sesamoid fatigue fracture as well as reviewing the pertinent literature.
The efficacy of transvenous cardioversion and defibrillation for treating life threatening spontaneous ventricular arrhythmias was assessed in a study of 17 patients in a cardiac care unit. Eleven had ventricular tachycardia, five had ventricular fibrillation, and one had both. Transvenous cardioversion successfully terminated tachyarrhythmias on 42 separate occasions in ten patients. Stable electrode positions could not be achieved in two patients, recurrent late displacement occurred in one, and four patients had no further arrhythmias requiring cardioversion once the lead was placed. The energy levels required for successful cardioversion ranged from 0.05 J to 25 J for ventricular tachycardia and from 1 J to 25 J for ventricular fibrillation. The nine successful shocks of 1 J or less did not require sedation or general anaesthesia. High energy (25 J) endocardial shocks were unsuccessful in terminating arrhythmias in two patients, one with ventricular tachycardia and the other with both ventricular tachycardia and fibrillation. Minor unwanted effects of endocardial shocks occurred in five patients. These were acceleration of ventricular tachycardia in two patients and complications of pacing via the special lead in three others: failure of sensing occurred in all three and one patient also had a transient rise in pacing threshold. A postmortem examination in one patient who had received three unsuccessful high energy shocks revealed localised endocardial necrosis at the site of the distal electrode. Transvenous cardioversion offers advantages over external cardioversion but at present practical difficulties limit its application to patients with recurrent ventricular arrhythmias that cannot readily be controlled by conventional methods.
