A Duplicação Intestinal Cística é uma doença rara que está relacionada com uma anormalidade no desenvolvimento embrionário do intestino delgado, afetando principalmente o íleo e o jejuno. O objetivo desse relato é o de discutir a patologia, a fim de identificar suas apresentações, seus sintomas, diagnóstico e tratamento. Para isso, relata-se o caso de uma paciente, feminina, de 3 anos e 4 meses, que apresentava êmese e dor abdominal há 1 dia. A paciente foi ao tratamento com medicações sintomáticas e, após investigação com exame de ultrassonografia, foi submetida ao tratamento cirúrgico e posterior encaminhamento da peça cirúrgica para exame anatomopatológico, o qual identificou uma Duplicação Intestinal Cística no íleo. O intestino primitivo é formado durante a 4ª semana de gestação, podendo apresentar alguma anormalidade que desencadeie sua duplicação. O quadro clínico é mais comum em pacientes do sexo masculino, até 2 anos de idade. Os exames de imagem desenvolvem papéis fundamentais para o diagnóstico da patologia e o tratamento é feito via cirúrgica. Com isso, conclui-se que a paciente estudada não se enquadra nos padrões comuns de desenvolvimento da doença.
