What is known and objective: The aim of this study was to elucidate the effect of dexmedetomidine added to ropivacaine on the onset and duration of sensory and motor block and duration of analgesia of ultrasound-guided axillary brachial plexus block.Methods: Thirty-seven ASA physical status I-II patients with elective forearm and hand surgery under ultrasound-guided axillary brachial plexus block were randomly divided into 2 groups. Patients in ropivacaine-dexmedetomidine group (group RD, n = 19) received 15 mL of 0.5% ropivacaine with 100 μg (1 mL) dexmedetomidine, and patients in ropivacaine group (group R, n = 18) received 15 mL of 0.5% ropivacaine with 1 mL of normal saline. Onset time and duration of sensory and motor block and duration of analgesia were assessed.Results and discussion: Duration of sensory block (U-value = 35, P < .001), duration of motor block (P = .001) and duration of analgesia (P < .001) were extended in group RD compared to group R. Onset time of sensory block in group RD was significantly faster than in group R (U-value = 65.5, P = .001). Onset time of motor block showed no significant difference between the 2 groups (U-value = 116.5, P = .096). Adverse reactions were reported only in group RD (bradycardia in 2 and hypotension in 3 patients). What is new and conclusion:Our study indicated that dexmedetomidine 100 μg as adjuvant on ultrasound-guided axillary plexus block significantly prolonged the duration of sensory block and analgesia, as well as accelerated the time to onset of sensory block. These results should be weighed against the increased risks of motor block prolongation, transient bradycardia and hypotension and allow for attentive optimism, only if prolonged clinical trials provide a definitive answer. Context: Postural change during anaesthesia has a complex effect on systemic and cerebral circulations. Aim: The goal of the study was to evaluate the effects of desflurane and propofol on cerebral oxygenation during spinal surgery in the prone position. Settings and Design: A prospective randomized double-blinded trial. Methods and Materials: Fifty-two patients scheduled for spinal surgery were randomly allocated to propofol (n=25) and desflurane (n=27) groups. Anaesthetic agents were maintained to obtain a bispectral index of 50-55. SAP, DAP, HR, SPO2, ETCO2 and right and left rSO2 were assessed at seven-time points: supine position without oxygen administration (T1), supine position with oxygen administration (T2-baseline), intubation in the supine position (T3), just after prone positioning (T4), 10 minutes after prone positioning (T5), at the end of surgery in the prone position (T6) and at the end of anaesthesia in the supine position (T7). PCO2, PO2 and Hb partial were also recorded at T3 and T7. Results: Demographic data, pre-oxygenation hemodynamic variables and rSO2 were comparable between the groups. There was no significant difference between groups in SAP, DAP, HR, SPO2, and ETCO2 (p=0.095, p=0.061, p=0.357, p=0.088, p=0.328 respectively). PCO2, PO2 and Hb were not significant different between groups (p=0.542, p=0.394, p=0.768 respectively). rSO2 values were not significantly different between groups. In the propofol group, right rSO2 was significantly higher at T3 (p=0.017) and significantly lower at T5 (p=0,019) and at T6 (p=0,028) compared to baseline. Left rSO2 decreased significantly from baseline at T5 (p=0.026) in the propofol group. Left and right rSO2 in the desflurane group decreased significantly from baseline at T5 (p=0.0004 and p=0.0115). Conclusion: In the prone position, desflurane and propofol were associated with a significant decrease in rSO2 without differences between these anaesthetics.
Background and Aims Idiopathic transverse myelitis is a rare focal inflammatory disorder of the spinal cord causing motor, sensory, and autonomic dysfunction. Although it has significant anaesthetic implications, there are few reports of obstetric anaesthesia management of patients affected by this condition, with even fewer describing a neuraxial anaesthesia approach. Methods The authors describe the successful use of neuraxial anaesthesia in a 40-year-old female patient (69kg, 1.52m) at 39 weeks pregnancy with idiopathic transverse myelitis proposed for elective caesarean section. After multidisciplinary team discussion involving a neurologist, and considering the patient´s currently asymptomatic neurological status, a combined spinal-epidural anaesthesia was performed with an initial subarachnoid injection of 1.5 ml of 0,75% ropivacaine and 2.5 mg of sufentanil followed by placement of an epidural catheter for postoperative multimodal analgesia. ResultsThe procedure was uneventful and postoperative recovery was unremarkable with no reappearance of previous symptoms. The patient was discharged three days after the procedure. Two months after the anaesthesia no new neurological changes have been identified or reported when compared to the preoperative setting. Conclusions Neuraxial anaesthesia may be a safe approach for elective caesarean section in patients with idiopathic transverse myelitis under similar circumstances. However, neurological assessment before and after neuraxial block is essential, as well as obtaining informed consent.
Το σύστημα των ενδογενών κανναβινοειδών αποτελεί αντικείμενο έρευνας πολλών επιστημονικών ομάδων παγκοσμίως. Σκοπός της δικής μας έρευνας ήταν η μελέτη της δράσης των συνθετικών κανναβινοειδών στην αντιμετώπιση του μετεγχειρητικού πόνου. Υπάρχουν ενδείξεις ότι οι περιφερικοί υποδοχείς των κανναβινοειδών CB2 είναι καλύτερος στόχος για τη θεραπεία του πόνου, καθώς αποφεύγονται οι παρενέργειες του κεντρικού νευρικού συστήματος (ΚΝΣ), που προκύπτουν από την ενεργοποίηση των CB1 υποδοχέων των κανναβινοειδών. Γι’ αυτό στη μελέτη χορηγήθηκε ένας εκλεκτικός αγωνιστής των CB2 υποδοχέων, ο (R,S)-ΑΜ1241. Επιπλέον, μελετήσαμε την αποτελεσματικότητά της πολυπαραγοντικής αναλγησίας χορηγώντας το κανναβινοειδές σε συνδυασμό με την τραμαδόλη και της προληπτικής αναλγησίας χορηγώντας το κάθε αναλγητικό όχι μόνο μετεγχειρητικά, αλλά και προεγχειρητικά. Η σύνδεση των δύο αναλγητικών με ειδικούς υποδοχείς (φαρμακολογική ειδικότητα) μελετήθηκε με τη συγχορήγηση των αγωνιστών με τους αντίστοιχους ανταγωνιστές. Καθώς ο πόνος και το ανοσοποιητικό σύστημα επηρεάζουν το ένα το άλλο, χωρίς να είναι ξεκάθαρο, αν ο έλεγχος του πόνου ευνοεί τη μείωση της παραγωγής των προφλεγμονωδών κυτοκινών ή αντίστροφα, με τη μείωση των προφλεγμονωδών κυτοκινών προκαλείται λιγότερο σοβαρός πόνος, επιλέχθηκε να μελετηθεί η επίδραση των κανναβινοειδών και της τραμαδόλης στο ανοσοποιητικό σύστημα προσδιορίζοντας τα επίπεδα της ιντερλευκίνης 6 και της ιντερλευκίνης 2.Για την έρευνα χρησιμοποιήθηκαν 57 άρρενες επίμυες της φυλής Wistar, βάρους 250 έως 300g. Μετά την επίτευξη συνθηκών γενικής αναισθησίας, ο επίμυς τοποθετούνταν στη χειρουργική τράπεζα και εφαρμόζαμε το μοντέλο μετεγχειρητικού πόνου κατά Brennan. Χορηγήσαμε τον CB2 αγωνιστή (R,S)-AM1241 (1mg/kg, i.p.) και την τραμαδόλη (10mg/kg, i.p.) 30 λεπτά πριν (προεγχειρητικά) ή αμέσως μετά την επέμβαση (μετεγχειρητικά). Τα δύο αναλγητικά χορηγήθηκαν μόνα τους, αλλά και σε συνδυασμό. Επιπλέον, χορηγήθηκαν και σε συνδυασμό με τους ανταγωνιστές τους, τον SR144528, που είναι ανταγωνιστής των CB2 υποδοχέων, και τη ναλοξόνη, που είναι ανταγωνιστής των οπιοειδών. Η αξιολόγηση του πόνου έγινε με το ¨ Hot plate Analgesia meter¨. Οι χρονικές φάσεις μέτρησης ήταν τέσσερεις: η πρώτη (baseline) γινόταν μία ώρα πριν την έναρξη του πειράματος, και οι άλλες τρεις, 30, 60 και 120 λεπτά μετά την επέμβαση. Στο τέλος του πειράματος γινόταν ενδοκαρδιακή αιμοληψία και τα δείγματα αίματος χρησιμοποιήθηκαν για τον προσδιορισμό της IL-6 και της IL-2 με τη μέθοδο ELISA.Η καταγραφή και ο υπολογισμός των μετρήσεων έδειξε τα ακόλουθα: •ο (R,S)-AM1241 προκαλεί σημαντική αναλγησία, η οποία δεν επηρεάζεται από τον χρόνο χορήγησής του το και δεν παρουσιάζει σημαντικά καλύτερη αναλγησία από την τραμαδόλη•ο SR144528 αντιστρέφει πλήρως την αναλγησία, που προκαλεί ο (R,S)-AM1241•ο (R,S)-AM1241 προκαλεί σημαντική μείωση της IL-6, μόνο όταν χορηγείται προεγχειρητικά •ο (R,S)-AM1241 δεν επηρεάζει σημαντικά τη συγκέντρωση της IL-2•η τραμαδόλη προκαλεί σημαντική αναλγησία, η οποία δεν επηρεάζεται από τον χρόνο χορήγησής της•η ναλοξόνη αντιστρέφει εν μέρει την αναλγητική δράση της τραμαδόλης •η τραμαδόλη προκαλεί σημαντική μείωση της IL-6, μόνο όταν χορηγείται προεγχειρητικά •η τραμαδόλη δεν επηρεάζει σημαντικά τη συγκέντρωση της IL-2•η συγχορήγηση (R,S)-AM1241 και τραμαδόλης προκαλεί σημαντική ανακούφιση από τον μετεγχειρητικό πόνο, έχει σημαντικά καλύτερο αναλγητικό αποτέλεσμα συγκριτικά με τον (R,S)-AM1241 και δεν έχει σημαντικά καλύτερο αναλγητικό αποτέλεσμα συγκριτικά με την τραμαδόλη•η συγχορήγηση (R,S)-AM1241 και τραμαδόλης προκαλεί σημαντική μείωση της IL-6 •η συγχορήγηση (R,S)-AM1241 και τραμαδόλης δεν επηρεάζει σημαντικά τη συγκέντρωση της IL-2Το μέγεθος της μετεγχειρητικής αναλγησίας του κανναβινοειδούς (R,S)-AM1241 δεν είναι εντυπωσιακό, ούτε ανώτερο της ευρέως χορηγούμενης τραμαδόλης.
