Nasal vestibular furunculosis is a common bacterial skin infection among the general population mostly affecting adults and children. It is characterized by acute localized infection of hair follicle in the skin lining of the nasal vestibule caused by Staphylococcus aureus. Immunodeficiency also plays an important role in recurrent skin and soft tissue infections (SSTI) including Nasal furunculosis. Though, the lesion is small, it is extremely painful and tender. Treatment is mostly conservative which consists of warm compresses, analgesics to relieve pain, topical and systemic antibiotics directed against staphylococcus. With rapidly increasing resistance to antibacterial agents, management of these bacterial infections is becoming increasingly difficult. In Ayurveda, it can be correlated with Nāsāruṇaśikhā (Nasal furunculosis). Morphologically, it appears like pīḍikā (Furuncle) which is characterized by Rāgayuktaśotha (inflammation with reddening. Jalaukāvacaraṇa (Leech therapy) is one of the ancient and important parasurgical procedures described in Ayurveda for treatment of various diseases. In this case, a leech has been applied at the affected area. After leech treatment, throbbing pain was reduced in its intensity followed by gradual reduction in swelling and reddening in two days. This case suggests the utility of leech application in Nasal vestibular furunculosis.
Tinea Cruris is the disease of fungal infection of groin by dermatophyte fungi. It is commonly seen in men, and also called as Jock Itch as the other name. It is the condition which shows characteristics of disease as erythema, itching, burning sensation, cracked skin, skin rashes which worsen by exercise, skin discoloration and important symptom like rashes does not clears but increases due to antifungal creams. The management of the condition can be done in Ayurvedic perspective. According to Ayurveda all the classified disease of skin are considered as in Kushta Roga (skin disease), where the conditions of fungal infections are correlated with the Dadru Kshudra Kushta (Tinea). Due the clinical features of itch, redness, circular patches. The disease having exaggerated state of Pitta and Kapha Dosha, moreover Kapha. The treatment principle in such conditions is Shodhan Purificaton therapy along with Shaman (Pacificatory) therapy, as both therapies are considered best in Kushta Roga according to Acharyas. This case report is of the female patient suffered from itching over left thigh region along with redness, pimples, and round patches since 3 months, the patient finds the relief after following the Ayurvedic treatment as Shodhan Karma and Shaman Karma with lifestyle changes. The symptoms were diminished by Vaman Karma (emesis). She got 80-90% relief after Shodhan Karma as if one can consider the best treatment for tinea cruris.
Skin disease is one among Deerghkalinvyadhi (Chronic disease) and also one of the AsthaMahagada (Eight dreadful diseases). The disease psoriasis comes under the Kustha roga. Psoriasis is one of the most common dermatologic disorder and a chronic skin disorder of present day. Almost all the skin disease is explained under Kushta- Rogadhikara (skin disease) and classified as MahaKushta and Kshudra Kushta (Major and Minor skin disease). Acharya have described that all Kushta’s have Tri Dosha (three energies) involvement but the type of Kushta depends on the predominance of particular Doshas. The signs and symptoms of Eka-Kushta (psoriasis) in Ayurveda are similar to that of psoriasis explained in modern medicines. Psoriasis is marked by periodic flare-ups of sharply defined red patches, covered by a silvery, flaky surface. Aswedana (Absence of perspiration), Mahavastu (Present all over body), MastyaShakalopama (Look like a fish scale) is the feature mentioned by Acharyas for Eka-kushta. In Psoriasis relapsing nature is most common, which suggests that it needs long term treatment. In modern there is no such treatment for psoriasis. Here is the case of 52 yrs old male patient diagnosed as Plaque psoriasis undergone treatment of Shodhan (Purification) i.e. Vaman (Emesis) as well as Shaman Chikitsa (Palitative treatment) having marked improvement. The study showed that combination of Ayurvedic modalities gives significant result in lakshnas (Symptoms) like Aswedana, Mahavastu, MastyaShakalopama.
Psoriasis is an autoimmune papulo-squamous disorder of the skin, characterized by sharply defined erythemato- squamous lesions covered with silvery scaling. It is chronic and is well known for its course of remissions and exacerbations. The worldwide burden of psoriasis is 2%, and the prevalence of psoriasis in India ranges from 0.44% to 2.8%. There is no radical cure for psoriasis due to its autoimmune nature. It can be correlated with Eka Kushtha, one of the types of Kshudra Kushtha. In Ayurveda, Vamana and Virechan karma are indicated in Kushtha, which results in the elimination of vitiated Doshas. The effect of Shodhana Chikitsa can be increased if it is followed by shaman Chikitsa which include both Antaparimarjan and Bahiparimarjan Chikitsa. So here a 41 years old female patient had complaints of whitish silvery plaques, itching, burning sensation and scaling since one year was successfully treated with Vamana and Virechan Karma, followed by Shamana Chikitsa like Gandhaka Rasayana, Vetapalli oil, S-kin powder, Padachira ointment for external application and Avipattikar Churna- 3gm twice a day before food with water for 15 days. Conclusion: The Vamana Karma and Virechan followed by Shamana Chikitsa orally, and local application is highly effective in patients of Eka Kushtha (Chronic Plaque Psoriasis).
