A dissecção axilar representa hoje um dos tópicos mais controversos no tratamento do câncer de mama inicial. Paralelamente à necessidade do estadiamento axilar acurado está a elevada morbidade associada à linfadenectomia convencional. A biópsia de linfonodo sentinela está emergindo como um método de amostragem axilar seletiva, minimamente invasivo e altamente sensível para a identificação de metástases. Relatamos a experiência clínica inicial de nosso Serviço com a aplicação da técnica em 47 pacientes portadoras de câncer de mama, no período de dois anos. A linfocintilografia, utilizada como método de identificação do linfonodo sentinela, foi bem sucedida em 95,7% dos casos. A identificação do(s) linfonodo(s) de captação significativa foi possível em 91% das pacientes com linfocintilografia positiva. A média de LNS obtidos por paciente foi de 1,6. A análise histopatológica detalhada dos LNS foi obtida por cortes seriados. A imuno-histoquímica foi utilizada como método adicional em 12 casos. Houve apenas um caso de acometimento do linfonodo sentinela, no qual foi realizada dissecção axilar complementar. Neste caso, o linfonodo sentinela foi confirmado como único linfonodo axilar comprometido. Em nosso Serviço, a biópsia de linfonodo sentinela tornou-se o método padrão de abordagem axilar em pacientes portadoras de câncer de mama inicial com axila clinicamente negativa.
Neste artigo, os autores apresentam o caso de uma paciente portadora de um carcinoma secretório de mama, tipo raro de carcinoma mamário que ocorre com maior freqüência em pacientes jovens. No caso apresentado, uma paciente do sexo feminino com 38 anos de idade, apresentando nódulo mamário com características clínicas de benignidade, foi abordada inicialmente através de uma punção-biópsia por agulha fina (PAAF), cujo exame citológico foi compatível com uma lesão benigna. A paciente foi, então, submetida a uma tumorectomia simples e, ao exame anatomopatológico, foi constatado tratar-se de um carcinoma secretório da mama. Após o diagnóstico de malignidade, optou-se por uma conduta mais conservadora no caso, uma vez que trata-se de um tumor de bom prognóstico, e foi realizada apenas a ampliação das margens cirúrgicas da primeira ressecção. Os autores fazem, ainda, uma breve revisão da literatura, a qual justifica o tratamento conservador instituído.
Introduction: Sarcomas are a heterogeneous group of malignant neoplasms and represent less than 1% of neoplasms among adults; 80% of these cases originate in soft tissues. The liposarcoma corresponds to 20% of the subtypes of sarcoma in adults. Objectives: To present a rare case of liposarcoma of the breast. Methods: We studied the case of A.N.R, a 54-year old male patient, from Belo Horizonte, Minas Gerais, presenting a progressive growth nodule in the right breast. Mammography and breast ultrasound suggested lipoma. Core-needle biopsy of the nodule in the right breast was performed, and the anatomopathological was compatible with lipoma. Case report: Due to the progressive growth, the patient was submitted to nodule resection, with safety margins. Anatomopathological and immunohistochemical of the surgical piece were compatible with well-differentiated liposarcoma. The patient was followed-up by the oncologist and had a staging computed thoracic and abdominal tomography without changes, thus not indicative of adjuvant treatment. Discussion: Liposarcoma presents as a painless mass, of progressive growth, being more common in extremities and the retroperitoneum. When the well-differentiated morphological subgroup is located in the extremities and the torso, its excision is curative, and its metastatic potential is null in comparison to other locations; however, it is not recommended to underestimate the risk of local recurrence. Conclusion: even though the prevalence of benign lesions in soft tissues is higher than malignant lesions, it is important to consider a differential diagnosis of malignancy when the behavior and presentation of the tumor are atypical (deep location to the muscle fascia, larger than 5 centimeters, progressive growth), therefore leading to better surgical and therapeutic planning and to a more accurate treatment for the patient.
