Influencia del uso de Papacarie en el sellado marginal de obturaciones directasThe influence of Papacarie use on direct restorations marginal seal Resumen El propósito de la investigación fue, comparar el sellado marginal de obturaciones directas, realizadas con la técnica sándwich, después de la remoción de tejido cariado con el gel de papaina (Papacarie) y el método convencional rotatorio. Para lo cual se seleccionaron 20 premolares humanos recientemente extraídos, con lesiones cariosas que comprometían la superficie oclusal y proximal. La muestra fue dividida aleatoriamente en dos grupos: Grupo 1: se hizo la remoción de dentina cariada, mediante la técnica químico mecánica utilizando el gel Papacarie. Grupo 2: remoción de dentina cariada mediante la técnica convencional, con instrumentos rotatorios. Después de la remoción del tejido cariado por ambas técnicas se colocó una restauración directa utilizando la técnica sándwich: colocación de ionómero de vidrio modificado por resina, sistema adhesivo de un solo paso y resina compuesta microhibrida. Posteriormente, después de un proceso de termociclado, los especimenes fueron sometidos a filtración pasiva del colorante azul de metileno, por 3 días, seccionados en sentido mesiodistal a través de la restauración, y observados de acuerdo a grados de filtración del colorante en la interfase diente-obturación tanto en el margen oclusal como el gingival de la preparación, con la ayuda del microscopio estereoscopio. El análisis de los resultados indicaron que: El gel Papacarie permitió la remoción químico mecánica de tejido carioso en la preparación de cavidades para obturación. Sin embargo, influyó significativamente en una mayor frecuencia de filtración de colorante comprometiendo el sellado marginal de las obturaciones, comparado con el método de remoción convencional rotatorio. AbstractThe purpose of the investigation was, to compare the marginal seal of direct restorations, carried out with the sandwich technique, after the removal of decayed tissue by means of papaine gel (Papacarie) and the conventional rotational method. So, 20 recently extracted human premolars were selected, with decay lesions that committed the oclusal and proximal surface. The sample was randomly divided in two groups: Group 1: the removal of decayed dentine was made, by means of the chemo mechanical technique using the gel Papacarie. Group 2: the removal of decayed dentine was made by means of the conventional technique, using rotational instruments. After the removal of decayed tissues by both techniques a direct restoration was placed using the sandwich technique, placement of glass ionomer modified by resin, adhesive system of a single step and micro hybrid composite resin. Necesitamos que los tratamientos que empleemos para combatir al biofilm logren oxigenar el ecosistema periapical y fundamentalmente raspar y desprender las bacterias fuertemente adheridas. Recibido: 11-09-15 Aceptado: 11-12-15 105Tratamiento de una periodontitis apical crónica reagudizada con pasta 3 Mix-MP Sharpened chronic apical periodontitis treatment with pasta 3 Mix-MP ResumenEl propósito de este caso clínico es mostrar el uso como medicación intraconducto de la pasta de Hoshino (3Mix-MP). Paciente de sexo femenino acude a la clínica de posgrado con el diagnóstico clínico de necrosis pulpar con periodontitis apical crónica de la pieza 42. Se le realizó un tratamiento endodóntico en el año 2013; en el año 2014 regresa con lesión apical más severa y con un diagnóstico de periodontitis apical con fístula extraoral. La paciente no desea perder la pieza dentaria, por lo que se evalúa la posibilidad de tratamiento de segunda intención no quirúrgica utilizando la pasta de 3 MIX-MP como medicación intraconducto. Se realizaron controles por 3 meses con medicación intraconducto, clínicamente se evaluó la desaparición de la fístula observándose radiográficamente la disminución de la lesión periapical mejorando el pronóstico, por lo que se procede a obturar la pieza dentaria y se evalúo radiográficamente en 12 meses observándose reparación ósea. Procediéndose a restaurar protésicamente la pieza. Palabras clave: Periodontitis apical crónica, 3 Mix-MP, medicación intraconducto. AbstractThe purpose of this case is to show the use of intracanal medication Oshino paste (3Mix-MP). A patient who had a pulp necrosis with chronic apical periodontitis in the piece 42, underwent endodontic treatment in 2013, but in 2014 returns with a more severe apical lesion and the diagnosis of apical periodontitis, extra oral fistula; the patient did not want to lose the tooth so that the possibility of nonsurgical treatment of second intention using paste 3 MIX-MP as intracanal medication was evaluated. Controls for 3 months with intracanal medication were performed, the clinical disappearance of fistula was observed and periapical lesion decrease, improving the prognosis. So we proceed to seal the tooth and evaluated for 12 months, coming to observe bone repair, then the prosthetic restoration of the piece was done.
