Nontuberculous mycobacteria (NTM) are emergent pathogens whose importance in human health has been growing. After being regarded mainly as etiological agents of opportunist infections in HIV patients, they have also been recognized as etiological agents of several infections on immune-competent individuals and healthcare-associated infections. The environmental nature of NTM and their ability to assemble biofilms on different surfaces play a key role in their pathogenesis. Here, we review the clinical manifestations attributed to NTM giving particular importance to the role played by biofilm assembly.

Reflections on the use of COVID-19 vaccines in children and adolescentsReflexiones sobre el uso de vacunas contra la COVID-19 en niños y adolescentes

ResumoObjetivo: Apresentar conceitos atuais sobre diagnóstico e tratamento de meningites bacterianas considerando agentes etioló-gicos, fisiopatologia, opções antimicrobianas, antiinflamatórias e suportivas.Métodos: Revisão bibliográfica utilizando o banco de dados MEDLINE abrangendo artigos publicados nos últimos 10 anos. Um artigo clássico anteriormente publicado e capítulos de livros-texto de doenças infecciosas foram também incluídos.Resultados: A antibioticoterapia empírica inicial é efetuada com base na etiologia provável de acordo com a idade. Na análise microbiológica do líquido cefalorraquidiano, a bacterioscopia pelo gram pode revelar a bactéria em 50%-80% dos casos, e a cultura em aproximadamente 85%. Os testes que detectam antígenos auxiliam no diagnóstico, mas apresentam baixa sensibilidade. Na etiologia das meningites neonatais continua predominando a E. coli, o Streptococcus B e a Listeria monocytogenes. Após esse período, a incidência de meningites por Haemophilus influenzae b tem declinado significantemente desde a introdução das vacinas conjugadas, enquanto o S. pneumoniae e a N. meningitidis seguem sendo freqüentes patógenos. Atualmente, as cefalosporinas de 3 a geração, ceftriaxona ou cefotaxima, constituem-se na antibioticoterapia de eleição, sendo utilizadas em associação com ampicilina até os dois meses de idade e após como monoterapia. A dexametasona tem mostrado eficácia na redução da resposta inflamatória e das seqüelas, principalmente seqüelas auditivas. Na terapêutica de suporte, não há vantagens com a restrição hídrica.Conclusões: O diagnóstico precoce seguido de imediato início da terapêutica são fundamentais para o bom prognóstico. Os novos conhecimentos sobre fisiopatologia, o surgimento de novos antibióticos e a crescente resistência bacteriana têm provocado mudanças no tratamento. AbstractObjective: To present new concepts on diagnosis and treatment of bacterial meningitis regarding etiologic agents, pathophysiology and options about antimicrobial, antiinflammatory and supportive therapy.Methods: Bibliographic review from MEDLINE data including articles published during the last ten years. One classic article published before this period and chapters of textbooks on infectious diseases were also included.Results: Initial empirical antibiotic therapy is chosen according to probable etiologic agents for the age group. In the CSF microbiological analysis, the gram stains can reveal bacteria in 50% to 80% of the cases and the culture in nearly 85%. The tests for detection of bacterial antigens are useful for the diagnosis but they present low sensitivity. The most common agents during the neonatal period continue to be E. coli, Streptococcus B and L. monocytogenes. Beyond this period, the incidence of meningitis by Haemophilus influenzae b had a significant decrease after the introduction of conjugate vaccines.
