Português / Inglês www.scielo.br/reeusp RESUMO A inclusão das ações de saúde mental no contexto do Sistema Único de Saúde (SUS) contribuiu para a consolidação da Reforma Psiquiátrica Brasileira bem como demanda a reorientação da práti ca das equipes de saúde da família junto aos usuários com necessidades do campo da saúde mental. Este estudo tem por objeti vo identi fi car e analisar na produção cientí fi ca as ações realizadas pelos profi ssionais da equipe de saúde da família na atenção à saúde mental. Mediante análise sistemáti ca emergiram os seguintes temas: visita domiciliar ao doente mental e seus familiares; vínculo e acolhimento; encaminhamento; ofi cinas terapêuti cas. Concluiu-se que as ações de saúde mental desenvolvidas na atenção básica não apresentam uniformidade em sua execução e fi cam na dependên-cia do profi ssional ou da decisão políti ca do gestor indicando que os profi ssionais devem apropriar-se de novas práti cas para desenvolverem uma assitência integral e, portanto, há necessidade de investi mentos para qualifi cação dos profi ssionais.
ResumoOs Centros de Atenção Psicossocial (CAPS) são considerados dispositivos estratégicos para a mudança de modelo de atenção em saúde mental e apresentam proposições que vão ao encontro dos conceitos da Reabilitação Psicossocial. Buscou-se compreender na pesquisa realizada, como ocorrem as práticas de inclusão social voltadas para as pessoas com a experiência do sofrimento psíquico, no intuito de contribuir para a avaliação deste serviço, considerado um importante avanço no processo da Reforma Psiquiátrica Brasileira. O objeto de estudo são as representações sociais dos profissionais sobre práticas de inclusão social realizadas pelos serviços substitutivos em saúde mental. Utilizamos como conceitos norteadores da pesquisa a Reabilitação Psicossocial e a desinstitucionalização italiana. Os dados coletados, por meio de entrevistas semi-estruturadas, foram submetidos à análi-se do discurso, a partir da qual foi possível reconhecer algumas categorias empíricas, dentre as quais, sobre o modelo de atenção em saúde mental, tratada neste artigo. A análise das categorias empíricas foi realizada sob a ótica da representação social. Dentre os resultados obtidos, observamos que os discursos revelaram concepções representativas do modelo psiquiá-trico tradicional. Tais representações podem vir a ser superadas a partir de uma maior clareza do projeto institucional do CAPS e de discussões dentro das equipes interdisciplinares. Adriana Leão AbstractPsychosocial Attention Centers (CAPS) are considered strategic devices for changing the assistance model concerning mental health and present proposals that involve concepts of Psychosocial Rehabilitation. This research study aimed to understand how social inclusion practices for people who have experienced psychological suffering occur, and also to contribute to the evaluation of this service, as it is considered an important advance in the process of Brazilian Psychiatric Reform. The objects of the study are the professionals' social representations on social inclusion practices that are performed by the substitutive mental health services. The guiding concepts of this study are Psychosocial Rehabilitation and the Italian deinstitutionalization. The data, collected by means of semi-structured interviews, were submitted to discourse analysis, from which it was possible to recognize empirical categories. One of them is the mental health assistance model, which is studied in this article. The analysis of the categories was carried out according to the view of Social Representation. Among the results, it was possible to observe that the discourses revealed concepts that represent the traditional psychiatric assistance model. These representations can be overcome through a higher clarity of CAPS' institutional project and discussions within the interdisciplinary teams.
The objective of the study was to evaluate the influence of pregnancy on the level of adherence with antiretroviral (ARV) drugs, in a prospective cohort of 72 pregnant women and 79 non-pregnant women. Adherence was measured by pill counting and self-reporting. Women were deemed adherent if 95% or more of all ARV had been taken as prescribed, in two occasions. According to pill counting, 43.1 and 17.7% of pregnant and non-pregnant women, respectively, met the criteria of adherence (P = 0.001); in the postpartum, adherence declined to 20.6% (P = 0.002). In both groups, adherence rates by self-reporting were significantly higher as compared with pill counting (P = 0.001). In multivariate regression analysis, age >29 years (odds ratio [OR] 3.58, confidence interval [CI] 95% 0.10-0.75, P = 0.011), mean number of pills/day <6 (OR 2.53, CI 95% 1.07-6.01, P = 0.035), and being pregnant (OR 3.33, CI 95% 1.36-8.13, P = 0.008) were independently associated to greater adherence.
OBJETIVO: Esta pesquisa teve como objetivo conhecer os índices de especificidade e sensibilidade da versão adaptada à língua portuguesa da escala de Waterlow. MÉTODOS: Aplicação clínica da escala adaptada para avaliar prospectivamente 44 pacientes, por 15 dias consecutivos, em unidades clínicas de um hospital universitário de porte extra. RESULTADOS: Os pacientes que desenvolveram úlcera por pressão apresentaram escores maiores do que aqueles sem a lesão; 18,1±3,4 e 12,8±5,4 (p<0.001), respectivamente. Por intermédio da regressão logística, o método Stepwise demonstrou que os escores para tipo de pele e apetite foram significantes para predizer o desenvolvimento de úlcera por pressão. A análise da curva ROC mostrou que o escore 15 obteve os melhores índices de sensibilidade e especificidade, 87% e 76%, respectivamente. CONCLUSÃO: Os resultados demonstraram que a escala de Waterlow adaptada à língua portuguesa foi um instrumento preciso e eficaz para predizer o desenvolvimento de úlceras por pressão na população do estudo.
Classically, patients with giant cell arteritis (GCA) present with cranial ischemic manifestations that are directly related to vascular involvement. However, a variable proportion of GCA patients may present without obvious vascular manifestations. Since a high index of suspicion of this condition in individuals over the age of 50 years is needed to prevent the development of severe complications, we have studied the different patterns of disease presentation in a series of 240 patients with biopsy-proven GCA diagnosed at the single hospital for the well-defined population of Lugo, Spain, between January 1, 1981, and June 15, 2004. During the study period, 203 (86.4%) GCA patients presented with headache. Patients with headache were found to have an abnormal temporal artery on physical examination more commonly than the other GCA patients (79.8% versus 35.1%; p < 0.001). Compared with the other GCA patients, those who presented with polymyalgia rheumatica (PMR) were younger (73.4 +/- 6.3 versus 75.6 +/- 6.9 yr; p = 0.013) and had a longer delay to diagnosis (13.4 +/- 12.2 versus 8.3 +/- 10.0 wk; p = 0.013). One hundred thirty-one (54.6%) patients presented with severe ischemic manifestations. Abnormal temporal artery on physical examination (odds ratio, 2.25) and anemia at the time of disease diagnosis (odds ratio, 0.53) were found to be the best predictors for severe ischemic manifestations of GCA. Eighteen (7.5%) patients presented without overt ischemic manifestations of GCA. Patients younger than 70 years of age at the time of diagnosis had a longer delay to diagnosis and exhibited PMR more commonly than older patients. Our observations confirm the presence of different disease patterns of clinical presentation in GCA and emphasize the importance of an abnormal temporal artery on physical examination and anemia as factors that may predict the risk of severe ischemic complications related to GCA.
Among the aspects studied, results showed that the prevalence of preterm births was higher than expected for the adolescent mothers, however, with a satisfactory perinatal outcome.
