Las ortesis de la mano deben responder a objetivos terapéuticos precisos, como estabilizar una articulación dolorosa o inestable, corregir un déficit de amplitud o una deformación, suplir un déficit motor, para dar soporte y estabilidad a la articulación, entre otros. Debido a la importancia del equipamiento ortésico en pediatría, el objetivo de este paso a paso es describir el procedimiento para la confección de una férula de mano tipo cock up. La misma puede utilizarse, por ejemplo, en muñeca dolorosa y/o con actividad articular inflamatoria en niño/as con Artritis Idiopática Juvenil. Este equipamiento es un componente más en los protocolos de rehabilitación en pediatría, por ello es fundamental delimitar los objetivos terapéuticos de manera precisa y adaptarlos en el tiempo. Conocer los diferentes materiales termomoldeables y las técnicas de realización permitirán alcanzar de manera eficaz esos objetivos.
Objective: To assess the psychometric properties of the Pediatric Quality of Life Inventory™ (PedsQL™ 3.0), Neuromuscular Module, version in Spanish for Argentina, for children aged 2-18 years with neuromuscular disease. Population and methods: Observational, analytical, prospective validation study conducted in Hospital Garrahan between March 19 th , 2019 and March 9 th , 2020. The retest questionnaire was administered 10-15 days later to validate it among patients who reported a stable condition. Results: A total of 185 children and their parents participated. In terms of the questionnaire's feasibility, its content was easily understood by participants. Its reliability was acceptable, with an internal consistency of 0.82 among children and 0.87 among parents and a retest intraclass correlation coefficient of 0.70 among children and 0.82 among parents. In relation to the construct validity, 8 of the 11 hypotheses established (72.7 %) were confirmed. Conclusion: The questionnaire's psychometric properties were validated.
Objetivo: Describir las características del uso del asistente mecánico de la tos (AMT) en el domicilio en niños/as con enfermedades neuromusculares (ENM) en seguimiento en nuestro hospital. Materiales y método: Estudio observacional, descriptivo, transversal y retrospectivo. Se incluyeron todos los sujetos pediátricos con ENM con AMT domiciliario con un uso del mismo no menor a seis meses, entre el 1 de enero de 2018 y el 31 de abril de 2019. Resultados: Fueron incluidos 59 sujetos, con una media de edad en años de 8,84 (± 4,01). El diagnóstico más frecuente fue Atrofia Muscular Espinal (AME) tipo II en 32 casos (54,2%). La mediana de edad al inicio del uso del AMT fue de 4 años (RIQ 3-7). El principal motivo de su indicación fueron las intercurrencias respiratorias a repetición. El equipo más utilizado fue el Philips Respironics E70® , en modo automático, con presiones de 40 cmH2O positiva y negativa, y la interfase más frecuente, la máscara oronasal. No se reportaron complicaciones. Conclusión: El diagnóstico más prevalente de los sujetos pediátricos con uso domiciliario del AMT fue AME, y el mismo se indicó mayormente por cuadros respiratorios a repetición. Los principales usuarios fueron los padres/ madres, en modo automático y con presiones de ±40 cmH2O.
El asistente mecánico de la tos (AMT) es un dispositivo que aplica gradualmente presión positiva a las vías respiratorias, seguida por un cambio rápido a presión negativa, facilitando una tos eficaz. Está especialmente indicado en aquellos sujetos que, por el compromiso de la musculatura respiratoria, son incapaces de efectuar un pico flujo tosido (PFT) suficiente, imprescindible para eliminar adecuadamente las secreciones. Puede aplicarse a través de una máscara facial, una pipeta bucal o directamente en la cánula de traqueostomía. Estudios en niños/as con patología neuromuscular han demostrado que el dispositivo es seguro y bien tolerado, provoca un aumento del PFT, disminuye los tiempos de la terapia respiratoria, facilita la extubación y reduce la frecuencia de las exacerbaciones o eventos agudos recurrentes. Es cada vez más frecuente la indicación y utilización del AMT, por lo que es fundamental conocer sus características, indicaciones y programación para su aplicación en la población pediátrica.
